Roof fall in the intake escape way inby No.4 belt drive, modified to restore power to No.4 Drive & to support both sides of the fall. Fall 8'-12' thick occurred at survey station 133 #4 intake entry #2 northwest mains. Roof is supported with 72' point anchor pattern bolts, 8' super bolts & 10' able bolts. Fall dangered off work, travel , power & airflow has been redirected.

A roof fall was discovered, without injury, at break #106 on the 10 bolt, approx. 19' wide x 30' long x 5' 8'thick. Appears to be a rider seam 6" above fall. Area had been supported by 5'resin grouted bolts on pattern.

EE was installing rib bolt when drill steel came out of chuck. EE reached to position steel & hit the rotation lever instead of the feed lever. EE caught a finger between steel and spinning drill head. The skin at tip of finger is torn off. EE had surgery on finger & they removed the bone at tip of finger to repair skin around end of finger. EE is off work & will have lost time.

Employee was shoveling belt line on section, employee stepped on loose rock & twisted left knee. Employee's initial treatment was at the ER & was instructed to follow up with employee's physician. Employee has loss time, first day missed was 07/09/2017 and is still off.

Injured 6/20/17 pulling cable by hand & cable was entangled & jerked employee. EE did not receive treatment until 11/24/17. EE did not miss any work until 11/27/17 & became reportable 11/24/17.

While running cable employee was lying on is back, a piece of draw rock fell out 8"thick 2'x2', striking right upper leg. EE received a small cut & bruising. No stitches. EE has loss time.

While bleeding water line, it came apart striking shoulder. This was a notice only claim until 12/4/17 when ee began missing work.

The 3rd shuttle car out of the lift from the block was being loaded when the cars started loading. The miner back out of the place as the MRS were being moved. A slicken sided sandstone slip fell catching the MRS. The fall was approximately 60'L x 18'W x 8'T, wedge shaped

First lift was being mined from crosscut between No. 5 & No. 6 entries. The pillar lift cut through into the gob, a high angle slip fell out of the lift & acres the entry above the anchorage of roof supports. The fall which was about 12' wide & over 10' high blocked 2 mobile roof supports. Roof supports were recovered by tramming them out No. 6 entry.

Employee was helping change scoop batteries when the batteries slid & caught left hand between the battery case and the frame of the scoop. It fractured, crushing the middle finger on left hand. Employee was transported to hospital & surgery was performed, received pins & screws in finger & hand was put in cast. Employee was release from hospital to go home.

The weekly examiner was traveling the bleeder inby the gob when examiner smelled smoke, then looked for the source found no fire in the pillar line. Examiner could see no flame. Alerted all employees, evacuated mines. Two inspectors from the Norton office were on the section at the time as of this time the cause has not yet been determined.

Employee was pulling cable into shuttle car, cable was tied off with tie wire. Tie wire broke, employee fell backwards catching self with left arm, causing pain in left shoulder. Employee did not starting missing work until 08/23/2016.

Unplanned roof fall discovered @ #3 entry,#2 northwest mains @ survey station 128, measured 20'W x 60'L x 11'H. The fall blocked secondary escapeway until the area was timbered off & life line was routed around the fall. Area was rebolted with 6' resin grouted bolts & 10'cable bolts. Brows on the inby/outby sides of the fall were supplement with 3 rows of 10' cable.

The event occurred on the MMU 002 in the 4A N Mains. The continuous miner was not operating at the time & was positioned in the right barrier lift in the #5 entry in by spad #4227. The CM & the mobile roof support operator stated they heard rock falling in the gob in by the pillar line. When they turned, they saw a flash & were knocked down by a sudden inrush of air & dust.

Received a hearing loss diagnosis for employee. He last worked on 9/18/13 and was termed on 9/18/14. No % disability received at this time.

Employee was changing batteries on a scoop and was on top of battery plugging up battery bell when he leaned back on feet and heard a pop in his left ankle. This is being generated pursuant to the Part 50 audit citation #9092165 issued on 2/24/2016. This employee was termed on 5/15/15 due to a reduction in force at this mine.

Employee was helping to change ram jack on scoop and had set it in place with a lo-trac on 2 crib blocks. A repairman was putting hoses on it when he decided to move it to give more room to hook the hoses and it tilted off the crib block mashing his right hand resulting in a fracture to his ring finger.

Received a CWP claim for employee.

Employee was taken to a doctor on 2/25/2015. As a result of repetitious lifting of his job duties, a doctor has diagnosed him with osteolysis. As a result of the occupational illness he started working restricted duty as of 3/3/2015. **It has been determined that the EE had a detached bicep tendon. Changed from an occupation illness to an injury.

Employee was in the process of installing a 6' bolt. As he was pushing the bolt up, the bolt started to bend and he hit his rotation causing the bolt wrench to strike him in the nose.

P&P contractor was knocking water line apart with anchor pin. The water line sprung apart catching his middle finger between the 2 joints of waterline resulting in a laceration requiring stitches.

Received a CWP diagnosis on 2/6/14. Employee was termed on 1/24/14 and is no longer employed with the company. The accident date of 1/24/14 is to accommodate online reporting per MSHA (Denver) instructions.

Received a CWP settlement on 12/18/2013 for EE.

While attempting to lower a miner cable ee foot slipped causing pain in his lower back.

The EE was operating a Diesel Low Trac Machine in the neutral entry between crosscut #28 and 29 on the MMU .001 Section. As the EE drove past a standing wooden timber, the wooden timber fell striking the EE on the left arm resulting in a break to the left arm.

On July 5, 2013, employee filed a black lung claim with the Office of Worker's Compensation, Division of Coal Mine Workers' Compensation. ***THERE ARE NO ACCIDENTS ASSOCIATED WITH THIS CLAIM.

On May 17, 2013, employee filed a black lung claim with the Virginia Workers Compensation Commission. Notice was received by our office on 6/12/2013. ***There is no accident associated with this claim***

Employee picked up a wrench out of drill tray and dropped it. He bent over to pick it up with his right hand. A piece of draw rock fell, hitting him on the right forearm. This resulted in a laceration which required 16 stitches.

Employee stepped off the front of the walk thru roof drill and twisted his left ankle resulting in a strain.

Employee has filed a claim for black lung benefits with the Virginia Workers' Compensation Commission. The Claim for Benefits was filed electronically on February 12, 2013. ***THERE ARE NO ACCIDENTS ASSOCIATED WITH THIS CLAIM.

Received a CWP diagnosis for EE on 1/17/2013.

Employee was on his knees shoveling belt. The conditions were wet and muddy. Employee strained his back while shoveling. Employee continued to work until Dr. took him off work on 01/30/2013.

MMU 001 #3 Right 2nd cut face. Employee was installing roof bolts in the last row. Piece of draw rock broke loose striking a glancing blow on his right shoulder and upper arm.

MMU001 No. 3 entry SS#3845. Employee had drill steel leaning against tray while installing a belt hanger, when he lowered the drill head the drill steel fell between the drill head and bottom causing the drill steel to bend and releasing striking employee on the right elbow causing a fracture.

MMU 001 Directly outby section power center. Employee was hanging curtain behind power center, dust fell off curtain onto his face.

mmu 001 2 cross cuts inby SS #3843 IN No. 1 entry. Employee was pushing a roof bolt up into hole manually straining neck/shoulder/back.

Employee was trying to clean off the top of a belt sequence sensor while the belt was in operation. The top roller caught his finger and pulled the tip of his finger between the sensor and the top roller causing a laceration that required stitches. The cut finger was on the right hand and between the thumb and middle finger.

The outby scoop operator was loading trash into the scoop bucket. When the employee went to load an empty pallet into the bucket the pallet mashed his left ring finger between the scoop bucket and the pallet. He was located at the designated trash hole.

The employee was pulling miner cable when he felt a strain in his right shoulder.

After employee drilled a hole for the roof bolt he was installing the glue in the hole when a piece of draw rock 18"x12"x3" dislodged from the roof striking employee on right shoulder, scraping right side of lower back.

Received a CWP diagnosis on 4/30/2014. Employee was termed on 3/13/2012 and is no longer with the company. Accident date of 3/13/2012 is to accommodate online reporting per MSHA (Denver) instructions.

Employee set t-bar against top. Piece of sandstone drawrock fell from top striking canopy and boom. Rock broke into four pieces. A piece approx. two ft. long by two ft. wide by one and a half ft. thick struck employee on ankle knocking him to the ground and causing a broken ankle and a small cut on the chin.

Employee was trying to get a bolt started in the drilled hole. The bolt was not bent enough to enter the hole and the employee tried forcing the bolt with his hands when he felt something pull in his groin area.

On the 2nd day of September, a roof fall occurred outby the sections at S.S. 1851. Separations in the mine roof strata due to layers of lamination is the leading factor to this recent roof fall. The fall measured approx.; 40'Length, 25'Wide, and up to 11' in height.

On the 002 section, in the No.3 entry, EE was finishing loading his shuttle car. Once loaded, EE stuck his legs out of the car to turn around when a rock dislodged between two bolts from the mine roof. The rock struck EE's right thigh causing a fracture to his right femur.

Employee was hauling supplies. Piece of draw rock fell from top, striking employee on back of neck and shoulder. Rock measured 36" long, 6" to 12" wide and 1" to 4" in thickness. Employee complained of burning sensation in left arm. Rock fell between 2 bolts. Bolts were measured to be 38" apart in this location.

Employee was watching cable while being pulled. Rope hanger ( a small nylon rope with aluminum hooks on each end ) broke. Hanger came loose on one end and struck employee in mouth. Employee suffered laceration to bottom lip, two bottom teeth were broken off even with gum line.

Roof fall occurred in the number 4 entry, while pillaring the left block on the 002-0 MMU. The fall was approx 22' high at its highest point & 60' long & 20' wide. Two mobile roof supports were caught in the fall area. A large slip was laying at a slight angle along the #4 entry when the continuous miner was pillaring the left block the entry begin to give indications of failure.

The EE and 4 other EE's were positioning a piece of metal to be welded onto the conveyor belt tailpiece dump box. The metal slipped and caught the EE's finger between the metal and the tail piece frame. This resulted in a laceration which required sutures.

A roof fall meas. approx. 18' wide X 15' long X 6' high was discovered in the #4 intake entry of the west mains area between survey station # 110 and 118. The intake was routed around the fall. The fall will not be cleaned up. There were no injuries involved.

Employee was stepping up on the center of the roof bolter and slipped backwards. He stated he fell backwards into a portion of the T-bar and when he caught himself he felt something pop in his back.

Employee was operating Lo Trac and said he twisted around to look under forks and felt a sharp pain in the middle of his back. He returned to work the next day but started missing work on February 1.

The individual was roof bolting and misplaced a 5' piece of drill steel. The drill steel lodged in a fan bracket and when the individual swung the drill head out the drill steel flew up and struck the employee in the mouth. No medical treatment has been sought by the individual.

During roof bolting operations, the individual states he had his hand on canopy handle and when he swung the drill head out, the canopy came in contact with the roof mashing his left ring and pinky fingers.

EE stepped off roof bolter into a water hole where there was an area of uneven bottom, twisting his left ankle and knee.

Individual was assisting in moving miner cable from outby rib to inby rib. He said when he pushed on the cable he felt something pulling in his back.

As the employee was operating a roof bolter, a piece of draw rock fell from the roof striking the individual on his right forearm causing a laceration. The individual went to the ER and the laceration was closed by gluing. This is a medical treatment only. There were no lost or restricted work days.

A roof fall was found on 7/24/10 at approx 3:00 pm in the #6 intake entry approx 100' outby SS 2197. This was a continuation of a previous roof fall. The fall did not cause any injuries & did not affect ventilation; and did not cause entrapment. The fall will not be cleaned up and the area was breakered off.

The EE was walking through the cross cut between the No. 1 and No. 2 entries on the MMU 002-0. He stepped in a hole and sprained his left ankle.

A roof fall was discovered in a return entry of 2 left off B west. Fall was between survey stations #2723 and 2730. The fall was approx. 20' X 30' X 6' in height and will not be cleaned up. There were no injuries or equipment involved and ventilation was not affected.

Individual was loading belt structure in the scoop bucket when he felt something pull in his back. He went to the ER the next day. The individual is a contract employee with Bates Contracting

On 5-4-10, a letter was received from MSHA notifying Dochester Enterprises Inc that indicated the individual had been diagnosed with pneumconiosis(black lung).

A rib rolled out onto the lower legs of the individual causing a fracture of the left ankle.

A roof fall was discovered in the No 2 entry of 1 right off No 2 north at survey station No 1847. The fall measured approximately 40 feet wide (intersection involved) x 50 feet long x 6 feet high. There were no injuries or equipment involved. Ventilation was not affected and the passage of men was not affected.

The injured was using a bar to clean loose coal away from the surface belt drive. The bar was caught in the roller and pinned his hand against the lower belt. This resulted in abrasions and lacerations to his right hand and fingers

EE was assisting in the removal of the canopy from a shuttle car, when the bolt was removed from the canopy leg, the canopy fell catching the tip of his left index finger against the frame of the machine, this resulted in the amputation of the end of his finger, the Dr was able to reattach the end of his finger.

A non-injury roof fall has occured on the 001-0 active pillar section while mining in the No 3 left panel off the No. 1 North. The roof fall occured after mining was finished catching the No 1 MRS. The mobil roof support was recovered and moved to the next block. The fall was at survey station No. 2086.

A roof fall has occurred in the No. 4 Entry of 1 right off of 1 north at spad No. 2197 and measures approximately 20 ft wide x 60 ft long x 10-12 ft high and involved an intersection. There were no injuries or equipment involved & the fall did not affect the passage of men or equipment or ventilation.

The injured was installing the last row of roof bolts in place when a piece of coal fell from the rib and struck his right knee.

The EE was servicing the roof bolting machine. He was positioned on the left side of the machine, when a piece of coal rolled from the rib and struck him on his lower back. This resulted in a bruise to his lower back area.

The EE was driving a rubber tired personnel carrier, when he ran over an object in the roadway. This caused the steering wheel to spin fast. The steering wheel caught his left hand, resulting in a fracture.

The employee was hanging the power cable and water line for the continuous mining machine and strained his back.

The employee was positioned behind the continuous mineing machine, which he was operating by remote controls, when the coal rib rolled and a piece struck his left foot.

The individual was walking down the mine entry when he stepped on a piece of loose coal and twisted his left knee.

The injured was drilling a roof bolt hole when the drill steel bowed out and struck his left hand. This resulted in a fracture to his hand.

The individual was walking down the mine entry when he tripped on a rock which caused him to fall. He struck his left knee on the mine floor.

Pillars were being mined on the b-left off 2 left off no. 1 north panel. a lift was being mined from the outby end of the block from the 3 right crosscut (1 break inby survey station 184) when a rock fell catching the head of the continuous mining machine due to the soft mine floor the machine could not back out of the place, a retriever was used to free the machine.

The injured was drilling a rope bolt hole when a small piece of draw rock fell and struck him on his right arm. This reaulted in a laceration which required sutures.

The miner was pushing a roof bolt up into the roof bolt hole when he strained his lower back. The miner did not start missing work until 8-14-07.

The employee was loading supplies onto the roof bolting machine when he stepped on an object and twisted his right foot.

A roof fall has occurred in no. 3 entry of no. 2 left off no. 1 north section. The fall is located 1 break inby spad 1801 and measures approximately 22' wide, 25' long & 8' - 10' high and involved an intersection. No injuries or equipment were involved. The fall did not affect the passage of men or ventilation.

A roof fall has occurred in No. 7 Entry of No. 2 Left off No. 1 north section. The fall is located 1 break inby spad No. 1737 and measures 20' wide, 20' long, 6-8 ft high and involved an intersection. There were no injuries or equipment involved. The fall did not affect the passage or men or ventilation.

A roof fall has occurred in No. 4 entry of No. 2 left off No. 1 North Section. The fall is located 1 break inby spad no. 1721 belt entry, and measures approximately 22' wide x 22' long x 6-8 ft high and involved an intersection. There were no injuries or equipment involved. The fall did not affect the passage of men or ventilation.

The 2nd lift was being mined from the left side pillar located out of the no. 3 entry of no. 2 west mains (1 Break inby survey station 512). the fall measured approx. 20' long x 20' wide x 5' high and caught 2 mobile roof supports mrs. the mass have been recovered and the section has moved from the no. 2 west mains panel. There were no injuries.

The injured was advancing the line curtain when a piece of draw rock fell from between roof bolts and struck the top of his foot. This resulted in fractures to his four little toes on his left foot.

A roof fall has occured in no.4 entry of no a left off 1 left section. The fall is located 1 break out by spad no 1568 and measures approx. 22' wide x 22' long x 12-14 ft high and involved an intersection there were o injuries. There was 2 pieces of equipment involved. The fall did not affect the passage of men or ventilation.

The injured was installing roof bolts, when a coal rib rolled and struck him on the back of his right leg.

A roof fall was discovered in the No 5 intake entry of 1 left off of 1 north. The fall is located 1 break outby spad no. 1504 & measures approx. 19 ft wide, 22 ft long x 10-12 ft high and involved an intersection. There were no injuries or equipment involved. The fall did not affect the passage of men or ventilation.

Injured was drilling a roof bolt hole when a piece of draw rock fell catching his left index finger against the roof drill pot. This resulted in a laceration which required sutures.

THE INJURED WAS OPERATING A REMOTE CONTROLLED CONTINUOUS MINING MACHINE. HE HAD MINED THE RIGHT SIDE OF THE FACE AND WAS STEPPING THE MACHINE OVER TO MINE THE LEFT SIDE. AS HE WAS MOVING THE MACHINE, A PIECE OF DRAW ROCK FELL (BETWEEN THE RIB AND RIB BOLTS) STRIKING HIS LEFT LEG.

A right to left sequence of pillaring was being used on the 4-LT off a west panel. The first lift was mined from the no. 2 pillar and the mining machine backed out of the lift. The two MRSs were being positioned for the next lift when a rock fell thru the 1st lift and across the entry in front of the MRSs. The fall was 25' long, 4' wide & 8' high & located 50' inby SS 1307.

A roof fall occurred on the 3 left off southwest mains section (mmu 001-0. The fall was located in the No. 3 entry, 1 break, outby survey station No. 1150 and measured approximately 20' long x 20' wide x 6' high (intersection). There were no injuries or equipment involved. The fall will not be cleaned up.

A ROOF FALL WAS DISCOVERED IN THE NO. 5 ENTRY OF THE 3-LEFT OFF A WEST PANEL. THE FALL WAS LOCATED IN AN INTERSECTION, ONE (1) BREAK OUTBY SURVEY STATION NO. 1171. FALL MEASURED APPROX. 5-8' HIGH X 15-20' WIDE X 30' LONG. VENTILATION & THE PASSAGE OF PERSONS WAS NOT AFFECTED. THERE WERE NO INJURIES, NO EQUIPMENT INVOLVED. THE FALL WILL NOT BE CLEANED UP.

A roof fall occurred int he no.4 entry of no.2 west mains outby survey station 640. catching the conveyor belt. the fall measured approx. 45' long x 16' wide x 6' high. the area was bolted with 5' fully grouted tension rebar and supplemented with 8 bolts. There was no other equipment or injuries involved.

The injured was operating a shuttle car and ran into a hole in the mine floor. The shuttle car bounced and the jolt resulted in bruising and a sprain to his right hip. He continued to work his regular job until June 14, 2005 when he started missing work from this injury.

A ROOF FALL WAS DISCOVERED IN THE #5 INTAKE ENTRY OF #2 WEST. THE FALL IS LOCATED 1 BREAK INBY SURVEY STATION #425. APPROX. 18'W X 25'L X 12'H & INVOLVED AN INTERSECTION. THERE WERE NO INJURIES OR EQUIPMENT INVOLVED. THE FALL DID NOT AFFECT THE PASSAGE OF MEN OR VENTILATION.

EMPLOYEE WAS INSTALLING ROOF BOLTS IN #5 HEADING. HE WAS HOLDING HIS DRILL STEEL IN LINE TO START A BOLT HOLE WHEN HE STARTED DRILL ROTATION, THE STEEL CAUGHT HIS GLOVE AND TWISTED HIS LT. HAND, FRACTURING HIS LEFT THUMB.

The injured was preparing to drill a roof bolt hole. The jack was set on a small piece of rock and as the canopy was raised the rock broke. This caused the drill head to suddenly drop about 4 inches striking him on the head.

Employee had pain in lower back while bending roof bolts to be installed.

EE WAS INSTALLING ROOF BOLTS. WHILE PUTTING HEAD OF BOLT IN DRILL CHUCK TO PUSH BOLT UP AND TIGHTEN, HE SPUN BOLT CATCHING HIS GLOVE ON PLATE AND JERKING HIS RIGHT ARM.

INJURED WAS BOLTING #2 WHEN A ROCK MEASURING APPROX. 3'LONG X 2'WIDE X 1"THICK FELL ON THE OPERATORS CANOPY & SLID OFF & HIT OPERATOR ON SIDE OF HEAD & RIGHT SHOULDER.

THE EE WAS WALKING THRU AN AREA THAT WAS WET AND MUDDY. WHEN HE PULLED HIS FOOT UP OUT OF THE MUD, HE FELT PAIN IN HIS LOWER BACK AND IN CALF OF LEFT LEG.

EE WAS AT 7 ENTRY SPAD 318 MOVING PUMP WHEN HE SLIPPED AND FELL, CATCHING HIMSELF WITH HIS LEFT HAND TO BREAK THE FALL.

ON 3-19-03, WHILE ON VACATION, THE INDIVIDUAL CALLED THE MINE AND ALLEGED THAT HE STRAINED HISBACK WHILE MOVING A ROCK OFF THE CONT MINING MACHINE WITH A SLATE BAR ON3-13-03. PHYSICIAN RE LEASED HIM FROM WORK AND HE RETURNED FOR A COUPLE OF DAYS. PHYSICIAN DETERMINED HE HAD DEGENERATIVE DISC DISEASE.

A ROOF FALL OCCURRED IN THE LAST OPEN XCUT, LOCATED BETWEEN THE NO. 4 AND 5 ENTRIES OF THE MMU 001-0. THE FALL WAS APPROXIMATELY 30' TO THE RIGHT OF SURVEY STATION NO. 159 AND MEASURED APPROXI MATELY 16' LONG X 18' WIDE X 8' THICK. THERE WERE NO INJURIES OR EQUIPMENT INVOLVED. VENTILATION WAS NOT AFFECTED.

INJECTING PUR INTO ROOF, OBSERVING INJECTION CREW. EE WAS 14-14 FT FROM INJECTION POINT WHEN A ROCK FELL FROM 10 FT WEIGHING 25 LBS. EE WAS HIT ON THE HEAD AND RIGHT LEG.

EE WAS IN A BUCKET TRUCK MAKING CONNECTIONS FOR POWER ON POLE MOUNTED TRANSFORMERS, USING POCKET KNIFE WHEN THE POCKET KNIFE SLIPPED CUTTING EE'S LEFT INDEX FINGER.
