The employee was walking down the stairs when, at the bottom step, EE felt EE's hip roll and felt pain. EE did not trip on anything and did not fall.
The miner was moving a new wire reel to power the hoist. Miner was using a socket to remove a bolt when the socket slipped and hit the miner in the mouth. The bottom of the miner's two front teeth was chipped off.
The employee was assisting in installing a new drop pipe. The pipe remained secured but shifted from its placement, and the employee on the ground had a hold of it to keep it from swinging and felt the impact of the pipe shifting and felt discomfort in EE's back.
Lost the kiln mcc and lost power to kiln draft, and the atox feed belt caught fire. Fire was controlled in about 15 minutes.
The employee was moving thin sheets of metal (not wearing the appropriate PPE, cut-proof gloves) and cut EE's finger, resulting in five stitches.
The employee was bent down picking up extra bolts when they felt a pop in their knee as they stood up. It was determined that the employee tore their meniscus and needed surgery.
Miner was replacing rear belt skirting. After removing the old skirting, the miner used it to trace and cut a new piece. As the affected miner was starting to cut the new piece, the knife slipped and embedded itself about an inch into their leg. The miner was taken to urgent care, where they received four stitches just above the knee.
Miner was called in to repair the splice in a belt. After completing the repair and reassembling the equipment, the miner checked the belt's operation. While inspecting the belt, the miner accidentally stepped on some clinker on the ground, causing them to slip and hyperextend their knee.
The employee chose a 4-foot pry bar instead of a 6-foot pry bar to open a valve on a silo pump. The employee strained and felt a pop in their shoulder.
The employee was tightening bolts and aligning a motor on a compressor. EE felt fine during EE's shift but felt discomfort 27 hours later, after driving 5 hours on personal time. EE felt discomfort in EE's right rib area. The injury is still under investigation to determine whether it is work-related or not.
A miner received fatal injuries. After a bridge collapsed that the miner was operating a skid steer loader on. The miner was clearing refractory brick form a rotary kiln. The bridge that was installed by the property owner collapsed. The skid steer fell approximately 10 feet into a hopper, causing the payload of brick to engulf the operator causing fatal wounds.
Metal shaving from lathe operation struck miner in left index finger. Miner received 3 stitches
Miner was adjusting a belt scraper using an adjustable wrench. The wrench slipped off the scraper with a lot of force and hit the miner in the mouth.
Employees were using a chain hoist to lower parts down into the tunnel. One employee grabbed the chain on the hoist and had EE's hand on the handrail, which created a ground, causing EE to receive a shock of 270 volts. The 480-volt outlet was wired incorrectly. The employee was taken to the clinic, evaluated, and returned to work with no ill effects.
EE was replacing a chain within the Primary Cone Crusher. As EE was kneeling on the concrete ring at the base of the cone cutting a chain link - the angle grinder they were using kicked back striking left thigh above the knee cap causing a laceration. EE was taken to the walk-in clinic where the wound was sutured - EE was released back to full duty.
A visiting GCC EE had exited a company Pickup truck and was walking to the Maintenance Shop entrance. As EE was stepping up on to the sidewalk apron (approx. 6" high) EE caught the toe of EE's boot on the sidewalk lip and fell forward. The resulting impact of the fall fractured EE's right hip requiring surgery to repair the hip.
EE was barring/removing consolidated material from the #5 Finish Mill Lime-Stone Feeder. The bar slipped causing EE's hand to contact the corner of the feeder. The fingernail was caught and peeled off within EE's glove. EE was treated at an Urgent Care facility where the wound was sutured, cleaned and bandaged. EE returned to work on the next shift without restrictions.
EE was performing maintenance and cleanup (during outage) around the Pan Conveyer. EE was attempting to remove a large piece of clinker without removing the guard and pinched ring finger on right hand between bottom of guard and clinker. EE saw physician 2 days later and had fingernail removed.
EE attempted to close pump 5 valve when EE felt a 'pop' in chest. EE called for assistance-reporting discomfort in chest-EMS transported to ER for exam.-Was released w/instructed to see PC Provider. 8/16/22 CNP examined released w/10lb lifting restrictions re: strained/pulled pectoral muscle - follow-up Appt. 10 days. No lost workdays to date.
Two employees were carrying a pump screw to place in a box. One end slipped out of an employee's hands causing the other end to fall while the other employee still held it. This resulted in a smashed finger between the wooden box and pump screw.
2 employees were moving a water pump down the coal tunnel on a sled. Injured employee was above and holding onto pump with left hand and railing with right hand. While moving downhill, the employee's right arm extended backwards and employee felt a popping in forearm. Diagnosed with torn bicep and placed on restricted work duties.
Employee was using a small sledge hammer to beat material out of bin bottom. At times, employee was swinging in a back handed manner. Felt muscle soreness later in shift. Monday evening employee was feeling numbness/tingling in fingers and zingers in left arm originating at the elbow. Employee was seen by physician and placed on modified work duties.
Employee was walking across the plant and slipped on ice. EE hit their head, which required 4 staples.
Employee was grinding metal that had recently been welded in the hammer mill when a piece of metal flew up under EE's face shield and got into the left eye. EE's safety glasses were not fitted tightly due to wearing an N-95 mask.
Employee was standing on a 3' high trailer, taping a tarp cover over a motor. EE's glasses fogged up due to wearing a mask, and when EE went to take a step EE fell off of the trailer, hurting EE's shoulder.
Employee was climbing down out of a haul truck, while holding jacket in one hand, and EE missed the grab handle and fell backwards to the ground.
Employee was pulling a cover off of a trolley line in clinker shed but cover was really heavy and when EE pulled harder EE pulled a muscle in EE's right, upper back.
Employee was working on cleaning a silo and as EE was walking EE stepped on a small rock causing EE to slide and hyperextend EE's right knee.
EE was lying on an undercarriage to access the hoses under an excavator. As EE was wheeling self around EE heard and felt a pop in lower back, followed by pain. Went to urgent care and was released with restrictions.
Employee was tightening a flange with adjustable wrenches when one of the wrenches slipped off the bolt and EE's hand hit the valve actuator jarring EE's wrist and hand. EE did not originally report until 1/29/20, at which time EE was sent to urgent care to get looked at and was released with restrictions.
Employee was working in an awkward position in the Feed Shute, swinging a sledge hammer to dislodge a liner when EE felt a sharp, shooting pain in the right shoulder. EE did not report the injury right away as EE thought they were o.k., however EE ended up going to the doctor and started physical therapy on 1/24/2020.
Stepped up on curb and slipped on snow covered ice, breaking left wrist.
Weather was blizzarding with heavy winds. Miner was climbing onto a locomotive with snow on the step and slipped. EE tried to catch self, but fell with all of EE's weight on EE's right arm.
Employee was walking out ot the blend silos and when thee stepped down off of the curb they rolled an ankle. There was a large pile of dirt that they tried to step around so they stepped down wrong.
Employee was placing a roller on D8N with a floor jack. The roller slipped off of the jack and employee tried to catch it, pinching EE's left finger between loose roller and already mounted roller. The tip of EE's finger was fractured, the nail was ripped out of the nail bed, and the tissue was torn down to bone.
Employee was doing maintenance on elevator when a large metal pin came loose, bounced off of the door and hit the employee in the face causing a laceration.
Employee states was sampling Raw Mix through a rectangular opening. The employee was holding the cover to keep it aligned and it came down and pinched thumb.
Forklift was parked, the operator climbed in to begin another task. Upon turning on the forklift the forklift rolled and rolled over injured employee's right foot. This forklift is a hydrostatic brake system, when you let off the gas the forklift is to stop.
Employee was lifting a port a power from a box when EE's foot slipped causing EE to drop the port a power back into the box. When the port a power fell it struck EE's left hand resulting in a fractured finger and laceration requiring stitches.
Employee was swinging a twelve pound sledge hammer to get rebar into position. The employee had hands choked up on the handle near the head of the hammer. When the employee swung the hammer, ee missed the rebar striking the rebar with finger between the handle and rebar causing a broken finger bone and a small laceration.
Employee was flipping over a plate to remove a pad eye. When the plate was on it's edge it fell back towards the employee landing across EE's right foot.
Utilizing a bar to tighten clamps between pump and bottom of silo. When employee let up on the bar the clamp snapped and bar hit chin.
Employee's foot was struck by a shoot.
Employee's foot was stuck by a skid causing a fracture.
Employee was sweeping and backed up and hit EE's elbow on the stairway. Talked with a nurse but did not seek medical attention until 3/13/2018. An MRI was schedule and medical release was given on 3/28/2018. Employee will be required to wear a splint while working EE's regular job.
Employee was installing the Christmas star and the wrench slipped and hit ee on the forehead causing a cut.
Emloyee was trying to put a radius on a twisted d-ring using the hyd press. When pressure was applied to the piece it slipped and sent one of the set up blocks flying . Hit the employee in both arms and lacerated the left elbow.
Notified today (9/20/2017) the accident aggravated a previous injury. Employee pulled a fuse without proper PPE. Did not see a doctor at the time. In July reported the injury. Has been undergoing numerous tests.
EE was removing mill liners, with a large hammer, and a piece of metal struck EE in the lower left calf muscle causing a first aid. EE said they were ok. That night when at home the EE called supervisor about 9 PM and said they would like to see a doctor, supervisor transported EE to Rapid City SD. Hospital for treatment. The doctor released EE to work next shift.
Cleaning coal tunnel, shoveling coal out of trough. Employee took a shovel scoop with a hand shovel and ee felt pain lower back.
