Employee was picking up a used liner off of the ground and turned and leaned forward to put it in the skid steer bucket and felt a sharp pain in chest.

Employee was descending off a conveyor while wearing fall protection and self-retracting lanyard. While descending the retractable whipped around striking the employee on the top of the head, causing a laceration to head that required four staples.

The Employee parked the loader to work on bucket leveler. The employee grabbed the arm to the leveler and it broke off and employee fell back onto a rock.

Employee was taking a guard off for maintenance and the guard twisted and slipped out of employee's one hand and pinched the employee's finger

Employee was getting in truck and closing door when the employee hit elbow on cab/door frame.

Employee was cleaning up a area of some 6x6 blocks and putting them in the bobcat bucket. EE lost one end of the block and pinched the pinky finger on the block underneath.

The employee was placing a new section of culvert pipe on a existing culvert pipe. While placing the new sleeve the edge of the new pipe cut the employee on tip of the hand on knuckles.

The employee was putting a return roller on the conveyor under impactor and felt a strain in hip/leg area.

Employee was taken off back plate of screen and it fell on hand/wrist area.

The employee was dislodging a rock that was hung in a conveyor and a piece came back and struck above left eye above safety glasses.

The employee was in process of replacing a hydraulic cylinder on crusher and the cylinder slid down and caught his pinky finger. The employee received two stiches and some glue.

The employee tossed an impact gun up to another employee on a platform and it came back down and he tried to catch it and cut his hand.

The employee was in the process of cleaning under the primary crusher using a John Deere 318 D skidsteer. While he was performing his work, a rock fell from above and struck him in his lower right leg.

The employee was assisting another employee to do a task, saw co-worker slip and fall, he tried to catch employee falling backwards off loader. The employee sustained a strained shoulder.

While changing crusher hammers the EE was setting down used blow bars on the ground and unhooking them from the boom truck. While standing on the ground and waiting for the next blow bar to be lowered the previous bar fell over hitting the EE's left shin above ankle.See Attached. EE notified company on 10/21/13 to see WC Dr.

EE was in the process of removing a tripod from the rear of his vehicle. While reaching in with his right arm to lift up,he felt a pop in his shoulder. At doctor visit the EE received a shot in the shoulder.

The employee was returning to his work truck and when he entered the cab of the truck, he felt pain in his lower left back.

The employee was in the process of changing a pressure relieve switch when it suddenly blew off. The reason why this occurred is because the employee failed to let off the hydraulic system resulting in the incident. The employee was hit in the right eye with hydraulic oil Sent to local ER for a complete eye washing and medication.

The EE was in the process of locking up site. He walked over to the pit area and had to close the sliding gate and lock it. While doing so the EE pinched his two fingers between two fixed pieces of metal causing a small cut on 2 fingers. The EE received stitches at the local hospital.

The impact crusher had stalled leaving rock lodged inside. Employee's were rolling boulder out of impactor when employee's hand was caught cutting his finger. The two employees were not communicating together and the rock slid onto the employee's hand causing the injury.

Employee was lifting drill steel to put into changer when his wrist popped. He had hurt his wrist the privious year while working at home. Employee went home that evening and came to work the next morning & said he experienced more pain. After a doctor's visit it was determined to be a sprain.

Employee was holding a cylinder with his left hand. While trying to set it in place, cylinder hung up and became dislodged. This caused the cut to his hand. The cut to his hand required 6 stitches.

Employee was lifting a cover weighing approx. 40lbx & his back was strained.

WHILE CHANGING SCREEN, EE STRAINED HIS NECK.

USING A LADDER TO PUT A BOLT IN AN IMPACTOR II, EMPLOYEE STRAINED HIS BACK.

EE WAS PULLING LARGE ELECTRICAL WIRES FOR THE NEW CONTROL HOUSE. HE WAS TRYING TO BEND THE WIRE AND HEARD A POP IN HIS RIGHT HAND.

Ok first was into he skid-steer cleaning up the plant, came into the parts trailer to eat lunch and hurt his back in the process of sitting down.

CHANGING GRATES IN HAMMERMILL AND SMASHED LEFT THUMB.

EMPLOYEE ALLEGES HE WAS HOOKING WELDER TO PICK UP. BENT DOWN TO LIFT UP HITCH AND FELT SHARP PAIN IN LOWER BACK.

WHILE REPLACING IMPACT CHAINS, EMPLOYEE'S LEFT THUMB WAS PINCHED BETWEEN CHAINS AND CHAIN HOLDER, PINCHING END OF THUMB.

EE WAS UNLOADING FILES AND LOADED TICKET BOXES FROM BED OF PICKUP TRUCK. FILES AND BOXES WERE BEING CARRIED TO STORAGE ROOM.

WHILE SHOVELING BASE ROCK-EE STRAINED THE MUSCLES IN HIS LOWER/MIDDLE BACK AREA

EE HELPING LIFT IMPACT CRUSHER HAMMERS ABOUT 200 EA WITH EE ONTO THE BED OF HIS PICK UP TRUCK, WHEN A SHARP PAIN HIT HIS BACK AND GROIN AREA.

HE WAS HELPING A FELLOW EMPLOYEE LIFT AN OLD ELECTRICAL BOX 3'X5' WEIGHT ABOUT 80#, WHEN HE FELT A CATCH IN HIS UPPER BACK. THE PAIN WAS IN HIS NECK AND RIGHT SHOULDER BLADE AREA.

EMPLOYEE WAS HELPING TO LOAD A SHOT FOR BLASTING. HE HAD BEEN CARRYING 50# SACKS OF ANFO ON HIS SHOULDER AND HIP TO THE BLAST SITE FROM DELIVERY TRUCK. INJURY OCCURRED 11-29-94 BUT EMPLOYEE DI D NOT REQUEST TO SEE DR. UNTIL 12-5-94.

EE & SEVERAL OTHER EES WERE POSITIONING LARGE STEEL BEAMS INTO PLACE FOR WELDING, THEY WERE FABRICATING A [?] AXLE CHASSIS FOR A ROCK CRUSHER & FEEDER. HE STATED THAT HIS BACK WAS HURTING AFTE R HIS WORK SHIFT.

AGGRAVATED INJURY FROM ACCIDENT DATED 6-24-94, WHILE CLIMBING DOWN LADDER.

EMPLOYEE WAS ATTEMPTING TO R4EMOVE SPILLAGE (LOOSE ROCK) FROM CATWALK WITH THE INSTEP OF HIS RIGHT FOOT. (KICKING MATERIAL FROM CATWALK). WHEN HIS FOOT CONTACTED THE LOOSE ROCK HIS RIGHT KNE E PIPPED ON THE INSIDE AND PAIN CAME TO HIS RIGHT KNEE. SHOULD HAVE USED A SHOVEL.

EMPLOYEE WAS CLEANING TAIL SECTION ON THE PRIMARY CONVEYOR WITH A SHOVEL. HE STATED TO THE SUPERINTENDED THAT HIS BACK STARTED TO HURT WHILE SHOVELING THE DAM AND WET MATERIAL FROM THE AREA.

THE SUPERINTENDENT AT THIS MINE STATED THAT HE ATTEMPTED TO LIFT AN I BEAM & HURT HIS BACK

DRILLER HAD JUST FINISHED DRILLING LENGHT OF 1 STEEL WHILE TAKING NEW STEEL OUT OF RACK, IT STARTED TO FALL, HE REACHED FOR IT AND SAYS HE PULLED SOMETHING.
