EE was installing idler. Pushed on idler to put bolt in to hold idler from moving EE felt a pop in their right wrist. Medical treatment sought. Work restrictions issued due to sprain.

Mostrando 50 de 161EE was moving an idler for conveyor. One end was in the air and fell on EE left foot missing the steel toe of EE's boot by an inch. Idler is 82" long by 6" wide.
EEs were in the process of removing old belting. Roughly 30lbs of belting per piece. EE lifted the old belting up with assistance from another EE onto a cart to take out of tunnel. When they bent over to pick up another piece they felt a sharp pain across their lower back.
ee was removing idlers from conveyor for a belt change. when they went to move the idler to the floor, their finger got pinched/rolled in between the cans causing a laceration / split to right pinky finger.
ee was pulling the slack out of a wash hose when they felt a pop in their back. Incident occurred on 8/15/2025 - ee was evaluated by medical professional and released to work without restrictions. Pain was not getting better so went back to the doctor and was put off work on 9/8/2025.
ee was throwing scrap steel into the skip when EE reaggravated a strain in EE's shoulder that occurred the day before.
EE was trying to lift new bearing housing into place on take up. EE said EE was in an awkward position and said EE hurt EE's left wrist.
Employees had a shaft in the upright position in the elevator. when they arrived their floor, one ee held the top while the other bent down to pick up the bottom end. Upon lifting, the ee felt a pop sensation in the groin area. The shaft weighed appx 100 lbs according to ee's. EE sought medical treatment on 3/6 and was put on work restrictions.
Employee was rolling a piece of pipe on the welding table and it hit a piece of angle that was also on the table causing it to fall off and land on employee's left foot just behind the steel toe leaving it bruised and swollen. Employee did not seek medical attention until a later date- ee was put off work eff 3/6/25.
EE was lifting trays of stone for sampling when they felt a strain in their shoulder. Incident was reported on 12/10. EE sought medical treatment on 12/18 and was issued work restrictions and continued medical treatment.
EE was lifting a piece of plate steel into place with left hand - felt pop in bicep upon lifting. Suspected rupture of tendon; currently under medical evaluation.
Ee was moving/loading reducers for transport when one shifted causing hand to get pinched between the two items resulting in laceration/stitches.
EE was heading to the top of the mill in the elevator to check an alarm. Upon reaching the 12th floor EE got out of the elevator and went to close the elevator door. EE was pulling the Strap on the outside of the door and EE's hand got crushed between the doors. Work restrictions issued after medical evaluation.
EE bent down to remove toilet & felt a strain in back - reported at time of incident, no treatment sought at the time. EE was treated on 8/12/24 in ER due to ongoing/increased pain. Was put off work - became reportable at that time.
EE was rolling fuel hose back into hose reel when end kicked up and hit EE in the mouth resulting in chipped tooth. Treatment sought on 8/6/24. Confirmed type of treatment that was provided with dentist on 8/21.
Employee was attempting to remove an oil drain plug, and when it broke free, employee smashed pinky finger on a pipe
EE lifted a 2 x 12 plank to move it out of the way - as EE lifted, EE heard a "pop" in left bicep followed by shooting pain down to fingers on left hand. EE did a report only at the time, but sought treatment a few days later and has been put on work restrictions with further medical evaluation.
EE reported discomfort in ankle - no specific event or incident contributed to the onset of pain. Following day, EE was experiencing increased swelling/pain and wasn't feeling well. Went to ER - admitted for severe bacterial infection in ankle assumed to be attributed to something in the water at work. EE was working in wet area at time of report. Still under investigation.
Employees finger was pinched between screen deck media & frame resulting in laceration and fractured finger tip.
EE was climbing over conveyor belt so EE and another EE could finish securing the new conveyor channel and support leg install and felt a strain in left groin area.
employee was working on replacing bearings which entailed repetitive lifting (with assistance), and resulted in an abdominal strain.

Steel plate slid out of one of the plate clamps then slid out of the other. The plate then struck EE's hand pushing it into a metal saw horse causing damage to two fingers on EE's right hand. EE was sent via ambulance to ER for treatment - xrays determined both fingers were fractured.
Employee was lifting a piece of scrap steel into the metal disposal dumpster, and felt a sharp pain in right shoulder. Employee has continued to work, however, the strain had caused damage to tendons which resulted in surgery on 2/1/22.
Injured was lifting a motor junction box back into place and felt a slight strain in their back.
Team of millwrights was putting flights back on the apron feeder. As the millwrights were attempting to slide the flight, it suddenly moved and EE's finger was pinched between two flights.
Employee was exiting a chute after welding on lifting lugs to remove wear plates. As EE was exiting, EE felt discomfort in EE's knee. EE saw a doctor on the 13th and was given restrictions
Employee was manually moving rail road ties to forks on loader when ties shifted pinching finger between two ties. Resulted in fracture and laceration.
Employee was positioning a wear plate in a chute. The wear plate shifted and employee's right index finger was caught between wear plate and chute, resulting in laceration and fracture.
Employee was handling a recently removed trougher roller to remove it from the area. As they began to tip it on to its side, the can rotated and a finger was pulled in between the cans resulting in a fracture to the left index finger.
While stripping a metal jacketed wire, electrician received a cut from metal jacket requiring 4 stitches.
Employee was tying up vessel to be loaded, when vessel hoist operator fed too much cable to employee causing cable to slip into water and employee to be jerked resulting in strain to back.
Employee was moving over the spray pipes on top deck of screen. When EE straightened up a "zip tie" that was holding a temporary light string in place went between EE's hard hat and safety glasses poking EE in the left eye. Prescription given for drops.
Employee was helping lift a piece of steel to take to the saw. When Employee lifted the steel Employee said Employee felt a pop in lower back. Took to doctor on 3/27/2018 and was given restrictions.
After manually clearing stone from a blocked screen, employee felt a pain in the back of the leg. After another shift with no improvement, employee went to doctor and was given physical therapy.
Employee strained elbow while opening elevator door in mill. Saw doctor day of incident. Treatment at that time was not reportable. Employee still experienced pain in elbow. Returned to doctor on 6/5/17 and was prescribe therapy and light duty for elbow.
Employee was attempting to retrieve a dropped tape measure with a magnet on a rope. Employee put rope in mouth to hold onto it. EE dropped the magnet and the rope jerked two teeth loose. Employee went to the dentist and teeth were reset with no permanent damage expected.
Employee was cleaning up at the end of the shift. Employee had just rubbed their eye and felt a irritation shortly after. The employee requested to see a doctor during the next shift. A small metal filing and rust was removed from the eye. Drops were prescribed.
Employee was carrying a leaf spring for the P135 pick up to the front of the pit in the automotive bay of the shop. Employee went to set the leaf spring underneath the P135 pick up when EE felt a pain and burning sensation in left shoulder. Leaf spring weighed approximately 50 - 60 lbs.
Employee was bending over to put on work boots and felt a pop in lower back. Employee was unable to move.
Surgery 12/19/16 - Carrying a pale of stemming, slipped down a hill, pulled groin
Employee was walking through final screening station when EE stopped to rub right eye. The employee removed glove and rubbed with right index finger. Shortly after that the eye became irritated and the employee flushed it with eyewash at the office. Further irritation resulted in employee seeking medical treatment.
EE1 and EE2 were moving HVAC window units in the Electrical store room to perform maintenance. EE1 was holding one of the units and the unit became wedged against a reel of cable. EE1 lifted that reel up over the AC unit with left arm out of their way. While doing, so ee1 felt a sharp pain in shoulder. EE1 did not see a doctor until 7/21/2016
Employee was training to operate the drill. After break, the employee was returning to the drill, walking over rough terrain, when EE felt a pain in their right calf. EE was taken to doctor for evaluation.
While changing bags in the bag house EE was kneeling down to change a bag and turned to grab a tool EE left knee popped.
Employee was performing maintenance on car 123 and discovered a damaged brake cylinder, which was removed and placed in back of service truck. The employee +1 were removing the cylinder from the truck to dispose of it, it slipped and crushed the EEs finger between the brake cylinder and step of the service truck, resulting in facture of right pinky.
Employee cleaned up some stone with a shovel (less than a wheel barrow). Had sat down. When he stood up to climb some stairs had pain in lower back.
EE was pulling on a conveyor belt when he felt a pain in his back. EE received modified duty on 7/20/15 from the treating physician.
Lab Tech was analyzing stone sizing in the lab, which is a repetitive motion, when she felt soreness in her right shoulder. She reported her soreness on the DOI but did not receive treatment until 7/7/15 which includes modified duty.
EE was reaching over the return belt next to the tail pulley and lifting an idler out of its channel when he felt a pain in his back. Received medical treatment and was given work restrictions 7/20/15.
