Miner was shoveling wet material at the entrance to the Lower Mill.
Employee cut a box open with a utility knife and placed the knife on top of the box. When the employee pulled the box open with EE's left hand EE's left index finger make contact with the blade of the utility knife resulting in a cut to EE's left index finger.

North Railroad switch in North Branford Quarry was frozen. Heat was used to melt the ice on the switch. It freed up but not all the way. Employee used pry bar to see where switch was hanging up. Pry bar slipped and employee fell backward onto right wrist.

EE WAS EXITING FROM THE BACKHOE LOADER. THE EE SLIPPED ON THE FRESHLY FALLEN SNOW WHICH HAD COVERED THE EXISTING ICE FROM PREVIOUS STORMS. THE EE WAS UNAWARE OF THE ICE BENEATH THE SNOW. EE SL IPPED BACKWARDS FALLING ON HIS LEFT WRIST WHILE TRYING TO BREAK HIS FALL.

EE WAS REPLACING CAM IN SPRING GUIDE. A BENT CASTING WEDGED AGAINST THE SPRING INSIDE THE DEVICE. WHEN THE PUSHER BOLT WAS BACKED OFF PARTIALLY THE LINE UP PIN WAS REMOVED BECAUSE IT WAS LOOSE IN THE BORE. WHEN HE TOUCHED THE END CASTING IT RELEASED THE SPRING TENSION PROPELLING THE HOUSING STRIKING HIS HAND AGAINST A WORKBENCH VISE.
EE WAS GETTING INTO HIS TRUCK AT THE BEGINNING OF HIS SHIFT AND ENTERED THE CAB IN A DIFFERENT MANNER THAN WHAT HE WAS USE TO . IN DOING TO HE TOUGHT HE PULLED A MUSCLE. HE ATTEMPTED TO EXIT T HE TRUCK LATER IN THE DAY AND BEGAN TO FEEL SHARP PAIN IN HIS UPPER LEG. HE WAS EVALUATED BY A CHIROPRACTOR WHO PLACED HIM ON LIMITED DUTY.
EE WAS GETTING READY TO DISCONNECT A STRING OF AMTRAK CARS REQUIRING HIM TO SHUT OFF THE AIR TOTHE 2 AIR BRAKE LINES. HE THOUGHT HE FORGOT TO SHUT OFF THE AIR IN THE LINE WHEN IN REALITY THE Y WERE ALREADY IN THE OFF POSITION. INSTEAD HE TURNED THE VALVES TO THE ON POSITION SENDING 150PRI OF COMPRESSED AIR THROUGH THE HOSES STRIKINGHIM IN THE FACE.
EMPLOYEE WAS CLIMBING FROM THE DOCK TO THE BARGEWHEN HIS RIGHT PLANTED FOOT SLIPPED FROM UNDER HIM CAUSING HIS LEFT LEG TO BE WHIPPED INTO A BIT.

EE CLAIMS HE FELT A "RIPPING" SENSATION IN HIS BACK WHILE PULLING RAILROAD SPIKES. HE IS CURRENTLY LOSING TIME FROM WORK. QUESTION IF WORK RELSATED DUE TO OTHER CIRCUMSTANCES.
