Employee was replacing an axle shaft of Komatsu HD1500-7 Water Truck. Employee found the axle bushing was stuck and used a glove hand to attempt to lift the sun gear side of the axle to free it. Upon pushing upwards on the sun gear, the employee lost grip on the oily surface and EE's forearm contacted a spline on the sun gear resulting in a 1.5" laceration requiring 5 stitches.

Mostrando los 35Two employees were at the coal barn installing a wing shroud (steel plate) on a dragline bucket. The upper piece was set in place and the crane was lowered. Both employees were going to adjust the piece of steel plate by hand when it fell. It slid down the side of the bucket, hit the ground and fell over hitting the employee breaking two Metatarsal bones in EE's right foot.
While setting up to change the hoist ropes on 102 dragline a pin that secures the shackle of the snatch block was not screwed in. This resulted in the shackle spreading apart and the snatch block popping out hitting the employee in the leg breaking the left tibia.
Employee was replacing a 2' x 4' x 3/8" piece of steel plate on a bulldozer blade. The employee stood the plate up resting the 2' edge on the floor and begin walking the plate corner to corner to the center of the blade. Once they got to the center of the blade they began laying the plate down on its opposite edge (4' side)when the injured employee felt a pull in left arm.
While on a break from ERT training, two employees were looking at a pocketknife. The injured employee made a motion to point out a function of the knife at the same time that the employee holding the knife happened to flip the blade open. The knife blade cut the injured employees right index finger and sutures were required to close the laceration.
While finishing drag rope change out on a 1570 dragline. The winch line dropped out of the horizontal fairlead sheave, employee was guiding the cable back into the sheave. During this process employee's left thumb was pinched between the outer and inner horizontal sheaves fracturing the thumb. The employee was wearing leather gloves at the time of the incident.
Employee pinched finger between synthetic sling and jib crane hook.
Employee opening the rear doors on a 53' tool trailer to gain access to the inside. Employee had release and pulled on the inner latch handle and was attempting to unlatch the outer latch when the handle came toward employee striking employee in the upper right lip. Employee received 8 sutures in upper lip. No equipment damage.
Tire technician was in the process of dismounting a 33.00X51 tire on the LF Steer of an end dump rock truck. When technician released the lock ring, it fell and rolled around and struck the technician's right big toe.
A mechanic was installing a pull block on the rear of a scraper. Employee got finger pinched between the pull block and a backup alarm mounting plate. The employee fractured the tip of right index finger.
Operator placed lunch box in cab. As ee stepped back to exit the cab foot slipped off the step. The operator reacted by swinging arm to catch self and the back of right hand contacted the handrail. An x-ray showed a cracked bone in right hand.
Employee was rolling up power cable. EE got off the truck to tie the rope to the cable. While rolling up the rest of the cable/rope the employee was unaware ee was straddling a loop in the rope. As ee rolled the cable up leg got pinched between the rope and equipment, injuring employees left leg. Employee was unable to return to work on next scheduled shift.
Pulling a transmission out from a Kress truck. The drive line guards fell off the module and landed on my left upper knee. Pushed it over my ankle to the floor.
Employee using hands to place Rip Rap (12" to 24" stone). When employee removed a stone another stone rolled and mashed the Left Pinky Finger against another stone causing injury. The employee was using all PPE. First Aid administered and employee taken to Occupational Clinic for treatment. Three stitches or sutures.
Individual felt irritation in his left eye while in the shop. Flushed the left eye out a few times, but still felt irritation. Took individual into the doctor for observation. Doctor removed material and prescribed anti-biotic prescription.
Traveling out of the pit, antifreeze was seen coming from the radiator. The operator placed his hand on the radiator cap to tighten it, in doing so it came off getting hot anti-freeze on his arm and leg.
EE was working on a machine placing a seat into the cab when he felt pain in his lower back
Three employees were positioning a 1"x8"x8' piece of plate when one end slipped off one stand and pinched the right thumb between the plate and the skid they were working on. Right thumb was fractured.
Employee was preparing to gauge a screw jack from bottom of Revolving frame and bumped a stand and the stand fell, pinching left ring finger, causing a laceration. Five stitches were administered.
Employee had something in his eye and had no idea when he got it in there or how he got it in his eye.
Employee was moving a peice of steel plate and the employee put plate down to get a better grip and the plate slipped and landed on the metatarsal part of his boot and fractured left foot.
Employee was removing a wire that was caught in a dozer track when wire was pulled on it cut finger.
Mechanic was swapping pickup beds from one truck to another and pinched his fingers between the truck frame and the box frame of the bed. This resulted in him receiving stitches in his 3rd finger and cutting the tip on his 4th finger.
Mechanic was hauling a rock guard, when stopped at the shop he noticed that the guard had shifted and was partially hanging off the back of the truck. He lifted the guard to push it back on to the bed, in doing so he caught his middle finger on his left hand pinching it between the bed and the guard. This required two stitches to repair the cut which ran under his finger nail.
EMPLOYEE WAS CHECKING A HEADLIGHT THAT DIDN'T WORK. HE HAD REMOVED THE HEADLIGHT AND WAS LOLDING IT IN HIS RIGHT HAND AND SHOOK IT TO SEE IF IT WOULD RATTLE. WHEN HE DID THIS, IT BUSTED IN HIS RIGHT HAND CUTTING HIS THUMB
Employee was doing a turn and undercut operation on hoist motor #2; at the time of the incident he was stoning the commutator. The stone kicked back and the middle finger of his right hand was caught between the stone and brush arm. Injury- fracture to tip of finger, cut on finger that required 9 stitches.
A mechanic was filling a coal hauler trailer strut with grease when he lowered his dark glasses to better his vision. At that moment, the grease came out of the relief valve slightly spraying into his eyes. The local Physician prescribed the eye drops Tobramycin only as a preventative measurement against any infections.
EE DISCONNECTED A HYDRAULIC HOSE WHILE UNDER PRESSURE STRIKING HIS RIGHT FOREARM, CAUSING 2 1/2" LACERATIONS.
EE WAS REPLACING WINCH LINE ON DOZER. AFTER INSTALLING THE LINE AND TYING THE KNOT IN THE END OF THE LINE, EE BEGAN TO TIGHTEN THE KNOT. THE TAIL STARTED TO SLIP THROUGH THE KNOT. WHILE ATTEMP TING TO RETIE THE KNOT, THE TAIL SUDDENLY CAME LOOSE AND STRUCK HIM IN THE MOUTH.
EE WAS STRUCK IN KNEE BY FLYING METAL FROM A DOZER RIPPR TOOTH DURING REPLACEMENT OF TOOTH. 2 STITCHES WERE APPLIED TO THE KNEE
A 6" BORING BAR WAS BEING ALIGNED TO BORES WHEN A TEMPORARY TACK ON BEARING FIXTURE BROKE ALLOWING BORING BAR TO FALL ON EE SHOULDER.
A 6" BORING BAR WAS BEING ALIGNED TO BORES WHEN A TEMPORARY TACK ON BEARING FIXTURE BROKE ALLOWING A BEARING PLATE FIXTURE TO PINCH EE LEFT HAND AND FINGERS.
EMPLOYEE WAS ROLLING CRIBBING OFF FORKS OF VOLVOLIFT. CRIBBING ROLLED OFFF FORK AND CAME CLOSE TO EMPLOYEES FOOT AND LEG. EMPLOYEE TURNED HIS BODY QUIKLY TO AVOID THE CRIBBING AND TWISTED HIS LEFT KNEE.
EMPLOYEES WERE REMOVING THE RETAINED PLATE FROM THE SWING CARRIER BEARING; THE BEARING FELL FROMTHE RETAINER, KNOCKING EMPLOYEE ON HIS BUTTOCKS.
EMPLOYEES WERE REMOVING THE RETAINER PLATE FROM THE SWING CARRIER BEARING; THE BEARING FELL FROMTHE RETAINER, STRIKING EMPLOYEES KNEE & JAMMING HIS LEG AND ANKLE.
