Employee stated that EE swung the drill station in to set the inside bolt when a bolt laying in the tray became fouled and sprung up and struck EE in the chin.

Employee was working on installing a belt scraper on the 11C head, when the outby belt unexpectedly started. The belt transported the employee approximately 30 feet before it was stopped by a pull switch. Resulted in a fractured ankle.

EMPLOYEE WAS WORKING ON INSTALLING A BELT SCRAPER ON THE 11c HEAD, WHEN THE OUTBY BELT UNEXPECTEDLY STARTED. THE BELT TRANSPORTED THE EMPLOYEE APPROXIMATELY 30 FEET BEFORE IT WAS STOPPED BY PULL SWITCH. RESULTED IN A FRACTURED ANKLE.

Employee stated that while they were drilling the outside bolt hole with the starter steel, a piece of rock that measure 6"x 8"x 1/2" fell out and struck them on the right forearm. This resulted in a laceration.

Removed lagging from drive roller and was buffering and cleaning up drum to install new lagging grinder got caught and kicked back resulting in laceration to pinky finger on right hand and small cut on ring finger .

Each employee will now be trained properly trained on using said machine, do not use any equipment without proper guard in place

EE was assisting in making a belt splice when a piece of metal splintered off a coworker's hammer and embedded in EE's forehead.

EE stated that was striking on a chisel with a hammer when the chisel flew back and struck EE in the left eyebrow, causing a laceration.

EE stated that as EE was walking to the section EE hit their head on a fly board.

Employee stated that they reached down to move a 5-gallon oil can off the CM cable. While grabbing the can, they cut their right index finger. The laceration required 5 sutures.

EE stated that B-Bond got on EE's pants and caused EE's thighs to get irritant contact dermatitis.

Employee stated that while they were sliding the CM cable over with their right foot, they felt a pain in their right knee.

EE stated that while walking the roadway to the Continuous Miner, EE slipped and fell in the # 6 entry.

EE stated that was setting the outside bolt on the 3rd row, when a piece of rock (5"x2"x2.5") fell and struck EE on the inside of the left knee.

Employee has been diagnosed with Occupational Pneumoconiosis.

EE stated EE was riding in mantrip when the operator lost brakes and derailed. The mantrip came off track and EE hit EE's head and stated EE's back was hurting.

EE stated that EE was preparing to build a stopping and was cutting the nylon bands off a pallet of blocks with a knife. The knife slipped and cut EE's left thumb and required 4 sutures.

Employee has been diagnosed with Occupational Pneumoconiosis

Employee stated that while they were building a stopping they tripped over a block and twisted their right knee.

A roof fall was found on the main line belt at the tail of the # 1 Belt at break # 22. The roof fall was approx. 15' wide x 15'long x 5' high. This roof fall was reported to the MSHA and State Hotline. State Ref # 34-36278.

Employee rolled up a curtain in the # 5 Lt. entry and when they walked back over to the # 5 Rt. entry the inby rib rolled off and struck them on the lower left leg.

EE stated that EE was using a pick hammer to pick up a timber when a rib rolled out and struck EE in the right hand causing a laceration that required 3 sutures.

EE stated that as EE reached down to move the CM cable over a rib rolled off and hit EE on the right arm and leg and knocked EE to the ground.

Employee has experienced a MSHA Reportable Hearing Loss in their right ear.

EE was drilling a hole to set a cable bolt when placed hand on the drill steel. EE's glove got pinched between the joints of steel and wrapped EE's right arm around the steel.

EE stated that EE was dropping the waterline and when EE went to lower it from EE's shoulder a shut off valve caught EE's shirt and bibs pulling EE to the ground over EE's right ankle resulting in a fracture to the ankle.

EE stated a rock had fouled under the conveyor chain. EE got a slate bar and climbed up on the CM to pry it out. The slate bar suddenly slipped causing EE to pull their back.

Employee was walking out of security building, on the porch, as they stepped down, they fell and sprained ankle.

EE stated that when EE stepped off the flat car, EE stepped on a rock and twisted right ankle. This accident occurred 9/16/2022 but EE didn't miss any work due to this accident until 10/10/2022.

EE was operating the # 19 4-Wheeler and when it powered out and lost power on the hill leaving the portal. It then started to roll backward while EE was trying to reset the tram, but EE then steered the 4-wheeler into the bank. The 4-wheeler climbed the bank, rolled over, and pinned EE underneath it.

EE stated was unhooking a chain from the CM conveyor pan when a burr cut through EE's glove and caused a laceration to the left hand.

EE was cutting fly pad material with a knife and accidentally cut self, which resulted in a laceration to EE's left arm requiring 3 sutures.

Employee stated that they were moving the CM cable closer to the rib when they felt a pop and instant pain in right shoulder.

EE stated that EE was striking the core breaker on the continuous miner with a sledgehammer when a piece of metal splintered off and embedded in EE's nose.

On 5/9/2022 employee stated that they were pulling on a 3/4 drive ratchet trying to loosen bolts on the CM planetary hub when they felt a pain in their right shoulder. Employee didn't miss any work due to this injury until 8/13/2022.

Employee stated that they were bolting top when a rock fell out and hit the canopy. Once the rock hit the canopy, it broke and then fell back in the drill station and struck them in the left forearm.

EE stated had the palm of right hand on the end of the shovel handle while cleaning up some gob under the # 2 Belt Take-up. The shovel hit some rock and it caused the handle to jam in the palm of EE's hand and break EE's wrist.

An unplanned roof fall occurred on the # 1 Unit Section on the 1st Lt. Panel off the Leatherwood Submains 1 Break inby spad # 15415 in the Belt Entry. This roof fall resulted in a MRS being covered up.

EE stated that while EE was exiting the scoop EE's foot got caught in the compartment and caused EE to trip. EE reached out to catch self and this caused EE to hurt EE's left wrist.

EE has worked in the coal mining industry for over 40 years and EE's hearing is progressively worsening. EE has had enough hearing loss to be reported to MSHA.

EE stated that EE drilled EE's 3 ft. hole and went to push EE's stack steel up the hole when the insert broke off causing the steel to miss the hole. The steel hit the top and then bowed out, broke, and hit EE in the right side of the face.

EE stated that the Motor quit pulling so EE raised the lid on the motor. While EE raised the lid and it slipped out of EE's left hand and fell and struck the index finger on EE's right hand.

Employee was carrying two cans of hydraulic oil when one of the handles came off and they tripped over the can. This caused them to fall and strike right shoulder on the track rail.

EE was exiting the Joy 10 End Steer Shuttle Car when EE hit EE's lower leg on the door latch causing a laceration.

IE was bolting on the operators. As they were bringing down the drill pot, left foot slipped under the drill pot and smashed foot.

A Roof Fall occurred at break 29 from the #6 to #7 entry on the # 1 Unit. It is estimated to be 60 to 70 ft. long and 6 to 8 ft. high

EE was drilling a 12' hole, so that EE could install a 12' cable bolt. EE stated "I had just put the last piece of stack steel in the drill pot to finish drilling the hole," at which time EE pushed the joystick / rotation, at which time the drill pot caught EE's glove, wrapping EE's arm around the steel once. EE walked self back around the steel.

The Roof fell from the # 4 entry at the outby corners all the way across to the # 3 entry that was stumped. The Roof Fall is estimated to be 10 ft. high x 70 ft. long in the # 4 intersection and # 4 Lt. crosscut at break # 20.

Employee was operating the off side of the roof bolter, installing the inside roof bolt at which time the rib rolled off, striking the employee's left foot, ankle.

EE was striking the conveyor chain pin on the continuous miner with a sledgehammer when a piece of metal splintered off and embedded in EE's right forearm.

Employee was handling badger cable, tripped and fell causing injury to knee.

Employee was hauling track rails off the # 1 Unit with the motor and rail car when a rail shifted. EE attempted to swing the rail back over by hand when a rail suddenly rolled over and pinched EE's left middle finger between two rails on the rail car.

EE stated that while EE was changing batteries on the scoop, EE grabbed the jumper cable that was connected to the scoop plug and pulled on the cable. The jumper plug suddenly disconnected and hit EE in the mouth thus causing a chip in EE's front right tooth.

Employee was striking the Connecting link on the CM conveyor chain with a sledgehammer when a piece of metal splintered off and imbedded in right leg just above boot.

EE set the last bolt in the face of the # 3 entry, hung the curtain on the curtain on the next to last bolt. While walking back around the curtain EE tripped over the curtain and fell. EE caught self on the canopy to keep from falling but felt something pop in EE's left shoulder.

EE was using a slate bar to remove a bad roller on the top structure while the belt was still running. The slate bar caught against the belt and struck EE in the right side of face resulting in a fractured cheek bone and fractured nose.

Employee was backing the miner out to set over and tripped over a rock. Employee put right arm out to catch self but cut right hand on a rock. This resulted in six stitches to the palm of right hand.

Loading oxygen bottle onto flat car and lost EE's grip. Tried to catch bottle mashing EE's finger between the bottle and the metal part of the ratchet strap.

Loading rock dust and felt a strain in lower back

Installing corner bolt and rock fell out striking EE on the shoulder causing EE to hit head on the roof bolter supply tray.

Pulling curtain out of gob, felt pop in right shoulder.

Received a CWP diagnosis for employee on 8/22/2019.

Was helping to change batteries on motor; finger was caught in chain link as slack was taken up

Pulled 3-10' cable bolts off of back of roof bolter and carried them up beside roof bolter, felt pain in upper back. Set a row of roof bolts and pain got worse.

IE and another EE were dislodging the manifold from the water line which was hung from the top. IE was holding the manifold while chain was hooked around it and other EE was hitting it with a sledgehammer. Once the manifold broke free, the chain unhooked and the manifold unexpectedly fell on IE's hand causing IE's right thumb to hit a belt structure bolt.

Received a CWP diagnosis for employee on 5/14/2019.

Using a slate bar to shift a track rail over to install track ties and had pain in groin area.

Received a hearing loss diagnosis for employee on 9/20/2018. EE last worked on 8/2/2018.

Employee was building a stopping and left pointer finger was pinched between two solid blocks. Employee received a laceration which required two stitches.

Employee was walking on section and tripped over a cable, as a result of the fall EE received a strain to the left pectoral muscle.

Employee was lifting a rock on the side of the shuttle car to place in boom. The rock tipped up and pinched EE's finger against the mine roof resulting in abrasions and lacerations to the left index finger.

Employee was assisting in mine belt recovery; placed a hand on sheave rope and turned their head, at this time the left pinky finger made contact with the sheave pulley which resulted in an amputation of the left pinky finger at the first knuckle of the finger.

Fresh water line blew apart and struck employee on shoulder. Continued to work unit 5/10/2017. Had surgery on 5/10/2017

Claim for occupational Pneumoconiosis due to exposure to mine coal dust

Scaling rock and a piece of the rock fell and struck employee on head and neck area, EE was treated by EMT-M, transported to surface, then transported by ambulance to Summersville Regional Hospital, x-rays were negative and was provided a full release, employee worked next scheduled shift, then when to the personal doctor with continued pain and was placed off work.

The operator was notified on 7/15/2016 that the former employee (Last exposure date worked 5/2/2014) has been diagnosed with an Occupational Hearing Loss.

Employee was walking thru supply hole & slipped, falling on right shoulder. Employee sought medical attention on 7/16/2016.

Sitting in feeder using the Air-Arc in an awkward position. When EE attempted to stand back up had extreme pain in back.

On May 25th, 2016 mobile roof supports were entrapped by the mine roof in the gob area. The operator was unable to retrieve the Mobile Roof Supports. Steps to Prevent Recurrence of Accident: Retreat mining on the section was abandoned & the cause of the Incident is being evaluated. Before retreat mining resumes the Roof Control Plan will be revised to address Incident Causes.

Employee stated EE felt pain in lower back while bent over pulling slack chain out of chain hoist. Diagnosed with back strain.

Roof fall was discovered in the return #8 entry 6 belt inby survey spad 5531. The fall measured 100'longx18'widex6'-9'high. The area had been supported with 5' fully grouted roof bolts. No injuries or damage to equipment occurred due to the fall. The passage of the weekly examiner was affected. Entrances to the fall was re-supported with timbers and the travelway was diverted.

Moving supply motor and got foot caught under motor.

The operator was notified on 12/2/2015 that the employee has been diagnosed with Occupational Pneumoconiosis.

Coal rib sloughed off striking employee in the left leg.

Employee was crossing behind a shuttle car as it was being loaded. As he was attempting to step on the shuttle car cable he slipped, the shuttle car backed out and struck employee. This resulted in a fractured pelvis.

Operating shuttle car when going around corner shuttle car canopy rubbed corner shearing off piece of rock that hit him on left hand.

Turned around to move out of walkway and tripped. Fell into mantrip deck onto motor.

Bent over to pick up a bag of rock dust and had pain in lower back.

Holding grease hoses together to grease take up bearing. Hoses came apart and grease from pressure gun went into right ring finger.

Pulling on belt rubber with channel locks strained shoulder.

Employee was operating a Johnson Industries four wheeler, traveling the belt entry as he approached Break 25 on the #2 Conveyor belt, the four wheeler went across a ledge causing a sudden jolt injuring employees back.

On 12/05/2014, the mine operator was notified that this employee received a diagnosis of simple pneumoconiosis, category q/q, 0/1.

EE was working underground at Alex Energy - Jerry's Fork Mine with another ee. EE was removing a bolt structure from the belt scale, and stated the he felt something pull in his back, as well as feeling sharp pain in his lower back that shot down his right leg. As a result it made it very painful to apply pressure to his leg, especially while standing.

Employee and 2 others were laying track. They had the end of the rail on a jack, so track ties could be put on. The end of the rail came off of the jack landing on the top of Employee's foot. This caused a small metatarsal bone in the top of his foot to break.

EE was dusting the belt line, the pull behind A.L. Lee duster went down. EE removed the guarding to the belt & pulley system. EE stuck his hand into the belt system. After applying a little pressure to the belts, the system activated & pulled EE's hand into the pulleys, causing a fracture & several small lacerations to his right hand. One laceration required 4 sutures.

EE was removing ventilation curtain in the #5 intersection to move the miner to the #6 heading. The EE could not get the curtain down. He then stepped on the bottom of curtain to gain leverage. The EE did not know there was a pogo stick on the back of the curtain. He pulled the curtain down bringing the pogo stick with it. The stick struck EE in the mouth, miner chipped tooth.

EE was crossing conveyor belt at tail of #4 belt, as he was going up steps on crossover he slipped on step & stated he pulled something in his lower back. He continued to work at his regular job at full duty until 2/25/2013 & as of this date he started receiving Physical Therapy. This accident became medical reportable only on this date due to phys. therapy, no days have been lost

A roof fall was discovered in the # 7 Mains 7 Entry Break 19 (abandoned works). The fall occurred due to subnormal conditions above the 4' resin roof bolts. The fall measured 70'L, 19'W and 7'H, ventilation and the passage of men was not affected. Entrances to the fall will be re-supported with timbers.

On Tuesday 4/8/2014, the operator was notified that this employee received a diagnosis of simple CWP; 1/1. His last day working was 8/6/2012 when he was disabled by his treating physicians for an unrelated heart condition. This employee was officially terminated on 2/9/2013.

Employee trammed a scoop into the oil storage area when the canopy of the scoop struck a fire extinguisher. The strike cause the nylon rope to be torn and as the employee was getting out of the scoop the extinguisher fell from the canopy and struck the employee's right hand. Injuries consisted of a laceration requiring sutures.

A roof fall measuring 140' long, 19' wide, and 8' high was discovered in old works # 1 entry inby survey station 3265, 9 right off 8 Mains. The fall occurred due to laminated rock and coal above the 4' resin roof bolts. Ventilation, the passage of men, and machinery movement was not affected. No injuries occurred as a result of the fall.

Employee reported that while operating a scoop he reached with his right arm to adjust the seat cushion and felt something pop in his right shoulder.

A roof fall measuring 80'long, 20'wide, and 9'high was discovered in the 3 right entry off #6 belt mains inby survey spad 3026. The fall occurred due to laminated rock and coal above the 48" resin roof bolts. No injuries occurred as a result of the fall and equipment or ventilation was affected.

Employee was installing a bent roof bolt in low coal, as he was pulling and pushing on the rod which was inserted 2' into the bolt hole felt pain and noticed pain in the abdomen. Employee was diagnosed with an abdominal hernia.

Employee was lapping belt on a mine sled on the surface, he was pulling on a loop of belt to straighten the laps and felt pain in the left knee.

A roof fall measuring 20' long, 20' wide and 9' thick was discovered inby spad 3147 # 2 entry in # 8 Mains. The fall was 8100' outby the working section, ventilation, equipment, and the travel of personnel was not affected. No injuries occurred from the fall, the type of roof support in the area was 48" resin bolts.

A roof fall measuring 20' long, 20' wide and 9' thick was discovered inby spad 2938 # 2 entry in the old # #6 Mains. The fall was 3600' outby the working section, ventilation, equipment, and the travel of personnel was not affected. No injuries occurred from the fall, the type of roof support in the area was 48" resin bolts.

Employee was standing on a concrete pier installing a kennedy when he slipped from the pier pulling a muscle in his back

A roof fall measuring 18' long, 18' wide, and 7.5' was discovered 7680' from the active section in the #5 entry inby ss 3195 the old #8 belt entry. Permanent roof supports installed were 4' resin bolts. No injuries occurred from the fall. The fall did not affect ventilation, equipment or the travelway.

This area of the mine is on return air. This area has been danger off and timbered off for a while.

AN UNINTENTIONAL ROOF FALL OCCURRED AT THE FIRST RIGHT OFF OLD #8 BOLT SPAD NO. 3144. THE APPROXIMATE AREA OF THE SAID FALL WAS A LENGTH OF 50 FEET, 18 TO 20 FEET WIDE, AND 6.5 TO 8.5 FEET THICK. THIS FALL DID NOT EFFECT THE OPERATION OF THE MINE. THIS AREA HAS BEEN CRIBBED AND DANGERED OFF PREVIOUSLY BEFORE THIS FALL. THE DISTANCE OF THE FALL TO THE FACE WAS 3,440 FEET.

Member was unloading 4 wide by 8 long pallets made out of 2 by 6s with 4 runners on the bottom off of a flat car into a scoop bucket. While doing so he experienced sharp pain in his lower back causing him to go to his knees with the pain. He took himself to Summersville Medical Center Emergency Room for treatment.

Roof fall neutral entry #2 entry 3 belt Spad No. 1280-1289, this fall is an extension of an older reported fall.

On 2/4/11 an unintentional roof fall occurred. The fall occurred on Old #8 belt right return #8 entry, break # 11 &12 spad #3180. This is a continuation of an old fall reported on 10/16/10. No injuries occured from this fall. Length 100' X 19' width x thickness. Above anchorage. Support 4' fully grouted bolts.

An unintentional roof fall occurred on 12-2-10. The fall occurred on old 48 belt, #18 crosscut, #4 entry (spade #2324) to #2 entry (spade #2318. 2 crosscuts our.) No injuries occurred from the fall. Length 140' X width 20" X thickness average average. Support 6" fully grouted bolts.

An unintentional roof fall occurred on 10-16-10. The fall occurred on #8 belt, #8 entry, left return at pad #3180. No injuries occurred from the fall (length 40' X width 19' X thickness 4'-6'). Support 4' fully grouted bolts.

An unintentional roof fall occurred on 6/7/10. The fall occurred on #3 belt, #6 entry - intake between spad #1668 and 1679. No injuries occurred from the fall (length 60', width 20', thickness 2 1/2') Support 6' fully grouted bolts w/timbers and jacks just inby.

An unintentional roof fall occurred on 5/11/10. The fall occurred on #3 belt mains, #5 entry, Bk #10, spad #1277. No injuries occurred from the fall (length 30'-60', width 10', Thickness 6' plus - above anchorage) (support 6' 3/4" Torque tension, 10' cable volts, and cribs).

On April 17, 2010 EE was rock dusting the face of No. 1 Entry by hand and struck the backside of his left knee against the battery end of the scoop. EE felt pain, however completed his shift and went to Summersville Memorial to have it checked. X-Rays revealed a sliver of metal approx 1.25" long and it was removed on 4/20/10. Stiches were required on incisions made to remove metal.

On March 10, 2010 EE was rock dusting w/scoop and duster. To get out of the dust, he backed-up the scoop and ran over a rock resulting in pain in his lower back. He completed his shift and went to have it checked. He was released and returned to work on his next scheduled shift. He returned to Dr on 4/19/10 due to continued pain & was placed off work. An MRI was performed.

On 3/8/10, EE was in process of removing the Miller Welder from #7 belt head. EE set the welder on some hanging cables. The welder started to fall and EE grabbed the welder w/his right hand to prevent it from hitting ground. EE heard his shoulder pop and felt pain. EE was referred to Dr where he was placed off work on 8/17/10, 1st day of work missed.

An unintentional roof fall occurred on 3/3/10. The fall occurred on old #2 unit at spad #2241. No injuries occurred from the fall (length 60' to 80', width 20', and thickness - above anchorage) (support 6'3/4" resin bolt).

On Oct 14, 2009 EE was breaking the banding on a bundle of roof bolts when the banding came up and struck his right forearm. EE was administered first aid and then went to Summersville Memorial Hospital for treatment.

An unintentional roof fall occurred on 8/24/09. The fall was on the 2nd left panel off #3 belt in #4 entry near spad #1570. No injuries occurred from the fall (6' resin and 10' cable bolts).

An unintentional roof fall occurred on 7/31/09. The fall occurred from #1 entry on #1 unit near spad No. 2236 to spad No. 2364 in #4 entry on #2 unit. No injuries occurred from the fall (3/4" 6' resin bolts).

An unintentional roof fall occurred on 7/24/09. The fall occurred on #6 entry (intake) near spad No. 2587. No injuries occurred from the fall (3/4" 6' resin bolts).

An unintentional roof fall occurred on March 3, 2009. Fall occurred on the #3 belt, cross cut between # 5 to #6 entry, break #2, Spad No. 1225. No injuries occurred from the fall. (6ft. torque tension bolts)

On 2-2-09 employee was in the process of hooking the rail car to the mantrip. While inserting the pin to couple the rail car, he caught his right thumb between the pin and the mantrip. First Aid was administered and he was transported to Summersville Memorial Hospital for treatment.

On 9/16/08, Ee was cutting steel banding from a 1000 foot roll of 48" belt. After cutting the banding, the end of the banding cut his right elbow. First aid was adminstered by cleaning and wrapping and He finished his shift. He was admitted to summerville memorial hospital on 9/17/08.

On 7/14/08 EE was in process of haging a line curtain and cut his right hand pinky finger on the roof bolt plate.

Employee had just completed bitting miner and was walking along the side of the miner when the rib rolled out on his foot. Rock was approx. 80" long, 38" wide and 8" thick. Employee was transported to the hospital with his left foot broken in 3 places.

ON June 19, 2008, EE was putting bolt in corner of No. 1 entry and corner rock rolled out, off rib, striking EE on back of his head. Rock measured apprx 24" X 12" X 12" thick.

On 6/9/08, EE was walking up No. 2 entry to fork lift chagrer and stepped on a rock in road way, twisting right knee. He was referred to a specialist on 6/16/08 and specialist took him off work on 6/17/08.

On May 7, 2008 employee was walking from section tail to head along #5 belt to shut main water coming to section off so his co-worker could install a fire valve at belt # 7. A piece of rock fell out of top hitting the belt line bouncing off striking employee in right rib cage.

On 4/18/08, EE was climbing up on miner and slipped, hitting right knee on rub rail. Note: EE was injured on the 18th of April, he finished his shift and worked on the 19th. He went ot ER at SMH on 20th. They took him off work starting the 21st.

An unintentional roof fall occurred on March 3, 2008. Fall occurred on #1 entry, across #3 belt way over in crosscut 3 to 4 Spad no. 1748 on #2 entry. Length of fall 110', width 30', height 6' . No injuries occurred from the fall. (3/4" 6', 4' resin bolts)`

An unintentional roof fall occured on 3/3/08, fall occurred on #2 entry across #3 halfway over in crossout 3 to 4 spad #1748 #4 entry. L110"xW20'x6". No injuries occurred from the fall. 3/4" 6"x4" resin bolts.

An unintentional roof fall occurred on the above date. Fall occurred on No. 2 belt, No. 2 entry, No. 9 crosscut, inby spad No. 1201. The fall measured 25' long, 25' width, above anchor point. Fall is in neutral entry - 4,000' outby working face. No injuries occurred from the fall.

An unintentional roof fall occurred on the above date. The fall occurred on 3 belt, No. 2 entry, No 8 to 9 break inby spad 1258. The fall measured 125' length, 20' width, above anchor point. Fall is in the nuetral entry 3000' outby working face. No injured occurred from the fall.

EE was hooking up a bonder to the scoop batteries. There was an arc, which burned this right hand, middle, ring, and index finger.

EE was putting fork arms on the forklift. He was trying to line up pin holder, and put his finger in the hole to see if it was lineing up. The fork arms moved, cutting and smashing his left middle finger.

An unintentional roof fall occurred on 8/15/07. The fall happened on four belt, break #11 in the left return on spad #1377. The fall measured approx 100' long X 20' wide X 6' high. The area was dangered off.

EE was moving supplies from on top of the hill to the pit area. He was climbing down the ladder on his loader; he stepped in a hole and felt a sharp pain in his left shoulder. He went to the doctor and was put on anti-inflammatory medication and put back to work on 8/21/07; he went back to see orthopedic and was put off work.

Employee was walking in front of the feeder to go help hang cable during a belt move. He slipped on a piece of rock and twisted his right knee.

On 5/9/2007, crosscuts from #1 to #3 started working. #1 Entry was cribbed off and additional roof support was installed to #2 and #3 entries. On 5/10/07 at approx 10:00am an unplanned roof fall occured above the anchorage point in the #1 entry inby. Fall measured approx 25 feet in length, 20 feet in width and from 0-6 feet in thickness.

On April 3, 2007, ee was hanging s.c. cable in #4 entry spad # 1284 60' outby. When hanging cable, anchor slipped and hit him in mouth, knocking and breaking out one of his front teeth.

EE WAS LEANING OVER TO CLOSE BATTERY LID ON LOCOMOTIVE. A LOCK ON HIS BELT TOUCHED THE BATTERY CAUSING AN ELECTRIC FLASH WHICH BURNED HIS LEG.

THE WATERLINE CAME LOOSE FROM THE SHEAR STRIKINGHIM ON THE RIGHT HAND/WRIST.

THE CREW WAS LOADING RAILS ONTO THE RAILCAR WITH THE UNDERGROUND FORKLIFT. THE RAILS WERE ON THE FORKS OF THE FORKLIFT AS EE WAS PLACING THE POST ON THE SIDE OF THE RAIL CAR. AS HE PLACED THE POST IN THE HOLE, TWO RAILS SLID BACK DOWN THE FORKS AND MASHED HIS FINGER BETWEEN THE POST AND THE RAIL.

ON AUGUST 30, 2001, EE STATES HE PULLED A MUSCLE IN HIS LOWER BACK. PART OF HIS WORK DUTIES DURING THE SHIFT INCLUDED INSTALLING A SHEVE PIN, THROWING A ROCK FEOM THE WALKWAY INTO THE CHAIN LI NE AND CRAWLING AT THE SHIELD AREA. HE ADDED NO SPECIFIC EVENT COULD BE RELATED TO THE INCIDENT.

EE WAS ATTEMPTING TO OPEN A SET OF AIRLOCK DOORSWHEN HE PULLED ONE OF THE DOORS OPEN. THE AIR PRESSURE CAUGHT THE DOOR AND CLOSED IT ON HIS ARM CAUSING THE INJURY.

EE SAID HIS RIGHT KNEE WAS HURTING HIM WHEN HE CAME TO WORK ON 7/19/01. HE ASKED TO GO HOME AND SEE ABOUT HIS KNEE. ON 7/19/01 HE WAS EXAMINED AT SUMMERSVILLE HOSPITAL.

OPERATING SHEAR TOWARD HEAD WHEN A ROCK CAME OVER THE SHEAR AND STRUCK FINGER ON LEFT HAND.

EE WAS FINISHING THE REMOVAL OF A STOPPING AFTER A SCOOP HAD KNOCKED THE CENTER DOWN. HE WAS TAKING DOWN THE ENDS WHEN A ROCK FELL FROM THE TOP HITTING HIM ON THE NECK AND BACK CAUSING THE INJ URY.

EE HAD DQUATTED DOWN AT THE TAILPIECE CONTROL LEVERS. HE TURNED TO HIS LEFT TO MOVE AWAY FROM THE AREA. AS HE TURNED AND STARTED TO MOVE, HE TWISTED HIS RIGHT KNEE.

EMPLOYEE WAS SITTING IN THE DECK TRAMMING THE ROOF BOLTER. THE BOTTOM OF THE ROOF BOLTING MACHINE DECK CAUGHT THE EDGE OF A ROCK CAUSING IT TO TURN INTO THE DECK STRIKING HIM ON THE RIGHT SHOU LDER AND BACK.

ROOF FALL OCCURRING ON LONGWALL SECTION BELT OUT BY STAGE LOADER (SPAD 524) NO. 3 ENTRY LENGTH 130 FT, WIDTH 20 FT, HEIGHT 8 FT.

EE WAS OPERATING RIGHT SIDE OF ROOF BOLTER. A PIECE OF MINE ROOF FELL STRIKING THE CANOPY AND SLID DOWN HITTING BACK OF RIGHT FOOT (HEEL).

EE HAD COMPLETED MINING IN THE FACE OF NO. 1 ENTRY AND WAS TRAMMING THE CONTINUOUS MINER OUTBY. THE TAIL OF THE MINER WAS SWUNG TOWARD THE RIB. EE WAS IN A BENT OVER POSITION BESIDE THE RIB WH EN HE WAS PINNED AGAINST THE RIB BY THE BOOM OF THE CONTINUOUS MINER.

ROCK FALL ON LEFT HAND. RECEIVED ACCIDENT REPORTON 3-28-01.

EMPLOYEE WAS PLASTERING A STOPPING THAT HE HAD JUST FINISHED BUILDING. HE TURNED AROUND TO GET MORE PLASER TO CONTINUE HIS WORK, WHEN THE STOPPING FELL CAUSING THE INJURY.

DURING THE PROCESS OF PLASTERING A STOPPING, THE B-BOND DUST GOT IN HIS LUNGS HE FELT FEVERISH AND EXPERENCED PROBLEMS BREATHING. HE WAS REMOVED FROM THE MINE. HE WENT TO THE HOSPITAL WHERE HE WAS ADVISED TO STOP WORK UNTIL HE FELT BETTER OR IMPROVEMENT WAS SHOWN.

EE WAS HELPING PULL A FLATCAR. THREW A SWITCH WITH A RAIL MOTOR USING A CABLE FROM THE MOTOR. THREW A SHIEVE WHEEL TO THE FLATCAR. THE CHAIN HOLDING THE SHIEVE WHEEL CAME LOOSE LETTING THE SHI EVE WHEEL HIT EE CAUSING THE INJURY.

THE CREW WAS MAKING A POWER MOVE. THE FEEDER CABLE WAS BEING PULLED WITH THE FORKLIFT. AS THE CABLE WAS PULLED THE SLACK TIGHTENED AND THE CABLESWUNG BACK AND STRUCK THE EMPLOYEE.

WALKING BACKWARD CARRYING ONE HALF OF A BEAM, HESTUMBLED AND STRAINED HIS LOWER BACK.

EMPLOYEE WAS HELPING CARRY A TAIL ROLLER WITH 3 OTHER MEN. HE BENT OVER TO LIFT IT THREW THE STOPPING AS HE DID SO HE FELT A POP AND A BURNING SENSATION IN HIS LOWER BACK. HE WAS TRANSPORTED T O THE HOSPITAL THAT DAY, HE WENT TO SPECIALIST AND RETURNED TO WORK ON THE NEXT AVAILABLE SHIFT.

EE WAS HELPING PUT A FLAT CAR BACK ON THE TRACK.HE WAS PUSHING DOWN ON A JACK BAR WHEN THE JACK SLIPPED. HE FELL FORWARD AND STEPPED ON A CRIBB BLOCK CAUSING THE INJURY.

EMPLOYEE WAS TRYING TO REMOVE THE R-BOLT FROM UNDER THE RIPPER HEAD OF THE MINER. TO REMOVE IT, HE BRACED HIS HAND ON THE PAN WHICH INEXPLICABLYRAISED UP CATCHING HIS HAND BETWEEN THE PAN AND THE SIDE GUARD ON THE HEAD MOTOR.

WHILE STEPPING OVER A BAG CONVEYOR WORKER CAUGHTLEFT LEG ON A PIECE OF ANGLE IRON & RECD A P[UNCTURE WOUND THE HAMMER GOT BETWEEN THE BELT & AND THE ROLLERPULLING HIS ARM & SHOULDER BETWEEN THE BELT & THE SCRAPPER ROLLER

EE BENT OVER TO PICK UP A SHUTTLE CAR CABLE AND EXPERIENCED PAIN IN HIS LOWER BACK.

ROOF FALL NO. 1 ENTRY, TAILGATE SECTION, 1 X-CUT IN BY SPAD 590 RETIRN AIRWAY.

EMPLOYEE AND CO-WORKERS WERE LOADING TIMBERS INTO THE SCOOP BUCKET. HE STATES THAT HIS BACK BEGAN HURTING WHEN LIFTING TIMBERS. HE HAD A PREVIOUS LOST TIME INJURY IN 1998 ON HIS BACK. HE STATE S THAT HIS INJURY IS A HERNIATED DISC,

ROOF FALL; BELT AND TRACK ENTRY AT SURVEY STATION 609. 35'L, 20'W, 12-14'H.

ROOF FALL OCCURRED IN THE BELT ENTRY ON THE HEADGATE SECTION FOUR CROSS CUTS OUTBY TAILPIECE. ROOF FALL MEASURED 20' LONG 20' WIDE 7.5' THICK.

EE WAS INSTALLING A ROOF BOLTY AND AFTER TIGHTENING THE BOLT HIS WRENCH WOULD NOT COME OFF. WHILE TRYING TO REMOVE THE WRENCH A PIECE OF SLATE FELL FROM THE MINE ROOF AND STRUCK EE'S LEFT HAND RESULTING IN A LACERATION TYPE INJURY.

ROOF FALL EXTENDING THRU XCUT BETWEEN NO. 1 AND NO. 2 ENTRIES AND INBY INTERSECTION OF NO. 2 ENTRY. LT 140' X WT. 20' X HT. 12'.

EE IS A CONTINUOUS MINER OPERATOR. ON 7/3/00 HE SOUGHT MEDICAL ATTENTION FOR HIS LEFT KNEE. THE INJURY WAS NOT REPORTED TO HIS FOREMAN OR TO MANAGEMENT. HE STATES HIS KNEE HAD BOTHERED HIM PRI OR TO THE EXAMINATION BUT COULD NOT IDENTIFY A PREVIOUS TIME OR EVENT WHEN THE KNEE BEGAN HURTING. HE CONTINUED TO WORK AS THE MINER OPERATOR THROUGH 8/7/00 BUT BEGAN MISSING WORK 8/8/00.

EE WAS BOLTING TOP AND AFTER INSTALLING THE OUTSIDE BOLT, HE SWUNG THE ASSEMBLY TO INSTALL THE INSIDE BOLT WHEN A PIECE OF ROCK FELL FROM THE MINE ROOF, STRIKING HIS LEFT FOREARM.

EE WAS LOADING SUPPLIES FROM THE SCOOP TO THE R-BOLTER. HE BENT OVER TO PICK UP A BUNDLE OF 5 OR 6 6' ROOF BOLTS. AS HE STOOD UP HE FELT PAIN IN HIS LOWER BACK. HE FINISHED LOADING THE SUPPLIE S. HE THEN WENT TO EAT LUNCH. HE DID N'T FEEL BETTER SO HE WENT OUTSIDE AND THEN TO THE HOSPITAL.

ALIGNING A PIECE OF BELT STRUCTURE, PIECE SLIPPED AND SO DID EE STRAINING MUSCLE IN NECK AND UPPER BACK.
