Employees finger was caught between a sliding section of angle iron and a fixed item on the plant. This resulted in a 1" cut and a fractured finger tip.

Miner was shoveling wet material at the entrance to the Lower Mill.

Employee cut a box open with a utility knife and placed the knife on top of the box. When the employee pulled the box open with EE's left hand EE's left index finger make contact with the blade of the utility knife resulting in a cut to EE's left index finger.

Employee was lifting a barrel of grease into position. While doing so their foot slipped on truck surface causing body to reflex and drop the barrel into position.

Employee was attempting to clear drill steel of clogged material when compressed air was released from the charged drill steel. This resulted in a cut to the bottom of the miner's thumb/palm and pelting of the top of the miner's right hand with dust/sand/stone chips.

The employee picked up a roller when EE lost EE's grip. It slipped out of EE's hands, and the shaft's keyway cut the ring finger on EE's right hand. The worker received four stitches to close the wound.

Employee was replacing brushes on sweeper when EE felt a sharp pain in EE's right bicep. Assumed to be a slight tear.

Employee went to pull belt out far enough from storage location to grab with fork lift. When initiating pulling, felt tweak in back.

While shoveling, employee slipped and fell, fracturing right lunate bone (wrist bone)

Putting beam into #2 screen. Sliding the third beam into place, it went a little to far. The bolt holes didnt line up. Went to pull the beam back into place to align the bolt holes and heard/felt a pop.

Removing wear plate. Wear plate bounced off wall of chute and contacted employee leg

Manually pulling on V belts for crusher installation, fingers of employee got pinched between the belt and the sheave.

Employee was changing a M1 gear. It was raining and employee's safety glasses fogged up multiple times. Employee hung glasses on shirt and tapped the gear in place and felt something in eye.

Employee was checking the main conveyor over the portable screen and as EE was climbing down, EE hyper-extended right shoulder backwards and felt a sharp pain. First aid was administered. Employee went to doctor on 10/13/16 and received two prescriptions.

Employee was changing a large acetylene tank on the welding cart. When ee picked up the large tank, ee felt pain in lower back and couldn't feel left leg.

While pulling door closed with left hand, employee placed his right hand on the door jamb. Employee caught his right index finger in the door by the window while seated inside the truck.

Employee stepped down off loader ladder and felt pain in lower back. Employee examined at medical facility on Monday, May 13, administered a prescription and released to light duty work.

Employee was shoveling under conveyor near tail-pulley when he felt his right knee pop.

Employee slipped while opening railroad car door and got his finger caught in a pinch point resulting in a laceration on his right index finger that required stitches.

Employee was removing new style motor base from it's pedestal when stored energy from the torsion bar broke free striking the employee's right index finger.

While working in a transformer vault and installing a 3-inch high voltage electrical cable, he started to adjust the cable supports. The cable slid back into the 5" conduit pulling his hand into it. Finger was sore and ice applied. An hour later, he went to squeeze a hook and felt a pop in his finger.

Employee was removing the lower tramp cylinder mounting pin with a drift pin and small sledge hammer while supported by a crane. When pin removed, the tramp cylinder shifted pinching the employee's fingers between drift pin and crusher base stud causing fractures and lacerations.

The employee was loading steel from a steel storage rack onto a truck-mounted overhead rack. A 23' by 1 1/2" angle iron fell from the truck-mounted rack striking him on head and neck. He was taken to William W. Backus Hospital for treatment.

While installing return idler, the idler fell out of the bracket striking his left middle finger.

While removing a bolt used to attach the cutting edge to a loader bucket, employee thought bolt was dropping out of hole. He put his hand under to catch it, and the spinning bolt cut his fingers.

Employee was positioning tire with axle of truck in preparation of mounting it when the tire began to fall back towards him. The employee was unable to support the falling tire when it began to fall. The tire knocked the employee to the ground with the tire resting on his left leg/knee.

Employee had finished cutting EMT (Electrode Magnetic Tubing) with a handheld bandsaw. Upon completion of the cut, employee rested bandsaw on tripod designed to hold saw when not in use. Employee bumped tripod causing the saw to fall. The employee attempted to catch saw and cut pinky finger on exposed blade.

Employee claims left and right wrists were injured from connecting and disconnecting trains; climbing on and off ralroad cars and locomotives and through throwing track switches.

employee was pulling on top screen of deck of AC-2 when he felt a pull in his lower back.

Employee was attempting to dislodge a broken RR Car door spring. When the spring dislodged it broke and a piece flew out striking the employee in the left thumb.

Claimant was on a vehicle when he twisted and hurt his back.

Top drill bar not in carosel properly when it fell onto employee's shoulder

EMPLOYEE STATES THAT BACK BECAME SORE AFTER MIXING PAINT TOGETHER

The employee had stopped the fuel truck that he was driving on a flat, clear, dry area of the quarry floor. He dismounted the cab of the truck and when his right foot hit the ground, as he exited the vehicle, he experienced pain in his lower back.

While removing the conveyor support legs from the receivers. The legs moved upward pinching employees finger between the support and conveyor frame causing

Employee was attempting to drive shaft out of sprocket/shaft assembly. To do this it was required that he lift this assembly onto two railroad ties to allow room for shaft to be driven out of sproket when hit from above. When employee set sprocket onto tie #2 he mis-judged the location of the tie and caught thumb between sprocket and railroad tie.

While lifting a RR Tie employee substained an abdomen hernia.

Employee felt pain in Right Wrist when removing steel door on bucket.

While cleaning out hopper employee experienced pain in his lower back.

while walking up ladder with a shieve, handed off and gave to another employee felt strain, pain in lower back

While cleaning out rail car, door shut swiftly on fingers & cut top middle finger.

unusual back pain while testing material (lifting heavy rock) has been diagnosed with back strain/sprain

employee picked up a heavy piece of steel that was too heavy for him to carry alone and he felt a pop in his lower left side of his back

While reaching to uncoil cable felt a pop in the back

Employee was assisting electrician at pulling some wire. Employee removed gloves to gain a better grasp on the spool of rope when the electrician pulled on the other end. This pulling action resulted in the metal ring in the center of the spool to cut the emplyees finger.

Employee felt an abdominal pain coming on during his work week.

Hyper-extended his arm while climbing a ladder inside the bin.
