Employee was in the process of removing the cutting edge bolts from the bottom of a John Deere 324G skid steer, with leather Tillman gloves using a Milwaukee half inch impact. While removing the second bolt it spun, and caused a burr on the bolt to cut EE's ring and middle fingers on the bottom of EE's right hand. Employee was taken to clinic and received 14 stitches.

Employee was holding a bearing still while another employee was hammering a chisel to remove race from the housing. During one of the hammer strikes to the chisel something broke off and the object went through sweatshirt of the employee holding the bearing and into EE's left forearm. Employee was taken to clinic where a piece of metal was removed; employee required three stitches.
Employee was helping with maintenance on the Norberg cone crusher. EE went to climb down the step ladder, the ladder shifted while still holding onto the cone with EE's left hand. Employee felt a pop in EE's shoulder. Employee had surgery on 02/19/2026 and was given restrictions making this reportable.
Employee straightened after stemming a hole and felt a sharp pain in EE's right pectoral muscle that didn't go away for the rest of the day. Commented on it to 2 other EE's. Dull ache with certain movements triggering more intense pain. Worsened over the weekend.
Employee was using carpenters knife to cut grease line when the knife slipped off the hose and cut left thumb knuckle. Did not require stitches, was placed in a splint to immobilize due to the location of laceration.
Technician was laying under machine to break back bead with a hydraulic ram. While utilizing ram, back bead broke then hydraulic ram fell and struck technician in the face resulting in 4 stitches to the upper lip.
Employee was adjusting the mirror on the dump truck when the employee's foot slipped off the step and EE struck right elbow on the storage box attached to the inside door panel. Incident became reportable on 9/10/2024 when EE went to the doctor and was issued prescription medications.
Employee was hooking up alligator clips to the battery to fuel up the loader when employee's back foot slipped on the rubber mat that was laid down on the ground. The employee while falling down put arm out to catch self and heard a pop in shoulder. This resulted in a fracture in the shoulder socket.
Employee was using a wire wheel on a rusted piece of metal when employee felt something go into eye, underneath safety glasses. Employee flushed eye but still felt something in there. Went to clinic, where they removed a piece of metal and issued prescription antibiotics.
Employee exiting equipment at end of day and EE missed step getting out and landed on side of left foot resulting in a sprain
Employee was picking up a used liner off of the ground and turned and leaned forward to put it in the skid steer bucket and felt a sharp pain in chest.
Employee was descending off a conveyor while wearing fall protection and self-retracting lanyard. While descending the retractable whipped around striking the employee on the top of the head, causing a laceration to head that required four staples.
Employee was holding pin with a pipe wrench. While attempting to hit pin with a hammer, the employee missed and hit pinky finger causing fracture to end of finger and four stitches.
Customer after getting loaded with rock, pulled off to the side at the tarping area to unroll the tarp. The customer had problems with the tarp and climbed on the side of the truck to get the tarp unstuck and fell to ground.
During the injury the employee was performing maintenance on a wash screen. The employee slipped and fell forward smashing EE's thumb on a bolt that holds the spray bar on the wash screen.
The Employee parked the loader to work on bucket leveler. The employee grabbed the arm to the leveler and it broke off and employee fell back onto a rock.
Working on stripping wires - it was cold and snowy making it difficult to strip the wires. Cut deep into right thumb. Had to have surgery to repair the tendon in that thumb and had to get stitches. Wore hand brace.
Employee was cutting flashing from under a rock box with a folding carpenters knife and the knife folded on employee and cut palm
Employee got into dump truck to check fire extinguisher and stepped out of truck back on ground and felt lower back pinch. About 30 min. later EE shoveled off some build up on a screening unit and felt back get worse.
Employee was adjusting impact crusher and one employee was raising and lowering cylinder and other employee was turning nut. When the one employee turned the nut the other employee pushed lever to lower cylinder, the other employee stuck hand back in quickly to re adjust and smashed two finger tips
Employee was taking a guard off for maintenance and the guard twisted and slipped out of employee's one hand and pinched the employee's finger
Employee walked up a bank to spray paint boundary line and turned to walked back down and feet came out from underneath EE and jarred EE's neck.
Employee was getting in truck and closing door when the employee hit elbow on cab/door frame.
Employee was taking off a bearing housing with a hammer and a piece broke off and hit another employee a distance away, who was working on something else.
Employee was cleaning up a area of some 6x6 blocks and putting them in the bobcat bucket. EE lost one end of the block and pinched the pinky finger on the block underneath.
The employee was using a 8" hand held grinder to square up the excavator bucket pin boss face. When grinding employee got a piece of steel in left eye. Employee received medical treatment to have it removed.
While climbing down off the machine the employee missed the last step of a two step ladder and turn and hit knee on the out rigger of the truck.
The employee was placing a new section of culvert pipe on a existing culvert pipe. While placing the new sleeve the edge of the new pipe cut the employee on tip of the hand on knuckles.

After awhile it did not get better so we had a MRI down which concluded he had a tear in the meniscus on 6/10/15 which will required surgery.
The employee was in the process of cleaning under the primary crusher using a John Deere 318 D skidsteer. While he was performing his work, a rock fell from above and struck him in his lower right leg.

While changing crusher hammers the EE was setting down used blow bars on the ground and unhooking them from the boom truck. While standing on the ground and waiting for the next blow bar to be lowered the previous bar fell over hitting the EE's left shin above ankle.See Attached. EE notified company on 10/21/13 to see WC Dr.
EE was in the process of removing a tripod from the rear of his vehicle. While reaching in with his right arm to lift up,he felt a pop in his shoulder. At doctor visit the EE received a shot in the shoulder.
The EE was in the process of removing wedges from an impact crusher. He was using two sledge hammers to work them loose. While he was hitting them back and fourth to loosen the EE missed the wedge and struck his hand with head of sledge hammer. No broken bones x-rays negative, RX for pain.
The employee was returning to his work truck and when he entered the cab of the truck, he felt pain in his lower left back.
The employee was in the process of changing a pressure relieve switch when it suddenly blew off. The reason why this occurred is because the employee failed to let off the hydraulic system resulting in the incident. The employee was hit in the right eye with hydraulic oil Sent to local ER for a complete eye washing and medication.
The EE was in the process of locking up site. He walked over to the pit area and had to close the sliding gate and lock it. While doing so the EE pinched his two fingers between two fixed pieces of metal causing a small cut on 2 fingers. The EE received stitches at the local hospital.
EE had driven wedges between bucket and cutting edge while torching off bolts. The cutting edge sprang forward and caught the ee in the knee. This caused bruising and swelling in his knee later that evening.
The impact crusher had stalled leaving rock lodged inside. Employee's were rolling boulder out of impactor when employee's hand was caught cutting his finger. The two employees were not communicating together and the rock slid onto the employee's hand causing the injury.
Employee was lifting drill steel to put into changer when his wrist popped. He had hurt his wrist the privious year while working at home. Employee went home that evening and came to work the next morning & said he experienced more pain. After a doctor's visit it was determined to be a sprain.
