Employee was replacing the head jack on a continuous miner. While lining up the new jack, the chain hoist slipped, pinning the employee's hand between the jack and the frame of the machine. This resulted in a jagged cut to the employee's right second finger which required two sutures to close.

Employee was bending a 6' roof bolt over the T-bar when he felt his back tighten up. He continued working the rest of the shift and the next days shift. He went to the doctor on 1/27/2012 and they put him off from work until he could see his regular doctor.

Employee was loading belt structure onto a Lo-Trac by himself when he felt a sharp pain in his lower back. the employee was 13 breaks from the section when this occurred.

EE was loading supplies on section scoop when he felt a pain in his lower back on the right side that went all the way down his right leg. EE did not go to the doctor until 9/24/11. He went to see his doctor on 9/28/11. He received a full release on 9/28/11.

Employee was installing a metal roof strap. Turned to get bolt out of tray. When turning back around the strap struck employee in the forehead. Employee received a laceration which required 3 stitches.

EE was loading belt structure when he felt pain in his back. EE worked his regular shifts until 3/1/11 when he went to the doctor . The conditions were wet and muddy.

Employee was building super cribs with 7' timbers on 004 section in 5 rt break. He was building super cribs with 7' timbers and crib blocks on 004 section in 5 rt crosscut. He was helping to put a timber on for the top when he stated that he felt something pop in his upper back just below the neck.

This accident occurred on the surface. The injured employee was on a mantrip and had his left hand on the back of the seat, when another employee moved a metal mandoor loaded onto the mantrip, which resulted in lacerations to the injured employee's index and middle fingers on his left hand.

Employee was roof bolting in the #6 right break on MMU. No. 003-0 section. The employee was attempting to bend a six (6') foot roof bolt and his right shoulder popped.

Employee had opened lid on the shuttle car reel compartment and was getting ready to open the second lid when the first lid fell hitting his left arm.

Employee was unhooking duster from mini-trac when he felt a pain in his shoulder. He told several co-workers about this. This was on 7/28/10. Employee continued to work the rest of the week. On Monday, his back and shoulder were hurting so he went to the doctor on 8/2/10. He was given muscle relaxers and 2 days light duty.

Employee was putting lid back on top of the ATRS on the roof bolter when it slipped catching his ring finger on his left hand.

Employee was pulling cable up, when he come by an rib strap that was sticking out. Hitting his left ear.

EE was pulling 10/3 cable at # 8 belt. The cable hung up on something. EE jerked the cable it came loose and knocked his safety glasses and hat off. The cable also hit his right eye.

Employee was building a super crib, when a timber from about five foot fell hitting his lower left leg.

Employee was loading pallets when he twisted his left ankle.

Employee was in the surface warehouse & picked up 2 buckets of roof drill bits. When the employee turned around, he felt a pain in his back but didn't think any thing was wrong. The employee went underground and was helping hang the miner cable. The employee got sick at his stomach and his back started to hurt very bad. Employee brought outside.

EE was raising lifeline to go under it. He dislodged some dust that went into his eyes.

EE was unloading a part (platform for bolting machine) from mantrip. Platform slipped catching his right pinky finger. This resulted in a laceration requiring 6 stitches.

EE was working on overcast, EE was stacking blocks when he picked up a 8" block he turned to the left to place the block on top of a 48" overcast when he said he hurt his back. Accident occured 3 brks outby #16 headdrive. No EQ involved. EE did not slip or fall

Employee was removing safety chain from tongue of trailer. Tongue fell mashing his left ring finger requiring 6 stitches.

Employee was pulling fire hose from scoop bucket when he felt a "pop" with a burning sensation in his lower back and right side. Employee finished shift and drove himself to the doctor the following day - 7/23/09.

The EE was lifting a piece of belt that covered the suctin hose on the roof bolter. The belt covering the suction hose had some rope bolts laying on belt. When the EE started to lift the belt, he felt a pain in his lower back.

Employee was pulling on M.R.S. cable in #7 entry, when he felt pain in his back.

EE was building super cribs in intake. He was lifting an 8' timber up to another EE when he dropped it. Timber struck his left knee as it fell.

Employee was installing bottom belt rollers in belt when the employee felt his shoulder pull hurting his muscle on the right shoulder.

Employee was working at the #13 head-drive. Employee was lifting wooden crib block to another employee when he felt a pull in the upper part of the middle of back and right shoulder.

Employee was shoveling belt conveyor. He came upon a large piece of coal that was to heavy to lift upon belt conveyor. He hit the large piece of coal to break into small pieces. When breaking large piece of coal and small piece broke off and hit him in the right eye.

EE was stacking 6" hollow concrete blocks, EE turned with a block in his hand when he suddenly felt a sharp pain in his lower back.

EE was weighing down a curtain with a rock. The rock slipped out of his hand and mashed his finger.

While loading crib blocks, employee struck a metal band with his right hand resulting in a laceration to his right thumb.

EE was picking up miner cable onto rib, when he felt a sharp pain under his left shoulder blade, near center of back.

Employee was picking up roof straps and a sharp end of the strap cut his right wrist.

Employee was dragging a curtain when he strained his groin. *Employee went on work restrictions due to this injury beginning 4/22/08.

Emploee was putting oil bath on beltdriver. His right hand got caught behind bath and was cut when the bath struck the frame of the belt drive

The injured was helping set timber. When the timber slipped & fell, it struck him on the rt cheek causing a bruise & fractured cheek bone.

Employee was carrying canopy post for roof bolter when he set it down and felt something pop in his lower back. Started physical therapy on 3/2/07.

Employee picked up an empty acetylene bottle when the lid came off and hit him in the mouth.

Employee was pulling belt under beltline when he felt pain in his left shoulder.

Employee was pulling a crib block out of tailpiece when he felt pain in his rib.

Employee was moving miner cable out of roadway and strained his right side.

Employee was pulling on a rope bolt from the roof bolter when he strained his right elbow. He did not start missing work due to this injury until 10/6/06.

While pulling a can of hydraulic oil out of some mud, employees left shoulder gave way.

Employee was loading tool bag onto Mac 3 when he strained his lower back.

Employee was loading finder in scoop bucket when he caught his finger between finder & shuttle car tire.

Employee was loading roof bolts from the ground to scoop bucket and the straps that are raised to hold the bolts in a bundle caught his left arm and cut it. Employee received 5 stitches.

EE was pulling on miner cable when he strained his right forearm.

Employee was placing a piece of structure in a bucket when he stood up and felt a sharp pain in his chest.

EE WAS MOVING MINER CABLE OUT OF ROADWAY AND TWISTED LEFT ANKLE

EE felt a sharp pain in his lower back and his right groin area when he picked up miner cable and placed it on the rib of the mine.
