Rock was stuck between grease truck back tires, mechanic removed rock and a knot formed on inside tire. Employee walked over to examine knot on tire when tire blew resulting in debris blowing in both eyes

Operator was pulling draw rock off top of miner when rock slipped off machine resulting in a cut to palm of left hand.

EE was attempting to drive onto drill bench when EE hit a bump in the road, causing EE's head to strike ceiling of truck cab. This action resulted in an injury to EE's neck.

Employee dismounted dozer to check an oil leak. When EE attempted to climb onto it again, foot slipped. To avoid falling, EE attempted to catch self. As a result, EE experienced a sharp pain in left shoulder. EE began missing work on 4/26/16 due to this injury.

Employee twisted Left Knee mounting a dozer. He sought medical treatment for his injury on 2/29/2016 & began missing work the same day.

Hearing Loss

Employee stated that he slipped dismounting a rock truck, injuring his shoulder. He claims to have a witness & to have reported incident to his foreman, who has no record of it. Employee also claims that condition worsened over time, until he first sought treatment for his injury on 2/3/16, at which point he was taken off work.

Employee was assisting in cleaning a screen. When handing a water hose to his co-worker, his feet slipped out from underneath him, resulting in a fall.

Employee was exiting equipment when he slipped, injuring his back.

Employee was assisting w/ the installation of a new sieve & strained his groin area. He hoped it would improve over time & when it did not, he sought medical treatment for a hernia on 5/20/15.

Employee was descending a flight of stairs when he fell, sliding all the way to the bottom and resulting in injury to his lower back.

Employee stepped into a hole after exiting mechanic's truck, injuring both knees. He began missing work on November 10th, as a result of this incident.

Truck driver tried to downshift while going downhill and did not get the truck in gear, then the truck ran down hill and turned over on private property approximately 1000 feet away. The driver bumped his head requiring stitches.

Employee was using a pry bar in an attempt to remove the hub from a water truck when he experienced lower back pain. As a result, he began missing work on 9/9/14.

Employee was lifting a sample bucket out of truck bed when he experienced pain in his shoulder & arm.

Victim was cutting open a seed bag with a pocket knife. The pocket knife glazed through the seed bag and the middle portion of his left forearm. This laceration resulted with twelve sutures. This man returned to work the following day.

Hearing loss - Premier Elkhorn Safety Dept personnel were notified that a former employee had filed a hearing loss claim.

Employee fell stepping out of the Raw Coal Control Room, injuring his Right knee.

TRIPPED ON STEP LEADING INTO GUARDSHACK

A rock became lodged in the auger tube. When the employee pried the rock loose, bolts securing the auger also loosened, causing the auger to fall from the sample tube. The employee suffered crushing injuries to his right arm as a result of this incident.

Employee was operating a rock truck when he encountered rough terrain, causing the truck seat to bottom out, resulting in neck and back pain. The employee began missing work on August 5, 2013, as a result of this injury.

Employee was using an impact gun & socket. When a pin came out of the socket, it struck the palm of employee's Left hand, resulting in a puncture wound that required suturing.

Hearing loss - Premier Elkhorn Coal Company Safety Dept. personnel was notified on 9/23/13 that the former employee had filed a hearing loss claim on 7/18/13

Premier Elkhorn Coal, LLC safety dept. was notified that a former employee filed & settled a hearing loss claim.

Employee twisted his left knee mounting a mechanic's truck.

Employee turned over his Right ankle after dismounting drill while walking across the drill bench. This action caused him to fall, and in turn, fracture his Left ankle.

Employee went to retrieve filters for a dozer from the parts trailer. Upon exiting the parts trailer, the steps gave way with the employee, causing him to fall to the ground, resulting in a fracture to his right foot.

At approx. 3:50 p.pm smoke was detected coming from a drill (Unit #5025) after mechanics completed repairs on it and left it unattended on the drill bench around 3:30 p.m. Dorton Volunteer Fire Dept. responded and the fire was extinguished by 4:30 p.m.

Employee was replacing a blown hose on a CAT D-11 Dozer and had to drop the front belly pan in the process. The belly pan was supported by a strap which became fouled, creating slack, and allowing the belly pan to drop 6" - 8". The belly pan struck the victim in the head, resulting in a laceration that required suturing.

Employee placed his left hand into an air-conditioning vent and was subsequently struck by the fan, resulting in a laceration to his middle finger.

Employee was lifting blocks in front of dozer blade to prevent against motion when he injured his lower back. He began missing work on 7/11/2011 as a result of this injury.

Removing chain hoist, chain slipped down and caught finger cutting off tip of left hand ring finger. Employee began missing work as a result of this injury on 01/25/11.

Employee did not report the incident until 2/2/2011.The employee went to the doctor complaining of respiratory problems.The doctor asked if he had been exposed to any chemicals and the employee said "another employee was cutting a bearing out of a pin bore when the grease caught fire.When a fire extinguisher was used to put out the fire, he breathed some of the chemical".

Employee chose to use bathroom beside the dozer blade. Employee was not wearing a hard hat. Rock fell from blade of dozer and struck employee in head, resulting in laceration that required stitches.

At 9:50 AM on 10/27/10, an unintentional roof fall was discovered during a pre-shift examination while an MSHA inspection was on-going. The fall occurred at Spad #165 on the belt. It measured approximately 25' L x 20' W x 6' H.

An unintentional roof fall was discovered during an MSHA inspection. (This was the second of three roof falls originally reported on 10/22/10). The fall occurred between #4 and #5 entries at Spad #111, measuring approximately 6' H x 20' W x 60'L.

Three unintentional roof falls were discovered during an MSHA inspection. The first occurred at the #5 entry at spad #83 (measuring approximately 6' H x 20' W x 50'L).
