Mostrando los 5Quality control technician was adjusting the forks on a Gradall forklift & placed right hand between the back of the fork and the back of the attachment, resulting in a laceration to the right ring finger and small finger.
Employee was replacing a troughing roller. During the process the roller began sliding, the employee caught it and felt a pop in EE's lower abdomen.
While running to assist customer, EE felt a pain in EE's right knee. EE was treated at clinic and was prescribed work restrictions.
The employee went to the company clinic on 9/5/08 and was placed on restricted duty; R-knuckle required sutures. The employee cut R-hand (ring finger knuckle) on shop truck tail gate access cover plate while removing spare water truck tire.
