Employee was cleaning out a plugged chute. When the material shifted, a 6 to 8 inch rock fell from the chute above, hitting his finger. He received a laceration which required stitches and the finger was broken.
Employee was unlocking a 5th wheel on a lowboy tractor and trailer. It was stuck and had to pulled hard to unlock. The employee injured the rotator cuff in his left shoulder that will require surgery to repair. Surgery is scheduled for 6-24-09.
Employee was moving a rolled up conveyor belt with a chain and a front end loader. During transport, the belt needed to be repositioned. While manually handling the belt the employee sustained a ruptured bicep tendon. The injury required surgery which was repaired on 5/20/09.
A new roll of belting was elevated off the ground with the boom of a service truck. There was a drill steel put through the center of the belting. The load shifted and the employee grabbed one end of the drill steel. His finger was mashed between the drill steel and the bumper of the service truck. He received stitches and broke the tip of his finger.
Employee was removing rock from the side door of the crusher. A loose rock rolled down and smashed his finger against the side door. He received a laceration that required three stitches and also broke the tip of his finger.
Employee was climbing the ladder of the haul truck. As he lifted his leg to climb, he felt pain in his back.
Employee was preparing to mount a cutting edge on a FEL. The cutting edges were shipped on a palet stacked three high. Employee was sliding the top cutting edge off of the stack. His finger was caught between the two cutting edges. The cutting edges weigh approx. 350 lbs. apiece. Employee sustained a cut that required stitched and a broken finger. Both were to the same finger.
REPAIRS WERE BEING MADE TO THE RIGHT FRONT WHEEL. THE WHEEL COVER DROPPED OFF AND ROLLED ON THE GROUND TOWARDS INJURED EE. AS THE COVER ROLLED BETWEEN HIS LEGS HE TRIED TO KEEP IT FROM HITTING HIM. IN DOING SO HIS FINGER ON RIGHT HAND GOT CAUGHT IN THE COVER AND BROKE HIS FINGER.
3 EE'S WERE REPLACEING THE BELLYPAN ON A DUMP TRUCK. AFTER THE PAN WAS LOCKED IN PLACE, INJURED EE DROVE AWAY IN THE TRUCK. 30-45 MINUTES LATER, INJURED TOLD HIS FOREMAN THAT HIS BACK WAS HURTING. HE HAD NOT MENTIONED THIS TO THE OTHER TWO CO-WORKERS.
EMPLOYEE WAS TURNING PULLEYS ON IMPACTOR TO PUT ON A NEW BELT. HIS GLOVE GOT CAUGHT BY THE BELT AND IT PULLED HIS HAND INTO PULLEY WHEN IT TURNED.
EMPLOYEE WAS WORKING ON THE TRACKS OF A CAT EXCAVATOR. HE BENT OVER TO PICK UP A 10 LB. PAD FOR THE TRACK AND HE FELT PAIN BETWEEN THE SHOULDERS.
