Holding grease hoses together to grease take up bearing. Hoses came apart and grease from pressure gun went into right ring finger.
Employee raised the lid on the battery box to check the hinges, battery leads and to check for combustible materials. He failed to insure the lid was locked and the lid fell breaking a bone in his hand.
EE was working underground at Alex Energy - Jerry's Fork Mine with another ee. EE was removing a bolt structure from the belt scale, and stated the he felt something pull in his back, as well as feeling sharp pain in his lower back that shot down his right leg. As a result it made it very painful to apply pressure to his leg, especially while standing.
Pulling on a Bolt on a 993K Loader & felt something pop behind Right knee.
EE was removing ventilation curtain in the #5 intersection to move the miner to the #6 heading. The EE could not get the curtain down. He then stepped on the bottom of curtain to gain leverage. The EE did not know there was a pogo stick on the back of the curtain. He pulled the curtain down bringing the pogo stick with it. The stick struck EE in the mouth, miner chipped tooth.
Driver was in MT4204-777 coal truck and he reached between the seats to get paper towels and got finger caught in vent fan.
Setting Hydraulic valve back into the tank on a 785, valve slipped out of hand causing right thumb to strike a baffle in the tank, cutting right thumb, requiring stitches.
Employee reported that while operating a scoop he reached with his right arm to adjust the seat cushion and felt something pop in his right shoulder.
Employee was lifting an acetylene tank to place back into storage area and felt pain in his back. He continued to have pain and on 6/14/2012 he sought medical treatment and was prescribed physical therapy.
EE was unhooking 1.5 cables from crane hook while holding the safety latch the back cable came off and hit him in the face busting top and bottom lip and chipping his front tooth
Employee was installing a bent roof bolt in low coal, as he was pulling and pushing on the rod which was inserted 2' into the bolt hole felt pain and noticed pain in the abdomen. Employee was diagnosed with an abdominal hernia.
Employee was lapping belt on a mine sled on the surface, he was pulling on a loop of belt to straighten the laps and felt pain in the left knee.
EE was picking up a pressure plate for a powder truck when he felt a pain in his left shoulder, resulting in a strain/sprain.
Member was unloading 4 wide by 8 long pallets made out of 2 by 6s with 4 runners on the bottom off of a flat car into a scoop bucket. While doing so he experienced sharp pain in his lower back causing him to go to his knees with the pain. He took himself to Summersville Medical Center Emergency Room for treatment.
On 1/9/11 EE had just coupled the Add-Car Cathead and was placing into the Add-Car tray, when it snagged on his glove, causing it to land on his left hand fingers. EE completed his shift and then sought medical treatment for a broken middle finger, left hand.
Removing tailgate when a bolt broke in tailgate causing tailgate to fall unexpectedly and knocking him from elevated position on ladder and falling on him.
On 9/21/10 ee was in the process of installing a hydraulic bake steering pump in the CAT 992D loader and while winching into position the pump lodged and when it came free it struck and mashed ee right hand against the transmission mount. First aid was administered and ee transported to Summersville Regional Hsp for treatment. EE was treated and released to RTW.
EE was setting up tooling to press out the king pin on the Rock Truck and while hooking up the chocker cable to re-position the 100 ton ram sitting on the spindle the ram fell over and the cable hooked on his left thumb. First aid was adm. and EE was transported to hospital for treatment. EE was referred to a orthopedist by the ER Dr.
EE was walking to the launch and while going around the pan loader he rested his arm on the back upright. When he pulled away a sharp burr of metal cut his right forearm. Seven stitches were used to close the laceration.
On April 17, 2010 EE was rock dusting the face of No. 1 Entry by hand and struck the backside of his left knee against the battery end of the scoop. EE felt pain, however completed his shift and went to Summersville Memorial to have it checked. X-Rays revealed a sliver of metal approx 1.25" long and it was removed on 4/20/10. Stiches were required on incisions made to remove metal.
EE was in the process of manually pinning the blast shield and while co-worker was working the handle to align the holes, injured EE was pushing the pin and cut his left hand ring finger. 1st aid was administered and EE was transported to Summersville Memorial Hospital by the safety department for treatment.
Employee was removing wire cable from ripper on dozer, when sharp edge caused a laceration of the left hand.
On 4/2/2009 Employee was in the process of hooking up a 4' retrieval hook link and it slipped out of his hand and fell on his right hand middle finger mashing it against the launch pad. First Aid was administered and he was transported to Summersville hospital for treatment.
On 2/23/09, EE was in the process of dismounting the D10R Dozer he was operating, when his foot slipped off the step, causing him to put strain on his right arm and shoulder. EE continued to work and did not seek medical treatment until 3/3/09 and again on 4/13/09. The first day EE missed was for pre-surgery testing on 7/27/09.
Member busted his lip, causing him to receive stitches.
Ee was removing a 1" steel cable from the path of the d11 Dozer and did not see that a couple of strands on the steel cable were broken. The weight of the cable caused the broken strands to penetrate his glove and cut the palm of his right hand. First aid was adminstered and he transported to hospital for treatment.
Removing tooling from minor bogie pin. While setting down ram, cut right palm on metal on ram.
Security officer saw someone on mine property. As he was running to security vehicle, he felt pain in upper left leg. No fall involved.
Employee was in the process of putting a wheel cover on a 988 coal loader. While trying to line up bolts with the bolt holes on the cover, the cover slipped off and landed on his finger.
Employee was stemming holes, twisted his back.
On 8/22/08 EE was pulling on the water line that was falling out of the car when the car dropped off the pad. His left foot was positioned under the car when the car dropped off the pad, crushing his left foot.
On August 15, 2008, Injury was dismounting the 992 G Loader to re-check the oil while the tire man was finishing up changing a front tire. He was walking down the steps when he twisted his ankle and felt a sharp burning pain in his ankle. He had 3 points of contact and did not fall down.
Employee was conducting pre-trip inspection when he strained his back lifting the hood.
While putting car on, EE was putting miner cable in trough of Addacar. Car dropped off pad and his right foot was smashed/crushed under car.
On 7/14/08 EE was in process of haging a line curtain and cut his right hand pinky finger on the roof bolt plate.
Employee was removing a rock from HWM and it fell and caught his index finger between the rock and metal on the car.
EE cut a tree. It fell on a tree that was previously cut when it had been logged. A stick from the tree on the ground flew up and punctured a hole in his shield on his hard hat and went into his eye.
On the above date, employee & co-worker were working on a door latch. When he was checking the latch, co-worker shut the door on employee's finger. He was taken to S.M.H. where x-rays on finger were negative. Doctor cleaned finger tip and applied a splint.
EE was putting fork arms on the forklift. He was trying to line up pin holder, and put his finger in the hole to see if it was lineing up. The fork arms moved, cutting and smashing his left middle finger.
EE was checking a tie rod on a utility truck and got some dirt on his eyebrow. He removed his safety glasses to wipe his eyebrow & knocked dirt into his eye. He worked the 16th and 17th of Sept before going to a doctor on the 18th. The doctor referred him to a specialist on 9/22/07 and he was given time off for a scratched eye.
Picked up blocks and positioned them on the ground and lowered pump onto blocks from boom.

On April 3, 2007, ee was hanging s.c. cable in #4 entry spad # 1284 60' outby. When hanging cable, anchor slipped and hit him in mouth, knocking and breaking out one of his front teeth.
while assisting with installation of lift jack on D10R dozer, light bracket that was laying on hood fell stricking hard hat.
Employee was checking antifreeze in truck. He started taking cup off of radiator and realized it was hot. He started putting cup back on when it blew off covering him with hot antifreeze.
Employee alledges that he was replacing a hydraulic hose on a jack when he cut the top of his hand on a pin sticking out of the top of the jack. Received 4 stitches.
