Employee was re-anchoring the shuttle car cable in the #4 entry between 203-204 crosscut on the MMU 002 working section when employee slipped, twisting and fracturing the right ankle.

While shoveling the #14 belt tailpiece, employee pulled something in the middle of his back. He finished the shift and it began to hurt worse on the ride outside.

Two workers were pulling a cable off of buggy reel when EE felt a pop in his back. He did not begin to miss work until 1/21/13.

Employee did not report until 12/27/2011 - While assisting lifting covers of RRII roof bolter he thinks he hurt his lower back. It did not start bothering him until 12/25/2011. He has had previous back injuries with previous employers.

EE was helping crew un-hang 1 1/2" water line hung up to roof bolt plates with basket hangers. When taking the hose out of the hanger, he cut his left ring finger on a roof bolt plate. He was not wearing his company provided meta carpal glove.

EE was moving on the #5 shuttle car. He was rolling up the cable when it hit him on the leg.

Employee was lifting the rub rail on the miner. He pulled his back. He finished the shift. As he left the lamp house, he told his foreman he hurt his back. That is all the information he gave or has given.

While moving miner cable, employee felt something in his lower back.

Employee was constructing a stopping with 8" solid blocks. As he lifted one of the blocks, he felt something hurting in his back. He finished the shift. On 8/12, he went to the ER and was treated for a lower back injury. He is still undergoing treatment.

Crew was setting posts in preparation for new bleeder system. Employee said sometime during the night he pulled something in his neck or back. He finished the shift and reported it to the shift foreman. He has not worked since 4/23/07.

Employee was shoveling #1 beltdrive to transfer area when he pushed the shovel under the belt to shovel, he felt something in lis lower back glue on strain. he finished the lift, went home, then went to er.

Lifting

ROOF BOLTERS AND SUPPLYMAN WERE LOADING SUPPLIES ON ROOF BOLTER. THE OFF SIDE OPERATOR WAS PUTTING BOLTS FROM SUPPLY CART ONTO HIS SIDE OF THE MACHINE WHEN HE FELT A TWINGE IN HIS BACK. HE STATED THE AREA WAS LOW AND HE WAS LIFTING NORMAL WEIGHT / LOAD. THE EMPLOYEE FINISHED THE SHIFT AS USUAL.

WHILE CARRYING TWO CANS OF WATER TO MIX WITH MORTAR, EMPLOYEE HIT HIS HEAD ON A ROOF BOLT.

EE WAS ASSISTING WITH REPLACING A 13 HP FLYGHT PUMP. HE WAS MANHANDLING THE PUMP TO GET IT INTO DEEPER WATER. HE SAID HE FELT SOMETHING FUNNY IN HIS BACK. WHEN HE RETURNED TO WORK HE SAID HE WAS HAVING BACK PROBLEMS. HE ATTEMPTED TO WORK SHIFT BUT LEFT AFTER 1 1/2 HOURS.

EMPLOYEE WAS STRAIGHTENING OUT A BOLT TO INSTALL IN ROOF TWISTING BACK.

EMPLOYEE STEPPED DOWN OFF OF BELTTAIL PIECE AFTER CROSSING. AS HE PUT HIS BODY WEIGHT ON THE LEG THAT WAS ON THE GROUND, HIS KNEE GAVE OUT AND HE FELT PAIN IN THE KNEE. 3 HRS LATER HE REPORTED IT TO THE FOREMAN AND WAS TOLD HE SHOULD HAVE ITCHECKED OUT. HE LEFT THE MINE AT 1 PM. EE STATEDHE DID NOT SLIP OR TRIP.

STRAIN OF BACK WHILE LOADING CABLE ONTO MANTRIP CAR. FIRST WENT TO DR. ON 8-17-02 DR TOLD EE DIAGNOSIS WAS ARTHRITIS RETURNED TO DR AND OFTEN MRI EE WAS TOLD HE HAD HERNIATED DISC. 1ST DAY OF WORK MISSED WAS 10-28-02. EE HAS HAD BACK PROBLEMS FOR YEARS.

EE WAS BUILDING A STOPPING WHILE IN A KNEELING POSITION. HE TURNED TO THE SIDE TO PICK UP A BLOCK AND FELT PAIN IN HIS KNEE.

EE WAS TRYING TO REMOVE A ROCK FROM THE SECTION TAILPIECE. HE WAS PULLING/LIFTING THE ROCK WHEN HE FELT PAIN IN THE BACK AND GROIN AREA.

EE WAS LIFTING/LOADING BELT STRUCTURE INTO SCOOP BUCKET STRAIN OF BACK.

EE'S RIGHT KNEE WAS INJURED WHILE LIFTING 42" POSTS. THE COAL SEAM HEIGHT IS 38" AND INJURY IS DUE TO WORKING ON KNEES.
