The employee was walking down the stairs when, at the bottom step, EE felt EE's hip roll and felt pain. EE did not trip on anything and did not fall.
The employee was assisting in installing a new drop pipe. The pipe remained secured but shifted from its placement, and the employee on the ground had a hold of it to keep it from swinging and felt the impact of the pipe shifting and felt discomfort in EE's back.
The employee was moving thin sheets of metal (not wearing the appropriate PPE, cut-proof gloves) and cut EE's finger, resulting in five stitches.
The employee was bent down picking up extra bolts when they felt a pop in their knee as they stood up. It was determined that the employee tore their meniscus and needed surgery.
EE was performing maintenance and cleanup (during outage) around the Pan Conveyer. EE was attempting to remove a large piece of clinker without removing the guard and pinched ring finger on right hand between bottom of guard and clinker. EE saw physician 2 days later and had fingernail removed.
EE attempted to close pump 5 valve when EE felt a 'pop' in chest. EE called for assistance-reporting discomfort in chest-EMS transported to ER for exam.-Was released w/instructed to see PC Provider. 8/16/22 CNP examined released w/10lb lifting restrictions re: strained/pulled pectoral muscle - follow-up Appt. 10 days. No lost workdays to date.
Two employees were carrying a pump screw to place in a box. One end slipped out of an employee's hands causing the other end to fall while the other employee still held it. This resulted in a smashed finger between the wooden box and pump screw.
2 employees were moving a water pump down the coal tunnel on a sled. Injured employee was above and holding onto pump with left hand and railing with right hand. While moving downhill, the employee's right arm extended backwards and employee felt a popping in forearm. Diagnosed with torn bicep and placed on restricted work duties.
Employee was pulling a cover off of a trolley line in clinker shed but cover was really heavy and when EE pulled harder EE pulled a muscle in EE's right, upper back.
Employee was placing a roller on D8N with a floor jack. The roller slipped off of the jack and employee tried to catch it, pinching EE's left finger between loose roller and already mounted roller. The tip of EE's finger was fractured, the nail was ripped out of the nail bed, and the tissue was torn down to bone.
Employee was doing maintenance on elevator when a large metal pin came loose, bounced off of the door and hit the employee in the face causing a laceration.
Employee states was sampling Raw Mix through a rectangular opening. The employee was holding the cover to keep it aligned and it came down and pinched thumb.
Employee was lifting a port a power from a box when EE's foot slipped causing EE to drop the port a power back into the box. When the port a power fell it struck EE's left hand resulting in a fractured finger and laceration requiring stitches.
Employee was flipping over a plate to remove a pad eye. When the plate was on it's edge it fell back towards the employee landing across EE's right foot.
Employee's foot was struck by a shoot.
Notified today (9/20/2017) the accident aggravated a previous injury. Employee pulled a fuse without proper PPE. Did not see a doctor at the time. In July reported the injury. Has been undergoing numerous tests.
Cleaning coal tunnel, shoveling coal out of trough. Employee took a shovel scoop with a hand shovel and ee felt pain lower back.
Driving a shaft out of a motor. The shaft fell and smashed fingers while holding the shaft.
Employee was turning fan for coupling inlining and felt a pop in the shoulder. The fan was turning real hard and another employee took over.
Employee reported the accident on 10-5-2016 but did not go to the doctor until 10/14/2016. EE said EE was climbing the ladder next to cab of the drill and felt a muscle in right arm pop and sharp pain. EE said EE had three points of contact and should have used EE's legs more.
Employee was cleaning in the basement. EE hit head on a pipe. EE was wearing a hard hat. EE reported soreness in neck and shoulder.
Employee was loading sticks of Powarnell powder into the pickup. EE felt a pain and something pull in groin and up into stomach and back.
The employee was putting a welding jig together and smashed left thumb tip between the 2 halves.
I was painting with enamel paint and inhaled the fumes.
Employee was picking up chunks of cement and twisted and then threw the chunk into the bunker, immediately feeling pain in abdomen.
Employee was trying to knock off old cement build up on an angle iron (2x2) that holds the elevator door in place and jammed left thumb against the build up.
The employee was removing the analyzer probe and were taking it down the stairs. The employee was in the lead and turned to maintain his grip on the front of the probe when he felt a pain in his lower back.
He was lifting a beaver tail onto pallet of forklift inside kiln 6. I hit the beaver tail against the hand rail and hyperextended left knee against beaver tail. Did not seek medical attention at that time. On 7/29/ asked to see doctor. Was put on restriction through 8/3/2015. Now waiting to start physical therapy and he states it still hurts.
While hoisting material up the preheater, EE stepped back and caught heel of foot on concrete pad and fell back. Turned while falling and landed on concrete pad bolts. Called ambulance and was checked out and released. Pain in left thigh and pain in left shoulder was intense.
While removing the second section of air slide, the slide shifted. The corner of the slide caught my left hand glove & pulled it into the supporting structure frame.
Employee was working inside Finish Mill 4 mill separator. He was removing old pieces of metal. He was holding on to a piece of metal when it cut through leather glove and into finger.
Turned to close electrical panel door when lower back pain came on.
Employee was wiping down machines in the machine shop when a foreign object fell into his eyes. He was wearing hard hat, safety glasses, white suit and dust mask.
Miner was pulling himself out of the classifier, he felt a pop and pain in the left shoulder. Miner was released to restrictive duty but voluntarily quit. The project has ended.
Employee was at the gyp pit drilling gyp when he started to have problems with the drill. He was in the process of changing the couplers when the drill steel slipped off the head of the sticker bar and pinched his thumb.
The employee was loading powder on the pickup from a semi trailer. The powder comes in bags 5" in diameter and weighs 30 lbs. Employee says he was picking up a powder stick and when he stood up he felt pain in his lower back and heard a popping noise. He did not seek medical care at this time. He thought it would be better in a couple of days.
Employee was trying to remove a chunk of cement from silo. There wasn't a large amount of cement above chunk. I removed a large chunk and caused cement on top of it to flow out and the same time I poked it again and cement hit my arm and caused the cement to blow into side of my face.
Employee was opening the elevator door at the top floor of the blend silo. The door was difficult to open because it was cold out. Employee stated he strained his shoulder and back and it was sore for several days. On 3/23/2011 the employee reported that his shoulder was hurting and he wanted to see a doctor. He saw a doctor who gave him a cortisone shot.
EE was cleaning bags in the alkali Baghouse and something blew in his eye. He had the eye washed out and cleaned and the Pysician Assistant gave him a prescription for ointment.
Employee was going to salt the area outside the lab door. He took a 30 gal bucket of ice salt and could not get the lid off. He picked up the bucket and attempted to pour the salt from the partially open lid. He said he felt pain in the area between lower neck and upper back. Employee did not report that he received medical treatment until 2/23/09.
Employee was manually rotating Kiln #4. After rotating the kiln with the auxillary drive, he disengaged the auxillary drive clutch. During the counter rotation on the auxillary drive, the clutch blew up, throwing shrapnel. A piece of the shrapnel cut his right fore arm.
Employee was walking from one catwalk to another when he felt his knee pop.
Employee was working on the 6th floor anylizer. He had to pull the probe end out a number of times as well as go up and down 120 stairs at least 7 times. The next morning, he felt moderate pain and tingling. The pain was on the lower right side of the back and the tingling went down the right leg.
He was lifting and turning the panel cover for the dispatch station for the 5 & 6 Finish Mills and smashed his finger between the panel and a piece of C-channel used to support the electrical conduit.
Due to temperature of 19 degrees below 0, this caused sand to stick more than normal & hang up. The crane operator got the lance pipe stuck in the sand due to the frozen sand in bin. He tried to manually pull the air lance out of the sand & strained a rib muscle
Employee was getting ready to change a valve on ep0630 around the base of the valve. when the valve fired, the control arm caught my glove pinching my left index finger between the valve and the control arm.
Employee was moving lines inside the raw mill by lifting them, twisting his back, and setting them back down. While he was lifting the pieces, he felt twinge in his back. He reported the incident to his supervisor the following day. He came in and filled out the accident report and request to see a dr. The dr provided a rx. Diklotenac, lifting restrictions no ladder til 12-19-05.
While working on coal belt EBC-274 removing down-comer roll for conter-weight. We were waiting for crane so we started getting everything ready to come out. Took guard off and took bolts out of bearing base. Went to hook up chocker on bearing shaft and the roll slid back pinching hand between bearing & angle iron.
EE WAS POSITIONING 24" CONVEYOR BELT FOR SPLICING WHEN HE FELT PAIN IN STOMACH. WENT TO THE DOCTOR ON 10/8/04. WAS REFERRED FOR A SURGERY CONSULT. SAME-DAY SURGERY SCHEDULED FOR 10/22/04.
WHILE TRYING TO REMOVE A TRACK PIN FROM THE EXCAVATOR THE TRACK PRESS BOLT BROKE AND THE CLAMP HIT EE IN THE FACE. STITCHES IN LIP AND HE WILL HAVE TO SEE DENTIST FOR CHIPPED TOOTH.
