Mostrando los 29Employee was pulling a shuttle car tire up into a trailer when he felt pain in his upper right arm. Doctor took him off work on 7/14/15.
Employee was helping to change ram jack on scoop and had set it in place with a lo-trac on 2 crib blocks. A repairman was putting hoses on it when he decided to move it to give more room to hook the hoses and it tilted off the crib block mashing his right hand resulting in a fracture to his ring finger.
Employee was taken to a doctor on 2/25/2015. As a result of repetitious lifting of his job duties, a doctor has diagnosed him with osteolysis. As a result of the occupational illness he started working restricted duty as of 3/3/2015. **It has been determined that the EE had a detached bicep tendon. Changed from an occupation illness to an injury.
Employee was finishing stacking a brattice. He lifted 2 blocks (6") and turned and felt pain in his lower middle back extending down through left leg. He has continued to work and started physical therapy on 12/11/14.
While taking curtain down, turned and hit rib cutting right ear resulting in stitches.
Employee was bending a 5' roof bolt when his hand slipped causing him to extend forward straining his lower back.
While installing 20' long panels on an overcast 4-breaks outby #9 drive, all five crew members were picking up panels and sliding them over the overcast walls. Two contractors were pushing a panel into place and employee could not get out of the way. The panel caught his left hand and middle finger. This resulted in ee left middle finger being amputated just below the knuckle.
Employee was lifting a piece of structure that was stuck to another piece. This resulted in a strain to his lower back. He continued to work. His first missed shift was on July 24, 2013.
Employee was on his knees shoveling belt. The conditions were wet and muddy. Employee strained his back while shoveling. Employee continued to work until Dr. took him off work on 01/30/2013.

EE had installed a bottom roller on offside of the belt. While crossing back over the belt he placed his left hand on the structure to step off the belt. EE felt a sharp pain in his elbow when stepping off of the belt.
The individual was loading crib blocks in the scoop bucket when he caught his left ring finger between the crib block and the scoop bucket. The Individual is a contract EE (Bates Contracting) This is a medical treatment only. There were no lost time or restricted work days.
The individual was unloading bottom belt rollers from the three wheeler, when he felt a pain in his back.
Individual was pulling slack up in mine cable and ran into a bolt hanging down from the top hitting the back of his neck on the bolt.
The injured and another miner were setting roof jacks along the haulway at SS #888. When the other worker took a jack from the jacks piled along the rib, another jack fell on the left foot of the injured.
Drill steel stuck in roof top and bent. When employee reached up and pulled on drill steel, it broke hitting employee's thumb.
EE was in the process of installing a roof bolt. He drilled a hole, inserted the resin and bolt into the hole. Due to mining height, the bolt had been bent prior to inserting it in the hole. He inserted the top part of the bolt in the hole and as he pushed on the bolt to straighten it out, He strained his back.
EE WAS UNLOADING ROOF BOLTS OFF OF THE SCOOP AND PLACING THEM ON THE ROOF BOLTING MACHINE. EE STRUCK HAND AGAINST A METAL STRAP WHICH WAS ON THE BOLTING MACHINE, RESULTING IN A LACERATION WHICH REQUIRED 4 SUTURES
EE STRAINED HIS BACK WHILE LOADING SEAL BLOCKS.
EE ALLEGES HE HURT HIS BACK WHILE HANGING SHUTTLE CAR CABLE ANCHOR.
EE WAS LOADING BELT STRUCTURE AND INJURED BACK.

EE WAS LOADING BELT STRUCTURE, BENT DOWN & FELT BURNING PAIN IN RIGHT KNEE. EE SAW A PHYSICIAN ON 11/28/01 WHO DETERMINED THAT THE INJURED HAD ATORN CARTILAGE.
EE WAS TAKING LID OFF OF DRILL & EXPERIENCED PAIN IN LOWER BACK.
EE HAD CUT TIE WIRE TO HANG MINER CABLE WHILE HOLDING TIE WIRE IN HAND A PIECE FLIPPED UP STRIKING EE IN RIGHT EYE
EMPLOYEE WAS SETTING TIMBERS ON THE BELT LINE WHEN HE WAS LOADING TIMBERS THEY FELL ON TOP OF HIS RIGHT FOOT JUST ABOVE STEEL TOES.
THE INJURED WAS HAULING SUPPLIES WITH A BATTERY POWERED SCOOP. HE STOPPED AND GET OUT OF THE SCOOP TO OPEN A SET OF DOORS. WHEN HE RETURNED TO THE SCOOP DECK IT APPEARS THAT HE ACCIDENTLY STRU CK THE STEERING CONTROLS WHICH CAUSEN THE MACHINE TO PARTICULAR THE INJURED'S LEGS WERE CAUGHT BETWEEN THE MACHINE AND THE DOOR.
