Employee was in the rollout chute removing a piece of 6" metal used for making 57's and then added plate steel for making 67's while doing this EE struck their ribs left side and stated they thought they felt a pop in their ribs and was having trouble breathing.

Mostrando 50 de 65Miner was changing a drill bit. While spinning the drill bit, it slipped from the miners hand and smashed their left middle finger at around 10:15 am. Miner let manger know at around 10:37 am and requested medical evaluation. As a result of their medical evaluation miner was diagnosed with a broken left middle finger.
During installation of drive belts on Warman Pump employee received a laceration on left pinky finger. The belts had already been installed. Employee was manually turning the shiv to check belt tension when the tip of finger was pinched between one of the belts and the shiv. X-rays revealed a nondisplaced fracture.
EE was spraying a hose when it slipped, hitting the right forearm. The hose clamp caught their arm causing a laceration.
EE was taking a small boulder off of a locked-out conveyor. EE pinched the tip of right ring finger between the rock and the conveyor structure when removing the boulder. Stitches were needed for EE's fingertip.

The employee and another employee were carrying a new screen up to the building. When they got to the top of the catwalk and stopped for a break employee noticed an uncomfortable feeling in employee's abdomen.
Employee was taking a conveyer pulley off a pallet. EE squatted and bent down to lift one end up and roll it off the pallet. As EE did EE twisted slightly causing EE to have pain in the lower left side of EE's back.
EE was attempting to get a piece of steel from the steel rack. EE had moved 2 pcs of channel to get to the pc of steel EE wanted & when EE moved the 2nd pc of channel the 1st pc of channel came back on finger in turn lacerating ring finger on left hand which required 7 stitches.
While ee was priming the fuel pump at tank 1 EE put fuel into the priming tube of the pump. Started the pump and went up on the truck to see if it primed. It didn't act like it primed so EE went back down turned the pump off and shut valves, when EE removed cap off priming tube diesel fuel shot out getting past EE's safety glasses and into EE's eyes and mouth.
Employee was removing the drain plug on the transmission of a piece of mobile equipment. While performing this task the employee strained his left shoulder.
Employee was working on power steering box on a Mack haul truck; the box swung down and pinched his left wrist/arm, causing a laceration.

The injured worker was assisting in the removal of a snubber pulley from a conveyor frame. As the snubber pulley was being lifted the IW's hand became momentarily pinched between the pulley and the conveyor frame. The IW suffered a broken finger and severe laceration. The IW returned to work with restrictions.
EE started shift @ 6:30 am & was working w/co-worker. At about 6:50 am, EE carried a come-a-long weighing about 20#. Immediately afterwards, EE commented he may have pulled a muscle in his back. He continued working, incl. walking, carrying tools & picking up 80# angle iron (@ 8:30 am). When he returned to co-worker & handed him 15# bearing he said he felt sharp pain in his back
While attempting to lift a cover plate to expose a broken hydraulic hose on a 980 Cat loader employee felt a sharp pain in his abdomen. He was treated for an abdomen strain & returned to restricted duty. He returned to his regular job as a mechanic. After additional testing it was determined that he has a hernia. He is OOW for hernia surgery as of June 18th.
Flipping cutting edge
EE was loading an old cutting edge into a one ton dump. He lost control of cutting edge and caught his left hand pinky finger between the cutting edge and the top of the tailgate causing a fracture & contusion needing stitches. EE was wearing heavy gloves and the cutting edge weighed approx. 65 lbs.
Employee was shoveling waste material under a conveyor when he felt pain in his right hip as he worked. chiropractic treatment was sought at first by didn't help. lost time began after treating with a physician who took EE out of work on October 27.
Shoveling gravel into holes at quarry shot, hurt shoulder.
While installing speed reducer on head pulley conveyor 26, EE pushed the speed reducer onto shaft. Injured had his hand on back side wiggling the reducer for it to slide onto shaft. When it shot on, EE's right hand fifth finger got smashed between the speed reducer and a piece of angle.
EE was lifting an empty oxygen bottle off of the welder truck when he felt pain in his right shoulder. Initially this was reported as first aid however after testing was performed it was determined that surgery was required. EE surgery is scheduled for 12/15/2010.
Employee reported on 3-1-2010 he felt pain in right groin while assisting in lifting a steel plate..he did not request medical treament at the time of report. 12 days later he did seek medical treament and was diagnosed with a hernia and scheduled for surgery on 3-26-2010 and estimated to be off work for 4 weeks.
EMPLOYEE BENT OVER AND PICKED A 5 GALLON SPRING WATER BOTTLE (FULL)WITH ONE HAND AND SLIGHTLY PULLED LOWER BACK MUSCLE
Employee stepped out on catwalk of loader to clean windows when he felt something pull in his left knee area.
EE was guiding inside part of breaker into case of main breaker when it pushed forward pinching finger between both parts cutting end of little finger almost off.
