A brow in the #17 crosscut of the intake primary escapeway fell and rolled into the entry. The brow was 6' x 5' x 4' thick and impeded travel in the entry. Two (2) 4ft resin bolts were pulled out when the brow fell. After examination by a certified employee, floor to roof support, in the form of wooded cribs, were built to support the area and the rock was removed.
Employee was re-anchoring the shuttle car cable in the #4 entry between 203-204 crosscut on the MMU 002 working section when employee slipped, twisting and fracturing the right ankle.
Roof fall in 207/208 block, Number 2 entry. Fell on the Continuous Mining Machine when Miner operator was removing Miner from area. Miner had to retrieve with hydraulic retriever.
Fall in intersection of #2 entry on 002 section in last open cross cut.
While riding out on the mantrip, the driver hit a crib causing employee to strike nose on the mantrip.
While sounding and scaling the roof, employee drew hand back quickly to avoid a rock that was dislodged straining right shoulder.
Intersection in #1 heading fell inby the tail piece over anchorage over the weekend while no one was underground.
While operating a rubber tire mantrip the steering wheel jumped out of employee's hand. Tried to stop wheel by grabbing it. The wheel hit right index finger breaking it.
Fall in return discovered in #8 heading 1/2 block out by spad 2100. Operator immediately ceased mining activity when the fall was discovered. This was an unattended area of the mine.
Intersection in #4 Entry at 199 block fell in. The fall extended into the #4-5 crosscut before the continuous miner was able to retreat from the mining area.
#90 crosscut 3-2 and straight of #3 Roof Fall in Primary Escape way. The fall is approximately 70' long, 5' high, and 17' wide. All approaches to the fall area were immediately timbered, and the escape way was rerouted. The fall will not be cleaned up.
While mining in the number two entry Cont. Miner off side panic button malfunctioned. Not allowing the machine to move. While immobile, the inby pillar cut fell in to the turn posts. No rock fell on the miner. After making electrical repairs, the miner was able to remove itself from the cut.
While cutting belt ee cut right hand with a razor knife.
EE was putting ride on charge. EE hit ear on a fire suppression spray. EE covered ear with a rag and finished shift.
A roof fall occurred out-by the 002 section on #11 belt between #208 and #209 x-cuts. The fall was 16 feet wide by 15 feet high by 45 feet long. The fall was reported because it was above anchorage. Approaches were re-bolted and cable bolted. Fall was cleaned up and a square set was installed according to our roof control plan.
A roof fall occurred in the 2 tol x-cut at #260 block outby the working section. Both sides of the x-cut had been cribbed and intersections were bolted with additional cable bolts. The x-cut was deemed bad long before it fell. It was cribbed off already. It will not be cleaned up.
#8 Belt tore apart. EE and others were repairing the belt. While cutting the belt to make a splice, the knife slipped out of EE's hand and cut his right lower thigh. The foreman, EMT, administered first aid, and EE went to ER to have it checked. He got 3 stitches, but the doctor said next time he did not have to come in for a cut like this.
While shoveling the #14 belt tailpiece, employee pulled something in the middle of his back. He finished the shift and it began to hurt worse on the ride outside.
On the 002 Section in the #2 entry face, a rock fell out of the roof striking EE while he was roof bolting and knocked him to the ground. The crew was able to remove the rock and transport him to the surface while performing CPR. He was taken by ambulance to Ruby Memorial Hospital where he was pronounced dead at 1130 pm.
On the 1 North Mains in the #4 face of the 002 Section, the continuous miner cut into an uncharted borehole which was an 8" pipe used as a power borehole to the former Humphrey mine. The pipe was sealed accordingly to plan and the area was made safe. There were no injuries.
A roof fall occurred in the #7 entry 1 block outby last open x-cut in an intersection. The fall was 18'x18'x9'high. No people or equipment was involved. All sides were cribbed and dangered off. The fall will not be cleaned up. We are now installing 6'/12' cable bolts in intersections until we reach x-cut #251.
Employee was running the miner making the initial cut in #2 to #1 x-cut. A small rock (less than 10 pounds) struck his hat then slid down his face cutting his cheek and lips on the left side of his face. The rock was very sharp. The area was fully roof bolted.
EE was positioning the drill station on tram side of bolter when a rock fell from between 2 rows of bolts. His ATRS was set and his canopy was up. He had his left hand on the edge of the drill steel tray when a small rock fell and struck his left hand. He was wearing metacarpal gloves.
Employee installed his 1 & 2 roof bolt on operator side. When he lowered his canopy, he started to clean rock off of it. A small piece of rock fell from the roof and cut his left forearm.
Two workers were pulling a cable off of buggy reel when EE felt a pop in his back. He did not begin to miss work until 1/21/13.
While welding, employee received a flash (arc) burn which did not affect him until later in the day. He is an experienced welder. He had no idea how it happened. There was no evidence of faulty equipment.
A roof fall occurred in the #2 entry at spad #3074. The mine was idle. The watchman discovered the power had gone off underground. Upon inspection the fall was discovered and reported. It was dangered off, cribbed 3 sides and posted on the front side. It will not be cleaned up. It was 30-35 feet long, both ways and 18-22 feet long ways. It was 16 feet high.
While drilling a hole for a 6' roof bolt, the off side bolter operator was struck in the face with a drill steel. It is not know if the steel broke or if it wasn't connected properly.
The #4 entry intersection 40' outby the face fell 18' X 18' X 12'. The #2 Roof Bolter was in by the last open X-cut when it fell entrapping the machine at the face. The bolter operators escaped without injury. There is no rock on the roof bolter. The fall will not be cleaned up. We will back cut the #4 entry to retrieve the roof bolter.
Roof bolters were going to bolt #4-5 x-cut. 2 rows outby the cut, they stopped to spot 2 or 3 bolts. After installing the first bolt near the rib, the left side operator swung head to spot another bolt. After drilling the hole, he was removing steels when a piece of rock hit the canopy, broke, & part of it came under canopy cutting his left arm with sharp edge.
Employee did not report until 12/27/2011 - While assisting lifting covers of RRII roof bolter he thinks he hurt his lower back. It did not start bothering him until 12/25/2011. He has had previous back injuries with previous employers.
After scaling the roof in the #2 to #3 x-cut 1 block outby the 002 section tailpiece, the roof bolter operator trammed the bolter forward to spot bolt an already bolted area. He set up the ATRS, crawled under the operator side canopy and then a rock fell striking him on the back, neck, & shoulders.
The crew was moving the miner from #5 to #3. The buggy men were assisting with the miner cable. EE bent over to move the cable when a rock fell from between bolts knocking his hat off. A second rock fell striking him on the back of the head. He fell to the floor and was administered care after being moved from the spot where it happened.
In the #4 Entry 1 block outby spad #3018, a roof fall occurred that was 18' wide x 18' long x 7' high. The area was dangered off, cribbed on all 4 sides, and cable bolts were installed for 1 block outby and 1 block inby the fall area. The fall will not be cleaned up.
A roof fall occurred at spad # 2335 in a 3 way intersection. The area was bolted with 5' & 6' full grouted rebar. It was 5'-6' high X 16' wide X 40' long. It was dangered off and cribbed. It will not be cleaned up.
002 section #1 intersection last open x-cut to #2 intersection in the last open x-cut a roof fall was found by the preshift examiner It was reported, timbered and dangered off. #1 intersection cleaned up and setting square set arches. #2 permanently cribbed off.
The injured EE was helping another EE get posts for wedges for 2nd mining. As EE was unloading wedges from a wagon the scoop being operated by the other EE accidentally bumped the wheel of the wagon which caused it to hit EE's left knee. The scoop operator thought he was far enough away from the wagon.
A roof fall occurred in the #2 entry 1 block inby spad #2942 in an intersection. The fall was 6' high x 17' long x 17' wide. There were no witnesses nor equipment involved. The area was breakered off on all 4 sides. It will not be cleaned up. This was outby the section tailpiece.
EE was helping crew un-hang 1 1/2" water line hung up to roof bolt plates with basket hangers. When taking the hose out of the hanger, he cut his left ring finger on a roof bolt plate. He was not wearing his company provided meta carpal glove.
A x-cut fell 17'wide X 46'long X 6'high. It was already cribbed on each end. The fall was examined and dangered off and reported. No further cribbing needed. It will not be cleaned up. It was in #7 to #8 x-cut near spad #2049 and #2050. It was in an outby return.
Helper side roof bolter was installing a new row of bolts. As he was swinging drill station toward the rib to install the #1 bolt, a piece of rock hit the canopy, then hit his back and right leg, knocking him to the floor.
EE was moving on the #5 shuttle car. He was rolling up the cable when it hit him on the leg.
In the #4 entry one block outby, face EE was assisting electricans with a cable splice. The razor knife slipped and cut him on the left knee.
While winding belt on a pull with scoop, the scoop bucket was swing toward this employee striking his left foot.
A continuation of the roof fall reported on 3/17/10 occurred. The fall started at previous fall and traveled 100' outby existing fall. The fall was 16 1/2 feet wide and 6' high. The area was bolted with 5/8 X 72" fall grouted rebar with 6X6 plates and 2X2X12 cap boards. This area cribbed and dangered off.
A roof fall was discovered in the #0 entry in the #280 intersection. The fall was 40' long x 18' wide x 6' high. No persons or equipment involved. The 3 sides were cribbed and it will not be cleaned up. This entry was already stopped.
Foreman and roof bolter were scaling loose rock. A rock hit the panic strip. While trying to free it a piece of rock fell hitting the roof bolter operator on the shoulder and arm.
EE was completing a stopping in the #9 entry outyby the 002 section. He stumbled over some concrete blocks. While falling, he caught himself with his left hand. He completed the stopping and finished his shift.
While bolting, the operator had a rock fall from the roof, striking the canopy and then it hit his right foot just behind the metatarsal part of his boot. The rock weighed 25 lbs. He was operating the helper side of the RRII roof bolter.
The intersection in the #1 entry 1 block inby spad #2259 fell. 21' long x 18' wide x 6' high. Fall area was cribbed & dangered off. It will not be cleaned up.
