Employee walking across location when EE felt pain in EE's left foot.

Employee was replacing screens on the plant when EE felt pain in EE's lower back.

EE was weed eating, hit a bees nest, and received stings on their right thigh. EE went to clinic, had 4 stingers pulled out, and received over the counter meds. EE had a follow up the next day where it was found that their stings had become infected. They were given a prescription medication. EE has remained full duty.

EE was walking down the steps of the surge pile tunnel. missed last steps, caught themselves with the handrails, tightened elbows and right shoulder, popped elbow, and now has pain in right shoulder to right elbow. EE went to clinic and was given restrictions.

EE was cleaning under secondary with dingo. EE pinched left wrist between the dingo and plant structure. Our telehealth service was called, and they determined the injury first aid. The telehealth followed up with EE several times. On 6/15, they recommended EE go to clinic. Clinic put EE on restricted duty with EE's left arm until 6/30. X-ray showed no broken bones.

Employee was in the rollout chute removing a piece of 6" metal used for making 57's and then added plate steel for making 67's while doing this EE struck their ribs left side and stated they thought they felt a pop in their ribs and was having trouble breathing.

Finger caught under crane cable after cable slack was winched taut resulting in a laceration requiring 6 stitches. Returned to full duty; no restrictions.

Miner exited work truck to begin drilling. Slipped and fell on ice/snow following snowfall.

EE drove a stake body truck down to level 12. EE exited the vehicle and then reached back in to turn off a strobe light. While reaching, EE stepped in a 6" hole and tweaked EE's back. Appears to of strained a muscle. Upon clinic visit, EE was deemed unable to currently work. Missed workdays and likely light duty ahead.

A large piece of rock fell from the ceiling at an intersection. Some of the rock struck the drill. There were no injuries associated with this incident.

The EE was loading lifters when a piece of rock (approx. 4'x 5' x 2') fell from the ceiling and struck the EE. The EE was not pinned under the rock. This resulted in a fatal accident.

Worker was positioning 10-inch plastic pipe into a 412 Fusion machine to be fused to another piece of pipe. On doing so, EE pinched EE's left pinky finger between the pipe causing a fracture to EE's knuckle.

EE was on the ground helping load a face for a shot. The EE stepped on a rock and twisted EE's left knee and fell to the ground. Work Care was called and recommended a clinic visit. The clinic did an x-ray and said nothing was broken and referred EE to an orthopedic doctor. EE visited the orthopedic doctor on 7/29 who placed the worker on restricted duty and physical therapy.

Employee was moving a section of 12" hose with the telehandler. They attempted to extend the boom out to place it down while on an uneven surface. The outriggers were not extended and the vehicle rolled onto its cab side. The employee was wearing a seatbelt but injured their leg from the impact of the rollover.

Miner was changing a drill bit. While spinning the drill bit, it slipped from the miners hand and smashed their left middle finger at around 10:15 am. Miner let manger know at around 10:37 am and requested medical evaluation. As a result of their medical evaluation miner was diagnosed with a broken left middle finger.

During installation of drive belts on Warman Pump employee received a laceration on left pinky finger. The belts had already been installed. Employee was manually turning the shiv to check belt tension when the tip of finger was pinched between one of the belts and the shiv. X-rays revealed a nondisplaced fracture.

EE was spraying a hose when it slipped, hitting the right forearm. The hose clamp caught their arm causing a laceration.

EE was taking a small boulder off of a locked-out conveyor. EE pinched the tip of right ring finger between the rock and the conveyor structure when removing the boulder. Stitches were needed for EE's fingertip.

As employee was stepping up and transferring their weight, they rolled their ankle.

Employee was repositioning PPE when a small piece of metal went into right eye. After flushing eye, employee received medical treatment for a scratch to cornea.

Slipped while climbing on drill rig. Grabbed something to prevent fall.

Employee fell on same level, resulting in sprain and strain injuries.

Dust was in the air after the use of a pneumatic air tool which caused irritation to the employee's right eye.

EE was attempting to enlarge a hole on a rubber skirt. The knife EE was using slipped and cut the web of EE's other hand. The EE received one (1) suture in their hand.

Employee felt pain in their lower back after shoveling clean rock. Employee was placed on work restrictions.

Employee's right finger was pinched between a come along and a metal plate causing a break to occur requiring a splint and restrictions.

Employee tried to stop a metal plate from falling while trying to position it to be welded.

Employee was erecting steel and placed hand in pinch point causing a laceration to the tip of the left ring finger without damage to the fingernail requiring stitches.

Employee's right little finger was caught between digging bar and back plate resulting in a laceration requiring stitches and a fracture to the finger.

While removing C2 tail pulley guard, the guard made contact with the employee's face around employee's chin causing a laceration requiring stitches for wound closure.

Employees hand was pinched between a metal plate and the magnet on C6 Conveyor.

Employee was struck by a small piece of metal in their nose as EE was helping on changing an anvil in the crusher. The employee had to have the metal removed and stitches for wound closure.

Contractor fell while trying to climb down OEM ladder on a CAT 773E haul truck.

While opening the access door on the ANFO truck, the EE's right index finger struck the tanks frame. A jagged/rough area cut through the EE's glove causing a tear drop shaped laceration across EE's right index finger. The EE needed ten (10) sutures to close the wound.

Employee sustained a strain to their forearm while lifting a screen.

Employee lost balance and fell while moving steel drill rod on bench. Drill rod fell on EE.

EE was descending down the ladder on the scaler. As they did, a foot slipped off of the second (2nd) step. The EE did not fall. They held on to the railing with their left hand and momentum spun their body. This caused their left shoulder to dislocate.

Employee was descending a fixed ladder when fell from the ladder striking the handrail below with their lower back causing injury.

Felt a pop in knee while exiting rear of straight truck

Employee was tying rebar mat on the ground and tripped. Employee got laceration to right arm from falling on end of rebar.

Service technician working on engine removal from a Skid Steer Loader and with impact gun on a fastener lacerated right middle finger while other hand was exposed to the other end of retainer.

Employee's left ring finger was caught between a multiplier handle and metal structure causing lacerations and fractures to the left finger.

Employee was walking when EE slipped and fell on ice causing a fall on same level leading to a contusion of the right shoulder.

Employee slipped on loose stone at the bottom of a ladder causing a slip and fall on same level causing pain to right hand.

Employee was operating a Haul Truck when the truck bounced, causing injury to employee's back.

While using a retractable utility knife, contact was made between the blade and employee's right knee causing a laceration.

Employee was washing down the scale area when employee slipped and fell onto the same level landing on knee. Employee used first aid until employee requested a clinic visit on 10/10/22 where PT was prescribed.
