EE was assisting with connecting air compressor to p/u truck. EE attempted to move the tongue of the air compressor to the hitch of the p/u truck when EE felt a snap in their left shoulder.

Employee picked up a flex hose to move it. During the movement something flew into EE' eyes. Later during the shift EE' eyes became itchy and irritated. The employee was wearing safety glasses at the time of the incident. The employee was taken to the Emergency Room where EE was treated for a bilateral abrasion of the cornea. EE was given an antibiotic to treat the scratches.

Miner was aligning steel frame for air door installation. Miner asked for a spud wrench to align the top and bottom steel frames. During this time the steel shifted and the tip on the Miners left index finger was pinched in the hole resulting in a crushing injury with lacerations.

Employee installed Leach Tank #3 lid section to fit into place. The lid was stuck causing employee to push lid with right leg causing overexertion of their knee. Employee did not notice any pain at that point in time. They reported the incident on 7-10-2023. Doctor recommended Physical therapy on 07/18/2023 to have done. Physical therapy will start on 7-21-2023 @ 4:00 PM.

Employee was breaking up a hardened caustic chunk so that it would fit into a hopper when EE smashed finger against a solid object. This resulted in a fracture to the tip of the pinky finger on the right hand.

The employees were working on a 150 lbs. head tank frame that was sitting on the walkway grating. As they went to move the frame, EE #1 noticed welding rods and attempted to move them as EE #2 moved the frame and pinched EE #1's middle finger, right hand, between the frame and the grating. As a result the fleshy end of the finger was cut requiring stitches.

Miner was unloading materials onto work deck in shaft from conveyance. Two beams were lying on the deck, miner was straddling the beams. When EE unhooked the rigging, the beams tipped and hit the miners foot.

Miner was lifting half of a 150 lb. deck grate and suffered a hernia on right side

The employee was using a sand blaster when the breather hose got stuck. The employee pulled on the hose slipped on the grit under his feet and lost his balance and lost control of the nozzle. Sand blasting grit struck the employee above the right elbow causing a 4 to 5 inch hole in is bicep.

The employee was lifting one end of a 250 pound cutting edge when he felt a sharp pain in the lower right side of his stomach. The pain went away but a report was filled out. He was later diagnosed with a hernia when the pain returned. On 10/4 he was put on light duty. On 10/18 he had surgery returning to light duty on 10/21.

Miner was drilling a hole in the rib and after completion of the hole leaned the drill against the rib and had his hand in a pinch point between the dog nut of the steel and the chuck of the drill.

While setting steel, a 10' piece of 3/8" angle iron fell three feet vertically onto the miners right foot. The steel punctured the miners right boot and also the skin on his foot. His status is Full Restriction, stay off feet and keep foot elevated. Possible small fracture and infection.

2 Miners were lowering a jackleg drill down from a raise, the drill slipped from their grasp and 1 miners hand was caught between the drill and a piece of wooden lagging.

Previous shift overflowed a tank with heated solution and soda ash. The crew then removed a set of grates in the sump area to pump the solutions back into the circuit. The crew then changed at shift change and did not barricade the sump area, nor told the oncoming crew about the removed grating.

Working in the # 2 manway compartments at approximately the 770 foot steel set, he was removing an old piece of lagging. While attempting to remove the wood cribbing from the bottom the top started to tip towards him. He put his hand up to stop it from falling.

The injured employee was working in a warehouse stocking shelves and organizing floor area under another employee who was stocking selves. He was stocking the selves dropped a 4" vic coupling striking the injured on the head.

EE was pulling a 40ft piece of plastic pipe when he felt pain in his left knee. He had previously injured this knee on May 13,08 at the topaz mine. He was sent for an assessment and has been refered to a specialist as there was no sign of injury. NOTE: this is a reoccurence of the previous injury, we have the worker on restricted duty until further assessment.

Employee was holding a sample to begin testing when he wrenched back.

Hit in the face with a pipe. Glancing blow while loosing a hugger.

3 inch plank laying in walkway. Injured moved the plank as it was a tripping hazard. In moving the plank she got a splinter in her finger.

While supporting a 10" drisco pipe for welding, a second employee pushed the pipe which pinched and cut the right index finger finger of the employee. The injured employee received several stitches.

Employee was tapping beaker to check for weakness. Beaker shattered and lacerated left hand with glass shards.

Employee was dismantling guard when it fell onto hand lacerating rt. big finger. He was going to replace the bolts on the centrifuge pump.

Employee was emptying drums of material onto conveyor for processing. The contents had hardened and had to be removed by hand. While removing drum from platform, the employee's hand was pinned between the drum and doorway, breaking his left pinky finger.

Man was sitting down, preparing samples for analysis dropped pipe and filled with acid (hxs04) onto lap, material spilled onto leg and ran onto leg & buttock area burning employee.

Dismantling an abandon sulfuric acid line. All valves were closed. Hydrogen gas build up in pipe caused a sudden release when the line was being disconnected. Sulfuric Acid material was blown onto the neck of the affected worker.

Subject was cleaning filter plates on filter press when he pinched his pinky finger between the plate and filter railings.

EE was loading a barrel into crushing machine, got left long finger wedged between barrel and frame, smashed and ripped finger.

Employee received caustic burns to both arms when material splashed up in the sleeves of his wet gear. PPE was not wore correctly. Employee then did not flush the area until after an hour had gone by. Then the employee washed the area for five minutes and then used a scrub brush on the affected areas. This caused the blisters that had arisen to rupture.

THE EE SWUNG HIS LEG OVER A HANDRAIL TO ACCESS A LADDER. THE KNEE NOT SUPPORTING THE WEIGHT POPPED.

EMPLOYEE WAS ATTEMPTING TO OPEN A LOCK TO DRESS FOR WORK. THE LOCK JAMMED, WHEN HE FORCED IT HECUT HIS FINGER.

EE WAS MOVING A 50 LBS SAND BLASTER WHEN IT CAUGHT A HOSE, SLIPPED FROM HIS HAND, THEN STRUCK HIS LEG.

SOMEONE HAD TAPED A UTILITY KNIFE TO A POLE FOR A PREVIOUS JOB AND NEVER REMOVED IT. EE REACHED OUT TO MOVE THE POLE, DIDN'T SEE THE KNIFE AND CUT THREE FINGERS.

EMPLOYEE WAS USING A 2 1/2 INCH FIRE HOSE TO WASH OUT THE BOTTOM OF A TANK. WHEN ADJUSTING THE WATER FLOW, HE WAS HIT BY A PRESSUR SURGE, LOST HIS BALANCE, TWISTED HIS KNEE AND FELL.

ORGANIC WAS CONTACTED WITH CALCIUM CARBONATE IN 150ML VIAL INPREPARTAION FOR ANALYSIS. THE HIGH ACID IN THE ORGANIC REACTED WITH THE CARBONATE BUILDING PRESSURE WHICH EJECTED THE MATERIAL FRO M THE VIAL & SOME OF THE MATERIAL ENTRED EE'S EYE

WHILE WORKING ON THE TROMMEL, EE LIFTED A PIECE OF ANGLE IRON. WHEN HE TWISTED TO POSITION IT FOR WELDING, HE INJURED HIS BACK.

WHILE POSITIONING A CARGO BOX FOR LOADING ON A TRAILER, HE WAS PULLING A TAG LINE WHEN HE HURT HIS BACK.

LIFTING A BLOCK OF CONCRETE, APPROX WEIGHT 120 LBS, TO PLACE IN A TRASH TRUCK. THIS AGGRAVATED AN INJURY OF 9/12/98 TO HIS BACK WHEN HIS FOOT SLIPPED WHILE MOVING DRUMS OF PROCESS MATERIAL WIT H A HAND TRUCK. HIS INJURY IS ACUTE SCIATICA. THIS REPORT DEALS WITH CONDITIONS OF THE 10/7/98 INJURY.

HELPING LIFT A 300 LB PIECE OF STEEL OUT OF THE BALL MILL.

EMPLOYEE HAD A BAKER'S CYST CONDITION THAT HAD BEEN A PROBLEM PRIOR TO EMPLOYMENT. HE HAD BEEN SEEN BY A DR. TWO OR THREE TIMES. WHEN THIS INCIDENT OCCURRED HIS PERSONAL DR. THOUGHT THAT IT WA S A CONTINUATION OF THIS CONDITION. AFTER AN MRI& FURTHER EXAMINATION IT WAS FOUND THAT PART OF THE PROBLEM WAS A "MINICUS TEAR". THIS WAS DISCOIN SURGERY. THIS IS WHY THIS REPORT IS LATE.

VICTIM WAS HELPING MECHANICS MOVE SOME ANGLE IRON. WHEN HE TRIED TO ROLL IT, THE LOAD SHIFTED, MASHING HIS FINGER. HAD TO HAVE SEVERAL STITCHES.

OPENING A STACK TAILGATE ON A PICKUP TRUCK. WHEN IT FINALLY OPENED IT JARRED HIS THUMB STRAIGHT BACK. HE SPRAINED HIS THUMB.

WHILE PUTTING A CAUSTIC LINE, EE KNELT DOWN UNKOWINGLY, INTO SOME SOLUTION WHICH WAS ONTOP OF THE TANK. THE SOLUTIO (CAUSTIC) SOAKED THROUGH HIS CLOTHING, BURNING HIS SKIN ON HIS SHIN.

EMPLOYEE WAS REMOVING TARP FROM TRAILOR BOX, WHEN CHAIN THAT WAS FASTNED TO THE TOP BOX BROKE, CAUSING HIM TO TWIST AND FALL BETWEEN THE TRAILER AND THE SCAFFOLDING. HE TWISTED HIS BACK. THERE WAS NOT MUCH PAIN AT THE TIME,BUT STIFFENED UP THE NEXT DAY.
