Employee was cleaning the slurry pit when EE's right leg sunk into the ground up to the knee. The employee pulled EE's leg out of the ground and EE's leg was covered with slurry. The employee washed off with a nearby water hose but forgot material under EE's sock and suffered chemical burns on EE's right foot. The employees wound got infected and EE was prescribed antibiotics.

On July 24 at 0415hrs a production employee was tasked with cleaning out south 4 baghouse compartment. As the EE was attempting to open the second clamp on the compartment hatch material came spilling out onto employee knocking EE's safety glasses down on EE's face and getting into both eyes. This incident caused chemical burns in the left and right eye of the employee.

While loading slurry into a tanker truck, slurry coming from a leaking hose splashed into employees face getting behind the lenses of the safety glasses causing a chemical burn in employee's right eye.

Employee(a) worked on 11/10 from 0730-1630hrs, after working EE's scheduled shift employee(a) was called back to work at 1900, 2218, 0134 and 0230hrs. On 11/11 around 0430hrs employee was replacing a gearbox with employee(b). While employee(a) was aligning bolts for installation, the gearbox slipped out of employee(b) hands and crushed the pinky finger of employee(a) right hand.

Employee was inserting a long handled sample scoop through a small window on Kiln 1 to obtain a sample of lime from the kiln. While performing this task lime dust and debris flew into employee's eyes.

Patching roof holes, setting plates down and rib pop out of place, left side, aggravation of an old injury.

Employee was installing and aligning the base for a gearbox and when sliding the base into position, employee's middle and ring fingers on right hand were pinned between the base and a piece of metal causing a laceration and fracture.

Employee felt pain in right elbow while rotating pulley on pulverized whizzer. Pain increased over several weeks. Employee requested medical attention on 3/20/20.

While exiting personal vehicle in employee parking area, employee swung the driverside door open and began to step out of the vehicle. The door swung back and struck employee in the head causing a laceration on the upper forehead which required 7 sutures to close.

Employee was removing lime in the Product Tower. Lime burn developed on wrist. Over the next week, the wound got infected. Medical attention requested on 1/9/20.

Employee was walking down a set of stairs leaving while carrying a bucket of tools. While walking down the steps, EE's knee began to hurt. The pain got progressively worse over the next couple of days and EE requested medical treatment on 10/25/19.

While in an awkward position, an Employee was pulling a transmission jack by the handle and developed pain in their lower back.

employee was removing lime piles and began to develop limes burns on the lower legs and wrist.

Employee was brushing dust off the front of shirt and as ee swiped right hand down left shoulder/chest area, the ridge of palm got jabbed by a the tip of a pencil in front pocket. Medical Visit on 10/24/18. The puncture wound got infected requiring medical intervention to drain the infection on 10/26/18.

Employee was lifting, bending and squatting to changing tires and working on other assignments. Felt sore all over while working. The next morning on 6/16/18 awoke to pain in lower back. Requested medical attention on 6/18/18.

Employee's eye became irritated while cleaning up spilled lime. Medical visit on 6/11/18.

Employee was entering the operator cab of a Haul Unit and bumped the top of head on the door frame. A headache and pain developed.

Employee was lifting a metal seal when hand slightly twisted causing a wrist sprain.

While exiting a vehicle wind blew material into employee's face. Flecks material went behind the safety glasses and entered left eye causing irritation.

Employee was throwing bags of trash into the dumpster and felt employee's left shoulder popped out of place.

Employee was lifting a box from the ground with fittings in it that weighed approximately 60 lbs and felt a muscle pull in left arm on the inside front elbow. The pain got worse over the next few weeks and on 5/10/17 employee requested medical attention.

Employee was doing preventative maintenance on the West Elevators. Wind blew material into face while employee was changing out goggles to safety glasses causing eye irritation.

Employee was moving 50 lbs bagged product and experienced pain in lower back.

As employee exited MCC a forklift made a turn in front of EE and blew dust around EE. Later that evening, EE's left eye began to itch and in the morning the top and bottom eyelids were swollen. Medical treatment on 9/2/16.

Employee was cleaning up spilled material with a shovel. Wind blew material behind safety glasses and into eye causing irritation. Requested medical attention on 7/6/16.

Employee was blowing down the bag flattener with compressed air to remove dust. Particles got behind EE's glasses and into EE's eye causing irritation. Employee requested medical attention on 6/1/16. Eye prescription prescribed.

Employee was transferring Brake Parts Cleaner from the 50 Gallon Drum into a 5 Gallon Bucket. As EE was cranking the hand crank attached to the 50 gallon drum the nozzle spun out of place and sprayed a stream of brake cleaner onto the top of the bucket. The brake cleaner splashed up into behind EE's glasses, onto EE's face and into eyes and mouth.

Employee was shoveling material out of an airlock chute on top of Bin #6 from a standing position. As employee shifted pressure back and forth from foot to foot in order to be able to reach the chute opening they felt pain in their right ankle.

Employee was removing a drive shaft from Loader. As EE was lowering the shaft down, it fell to the ground. One end of the shaft landed on top of EE's right hand.

Employee was shoveling material and experienced back pain. Bent over to pick up tools which caused more pain. On 2/13/16 experienced additional back pain. On 2/14/16 requested medical treatment for back pain.

Employee was closing the sliding doors of Building #1. One door stuck. Employee grabbed the door from both sides and pulled on it to try to get it to move. Pulling motion caused back pain. Requested medical attention on 2/3/16.

Employee lifted a 5 lbs bucket with tools inside of it (wrenches, hammers etc) from the ground. EE immediately felt pain in EE's lower back. EE requested medical treatment on 1/6/16. Diagnosis: Back sprain.

Employee was shoveling spilled material off of the cement pad at the P-13 Conveyor Belt. In the process of shoveling, his left knee began to hurt. He sought medical attention on 12/18/15. Diagnosis: leg sprain

Employee was using a shovel to clean up spilled material. As he was shoveling material, he felt pain in his shoulder. Diagnosis: Shoulder/arm sprain. Employee requested medical attention on 4/24/15.

Employee was putting 50 lbs bags of Pebble Lime onto pallets inside of the Pebble Bagging Building. His back/waist began to hurt the next day. Employee requested medical treatment on 3/12/15.

Employee sandwiched his middle finger between a T-bar handle and grate structure while cleaning the grate causing a contusion injury.

Employee was bagging pulverized lime at the Pulverized Bagger. Pulverized lime on his face shield either fell through a hole in the face shield which is used to secure the face shield to the bracket or through the thin gap between the face shield bracket and the brim of the hard hat. The material then fell behind his safety glasses and prescription glasses and got into his eye.

Employee spun a sheave clockwise with his left hand to position it and placed his right hand on top of the V-Belt between sheave and motor sheave. His thumb was underneath the V-belt as it moved. His right thumb got pinched in space between the V-Belt and motor sheave causing a fracture.

Employee opened a one-touch door above the separator to release some pressure. As he turned to walk away, material erupted from the chute creating a cloud of hydrated lime around him. Material got behind safety glasses and into both eyes.

Employee was cleaning a fuel tank with a mixture of water and Deox (a corrosive acid). As he poured the mixture over the tank, some spilled onto the top of his shoe and soaked through it. The mixture made contact with his skin and caused irritation and second degree burns to develop on the top his foot.

Employee was material into the open bed truck. While he was operating the load out controls, the wind blew material coming out of the load out chute in his direction. Material got behind his glasses and into his right eye causing irritation to develop.

Employee was switching out an empty barrel of grease with a full barrel that weighs approx. 120 lbs. As he moved the barrel to the ground he felt a stretch in his lower back but no pain. The pain started on 2/21/14 in the morning and at that time employee requested medical attention.

Employee was moving a storage rack when the top horizontal rail (that is attached to the vertical support with a hinged mechanism) slipped off its base of support and came down. Employee's finger was pinched between the vertical support and the top horizontal rail causing a laceration and fracture.

Employee removed a cover to unchoke a pipe located below Kiln #1 Primary Dust Cyclone. Upon removing a cover plate on the pipe, a large amount of dust was expelled into the atmosphere. Dust got behind the employee's safety glasses and entered both his eyes.

Employee was moving 50 lbs bags of ANFO from the mobile trailer to the holes on Face 7. He said he twisted at the waist and his back started to hurt shortly there after.

Employee was doing clean up on ground level around the Raymond Mills. He was bending down and shoveling almost all day. When he got home that same evening, his symptoms were worse and the pain was more severe. The following day the pain was still there and his neck was also very sore. He requested medical treatment on 10/3/13

Employee was cleaning up material under the 7C conveyor belt using a shovel. He was shoveling material that had a lot of clay in it so the material was wet and heavy. As he was shoveling the material, it put a lot of strain on his back and caused pain. He requested medical attention on 8/14/13.

Employee was working inside a compartment in the Bag house. As he was repositioning a map to kneel down on; he positioned his right hand under his right knee. His right ring finger got pinched between his knee and the metal floor causing his fingernail to rip halfway off.

Employee was unloading bags of ANFO out of the Buckley trailer and into trailer 901. As he picked up a bag on the pallet, he twisted at the waist and felt pain in his lower back.

Employee was removing the bolts from the skid pan on the transmission under a Loader. As he was removing the last bolt, the skid pan dropped onto the impact hammer he was holding in his right hand. The webbing between his thumb and index finger got pinched between the handle of the impact hammer and the yellow pit safety rail causing a laceration.
