Employee was assisting with the movement of machinery, with hook, clevis and chain attached to a track hoe. Clevis did not fit, so they were changing it out with another hook and clevis located in the track hoe. Track hoe operator tossed the hook from the excavator to the ground. EE tossed the clevis moments later, but employee reached for hook, and then clevis hit EE's hand.

EE WAS OPERATING A PROFILING-SHAPING MACHINE. STRIPS OF MARBLE ARE LOADED AND UNLOADED N THE FLOOD AND ARE PICKED UP MANUALLY TO LOAD.
EE WAS MIXING A CUP OF GLUE/PENETRANT FOR MARBLE. HE SET THE CUP DOWN, THE P[ICKED IT UP & DROPPED THE CUP. HE TRIED TO REACH FOR IT, IT FELL TO THE FLOOR, THE LIQUID SPLASHED UP & INTO THE LE FT EYE.
