Operator was pulling draw rock off top of miner when rock slipped off machine resulting in a cut to palm of left hand.

Employee was pulling on skirt rubber to replace belt conveyor when employee felt pain in upper and lower back. Employee started losing time on 8/30/2017

Employee was assisting w/ the installation of a new sieve & strained his groin area. He hoped it would improve over time & when it did not, he sought medical treatment for a hernia on 5/20/15.

Employee was lifting a sample bucket out of truck bed when he experienced pain in his shoulder & arm.

A rock became lodged in the auger tube. When the employee pried the rock loose, bolts securing the auger also loosened, causing the auger to fall from the sample tube. The employee suffered crushing injuries to his right arm as a result of this incident.

Employee was removing boxes from pickup truck, experienced pain in back area

Employee was replacing a blown hose on a CAT D-11 Dozer and had to drop the front belly pan in the process. The belly pan was supported by a strap which became fouled, creating slack, and allowing the belly pan to drop 6" - 8". The belly pan struck the victim in the head, resulting in a laceration that required suturing.

Employee placed his left hand into an air-conditioning vent and was subsequently struck by the fan, resulting in a laceration to his middle finger.

Employee was lifting blocks in front of dozer blade to prevent against motion when he injured his lower back. He began missing work on 7/11/2011 as a result of this injury.

Removing chain hoist, chain slipped down and caught finger cutting off tip of left hand ring finger. Employee began missing work as a result of this injury on 01/25/11.

Employee did not report the incident until 2/2/2011.The employee went to the doctor complaining of respiratory problems.The doctor asked if he had been exposed to any chemicals and the employee said "another employee was cutting a bearing out of a pin bore when the grease caught fire.When a fire extinguisher was used to put out the fire, he breathed some of the chemical".

Employee chose to use bathroom beside the dozer blade. Employee was not wearing a hard hat. Rock fell from blade of dozer and struck employee in head, resulting in laceration that required stitches.

EE had placed his 1" impact wrench on the top step of the loader. While he was hanging his hose on the railing, the impact wrench fell and hit hm on his left foot, right behind the hard toe.

Dozer tracks were being loaded onto a truck, when the hook broke, dropping a section of track. A piece of metal then chipped off the dropped track and struck the employee in the chest, resulting in a laceration that required suturing.

On May 7th, 2010, the employee was both pulling and lifting on a hose to attach to the water truck when he began experiencing upper back pain. Then, on 5/28/2010, the employee began missing work as a result of this accident.

Employee was kneeling sorting parts and had pain in lower back.

Employee was lifting a continuous miner cable while on his knees and strained his back while attempting to stand.

EE was tramming shuttle car when the miner operator pulled his cable too tightly, pulling it down from hangers in front of the shuttle car. The shuttle car operator then went around his car to re-hang the cable when his right knee popped.

Employee was lifting a 50 lb bag of rock dust when he experienced pain in his lower back.

Employee was shoveling material onto a conveyor belt in the raw coal tunnel when he experienced pain in his lower back. He did not report this accident until February 27th, 2009.

Employee was cleaning off roof bolt machine when he pulled on some loose wire that moved underneath his safety glasses causing lacerations to his right eye.

Employee was setting timbers when one fell over, striking the middle finger of his left hand resulting in multiple fractures.

Employee was loading belt structure onto a 4-wheeler when he experienced pain in his left shoulder. The accident wasn't deemed reportable until the employee began taking off work due to complications from this injury on Novemember 10, 2008

Employee was using the grab iron on the equipment to step on the rear of a 14G motor grader so that the engine could be checked for a leak. When he pulled on the grab iron he felt a pop in his right shoulder.

A co-worker pitched a bent anchor pin from the tail piece over to the rib. The pin glanced off the top & struck the injured employee's Right index finger while he was handling structure.

Employee was loading oil cans onto the RamCar when he dropped one can & reached to retrieve it. The can had an upturned tab on the lid, lacerating the employee's left pinky finger and requiring stitches.

EE bent over to pick up and load glue boxes onto the roof bolt machine. He then experienced pain and a popping sensation in his lower back. The injured worked his shift, then visited his family doctor, who advised him to take off work one day on 8/7/07.

The employee was lifting a glue box when he felt pain/burning and heard popping sounds in his left hand/wrist. The accident was not deemed reportable, however, until the ee began taking off work due to complications from the injury beginning on Sept. 19,2007

Employee was removing the lid from a Fletcher Roof Bolter to replace the hydraulic pump. When he slid the lid off the machine, he experienced pain in his lower back and left leg. The employee finished his shift but later sought medical treatment as a result of this injury.

Employee picked up a belt roller and threw it into a ramcar, straining the right side of his lower back. The employee later sought treatment for this injury at an emergency room.

The EE was loading plates when he turned to throw a bundle onto the roof bolt machine and experienced a sharp pain in his lower back and numbness in his right leg. The EE worked through the following Monday (5/7/07) and began taking off work on Tuesday (5/8/07) as a result of this injury.

Injured employee and a co-worker were sliding a raw coal screen into the bed of a truck when one of the victim's fingers became caught in between the pinch-point created by the two surfaces. The incident resulted in alaceration that required stitches.

The accident victim and witness were in the process of building cribs, when the witness threw down a crib block while the victim was attempting to pick up one. The block struck the victim's right hand (index & middle fingers) resulting in a laceration that required stitches.

Employee was installing test fittings in a wheel loader when he felt pain in the right side of his back. He initially thought he was getting a kidney stone. The pain continued to get worse throughout the remainder of the weekend.

EE strained his right knee while crossing the section tail piece.

Truck got stuck, picked up rocks to put under the tires of the truck, hurt right wrist

The injured EE was changing the discharge roller on the #2 head drive. When he removed a bearing off the roller, it fell onto the middle finger of his right hand, resulting in a fracture.

The employee twisted his knee exiting the car ram.

EE was shoveling coal onto the belt in the raw coal tunnel when he experienced pain in his neck and back.

Employees were pulling wood and rocks from the raw coal crusher in an attempt to unclog it. The injured ee later experienced back pain which did not subside. He sought medical treatment for this on 2-11-05.

THE EMPLOYEE WAS REPLACING A SIDE PLATE ON THE COARSE COAL SCREEN WHEN THE SCREEN SHIFTED, PINCHING HIS LEFT HAND AGAINST THE SIDE PLATE.

TAKING STEP OFF EXCAVATOR. STEP BROKE, CAME BACK AND HIT EE IN STOMACH AND GROIN.

EE WAS LIFTING A MAT CHAIN RIPPER PAD ONTO THE MAN BUS AND EXPERIENCED PAIN IN THE LEFT SIDE OF HIS LOWER BACK.

Employee said he was moving miner cable when he turned and twisted his left knee.

EMPLOYEE WAS PULLING STARTER FROM A D-10 DOZER, FRACTURED MIDDLE FINGER ON LEFT HAND.

EMPLOYEE WAS INSTALLING SPIRALS WHEN HIS LEFT HAND GOT CAUGHT BETWEEN THE SPIRALS AND FRAMEWORK. EMPLOYEE RECEIVED A CUT REQUIRING 10 STITCHES.

HE CLOSED THE DOZER DOOR ON THE LITTLE FINGER OF HIS RIGHT HAND (FRACTURE).

EMPLOYEE WAS LIFTING AUGER OUT OF TRUCK BED AND BACK BEGAN TO HURT.

EE WAS LIFTING A BUCKET OF OIL WHEN HE FELT A SHARP PAIN IN HIS BACK.
