Stress crack, and deterioration of mine roof on #18 belt

Chain being used to pull shuttle car snapped and struck employee in the lower right leg.

Employee got off the mantrip in the outby travel way at #2 Belt drive, took a step and felt right knee give out followed by pain and tightness.

A non injury roof fall was discovered in the #3 belt line entry at cross cut #75 in the 1 west mains #4 belt line. The fall appears to be approx. 25 ft wide, 20 feet long, and 8 to 10 feet thick and is in an intersection. Neither ventilations or escape ways are effected. Area has been dangered off at all points of entry.

An extension of a previous non-injury rock fall was discovered in the intake on #11 belt break #215 of the primary escape way (wall entry). It is approx. 12' thick 30' long and 18' wide. The affected area has been cribbed and dangered off from all points of entry. Mine has been idle since April 2016.

Employee was cleaning the shop at mine location. As employee was dumping a wheelbarrow full of dirt/mud/garbage into a front end loader bucket, felt a sharp burning and stinging sensation in lower back. Employee did not feel the need for medical/first aid. Worked the next three shifts and went to dr on 12/8/16.

Top and had been supported with 48" glue bolts. Roof fall is approx. 14' thick, 60' long, 18' wide. Fall occurred in the intake air cause, between brks 2-3 of the #11 belt line. Ventilation checked inby the fall area, no change in ventilation. Note: mine has been in non-producing status since April 1, 2016. There is no active section at time of fall.

A non injury roof fall was discovered in the #1 south panel in the #6 entry at 2375 & 2386 spad App 40ft long 18ft wide and 10ft thick. Neither ventilation or primary escapeway were effected. The affected area has been cribbed off and dangered off.

Employee was loading bolt supplies on to bolt machine. Employee pulled roof bolts from bundle and twisted. Employee said his upper back popped between his shoulder blades.

Employee pulled/strained lower abdomen area while lifting/pulling safe haven.

Employee walked to the back of the walk through roof bolter to get a box of glue. While carrying the box of glue back to the head of the roof bolter, he fell in the walk way of the bolter.

Employee was helping other rib bolt machine operator get rib fencing in place to bolt the rib, when rib rolled out (coal and rock) striking the employee on left arm and body.

Contract employee (ID #5UF) smashed his right hand while exiting the scoop. He caught his hand between the scoop door and the canopy.

Employee slipped/fell at #2 head drive while pre-shifting. He bumped his knee and head on the ground.

Contract employee (Contractor ID #5UF) was riding the mantrip when it hit hole, jarring his lower back.

Employee was struck by the scoop deck door while exiting the equipment. The door struck the employee in the back of the head and neck.

Employee pulled/strained right shoulder while cleaning out a rib bolt.

Employee was walking in the #1 entry, carrying a torque shaft to the continuous miner when he fell on uneven bottom striking the tail on the miner with his mouth.

Employee was putting up a bolt with a strap when a kettle bottom fell out of the roof striking employee and knocking him down. Resulted in a broke right leg above the ankle.

Employee pulled/strained lower back while pulling a "come-a-long" to make a belt splice.

While backing up roof bolter, cable reel struck employee in the right elbow. Resulted in a pulled muscle and chipped elbow.

Employee was shocked while moving car cable out of the roadway. He was wearing rubber gloves.

Employee caught left hand in rib fence wire while installing fence on the coal rib. Twisted left wrist.

Employee pulled lower back and groin muscle while lifting crib block.

Employee was shoveling the 001 section tailpiece and hurt his lower back.

A non-injury roof fall was discovered in the intake off of #16 belt at spad number 1491.5. Neither ventilation or primary escapeway lifeline were effected due to multiple entries of travel. The fall is between pillars, not in an intersection, and in a well entry. It is approx. 4 1/2 ft thick , 25 ft length, 16ft width. The effected area is to be cribbed off.

Employee's leg was ran over by a buggy and a welding cart while holding the door open, resulting in bruising and scraping.

Employee got dust inside both eyes.

Contract Employee slipped and fell while working on #2 head drive. He twisted his right knee and hit it against the turn buckle.

Employee was holding a shuttle car seat while the repairman was welding a bracket on. The employee was shocked when the welding rod touched the frame. Note: The power was disconnected from the shuttle car at the time of accident.

Employee was pulling a pump cable. The cable hit him in the left eye causing mud and coal to get beneath his eye lid. Eye was irritated and swollen.

Contract employee (Bates Contracting) was injured while servicing the continuous miner. A piece of rib measuring 10 ft long x 40 in. tall x 20 in thick rolled from the top of rib striking employee from behind and pinning him against the continuous miner.

A non-injury roof fall occurred in the #3 entry adjacent to the #2 entry #14 beltline just inby spad#2513 at break256. The fall partially blocked the #3 intake which was the main travel way to 001 section and is the main intake to 001 section, but didn't impede the ventilation. The fall was approx. 6'H. x 18' W. and 60' L. See attached map.

A non-injury roof fall was discovered on 9/24/13. This is a continuation of a previous roof fall that occurred on 2/27/12. This fall is located in the #2 return entry at survey station #2708. It measures 20' to 30' long, 4' to 6' high and 18' wide. *See attached map #1 for location of the original roof fall. *See attached map #2 showing continuation of the original fall

At approx 2:30 pm on 8/30/13, the mine examiner found a roof fall. The roof fall is located in the #3 entry of the 1-West Mains at survey station # 2700. The roof fall is approx 18' wide x 30' long x 6'-8' high. There are no injuries associated with the roof fall. The roof fall will be dangered off. Ventilation has not been affected. The roof fall was reported to the MSHA Hot-line

Roof fall found in #6 entry off of #9 belt between spad # 1718 and #1887. The roof fall was approx. 1,250' long and 3'-12 high in places. No ventilation nor escape way was blocked. This mine has been idled and in non-producing status for approx. 7 months. There was no old workings above or below this fall.

At approx.8:40a.m.mine exam.found a rock fall on the #2 entry(return)at the #75 xcut.The fall is 2 breaks outby the #5 belt head. The fall was in the intersect.Fall is approx. 35'L. X 20'W. X 5'H. MSHA hotline was called at 8:50 a.m.Area was bolted with 4'resinbolts and 5-10'rope bolts.No vent.was affected No Eq.involved.No injuries occurred.Area was dangered off.This mine is IDLE.

On 2/22/12 a Roof Fall was found during weekly examination of the airways and was reported to MSHA. The fall was 16' long X 5' high X 20' wide. No interruption in ventilation occurred. No injuries occurred. No equipment was involved. Area was cribbed and dangered off.

A roof fall was found by preshifter during his pre shift run. The roof fall was on #14 beltline 1 cc inby spad # 2521 on 001 section. Fall was approx. 18' to 20' W by 40' Long by 5'-6' thick. No ventilation was affected. No injuries occurred. Fall will be cleaned up and re-bolted.

A roof fall was found and reported to the outside at this mine. The fall had occurred at crosscut #75 between the roadway and the beltline. MSHA 800 hot line was notified at 3:05pm. The confirmation # for the call was 1-88-296882. The dimensions of the fall are 18'wide by 12' to 15'long by 4' to 5' thick. No ventilation interruption or inj occurred.

On 4/27/11 the EE was operating a Shuttle Car and hit a hole in the roadway causing him to bounce his head on the canopy. He cont'd. to work his regular schedule until 5/24/11 when he went to the Dr. because he was still in some pain and the doctor took him off from work.

Employee was operating a mantrip. He hit a pothole and the wheel got away from him. It hit his right hand causing lacerations to his index and middle fingers. These required 5 stitches. Employee returned to work at his regular job the next day. This was not a lost time injury.

EE was stepping off the back of a walk through roof bolter down onto the ground when he stepped onto a rock with his left foot. The rock suddenly moved under his feet causing him to immediately shift his weight to his left leg, resulting in a fracture to his left ankle.

The employee went to get on the mantrip to leave the section when he slipped and fell. As the employee was falling he tried to catch himself on the mantrip resulting in a cut to the palm of his right hand. The cut required (5) stitches.

EE was loading rock dust into a wet duster, when a section of the coal rib fell off from near the roof striking the EE in the head, neck and shoulders. The EE complained about pain in his neck, back and shoulders.

Employee had opened lid on the shuttle car reel compartment and was getting ready to open the second lid when the first lid fell hitting his left arm.

EE was roof bolting in #2 entry. The EE was putting up a metal strap. As the EE started spinning the bolt the metal strap spun around hitting the EE on the right arm. This resulted in stitches being used to close the cut.

EE was trying to push up a bent roof bolt. The wrench he was holding slipped, causing drill to mash his right middle finger.

Employee was helping move pressure pump up. Co-worker was getting ready to cut rope hanger down that was holding the flex water line up, when EE walked up to help with waterline. He walked into knife cutting his right forearm. (8 stitches)

Employee was pulling cable up, when he come by an rib strap that was sticking out. Hitting his left ear.

Employee was loading pallets when he twisted his left ankle.

A roof fall was discovered on the N0/2 belt conveyor at cross cut N0/11. The roof fall is approximately 25' long X 25' wide X 7 1/2' high. The roof fall is on the belt conveyor. The roof fall area will be cleaned up and re-supported. The belt conveyor will be re-installed.

EE was bolting top in #1 entry when a piece of draw rock fell striking his left hand.

Employee was at the scoop charger on 002 section. After changing the scoop batteries, the employee started to get onto the scoop and a piece of the coal rib fell out, hitting the employee on the shoulder and left lower part of back. Strained muscle in shoulder and back.

EE was raising lifeline to go under it. He dislodged some dust that went into his eyes.

EE was unloading a part (platform for bolting machine) from mantrip. Platform slipped catching his right pinky finger. This resulted in a laceration requiring 6 stitches.

EE and witness were removing a metal Kennedy stopper. While removing the last panel a piece of draw rock fell striking the EE on the head knocking the EE to the ground injuring the EE's head, shoulder and ribs.

Employee was pulling fire hose from scoop bucket when he felt a "pop" with a burning sensation in his lower back and right side. Employee finished shift and drove himself to the doctor the following day - 7/23/09.

Employee was getting brattice jack out of scoop bucket, when he lend over bucket the scoop operator let the scoop bucket down catching his left foot.

EE was cleaning out material from under chain lid on coal feeder. While replacing the lid, the slate bar slipped, causing the lid to slide and mashed and cut the EE's right hand between the lid and the coal feeder. This resulted in having 4 stitches to the top of the EE's right hand.

Employee was taking down a metal kenny brattice when a piece of draw rock fell from top hitting his thumb and forefinger on left hand.

Employee was pulling on M.R.S. cable in #7 entry, when he felt pain in his back.

Employee was working at the #13 head-drive. Employee was lifting wooden crib block to another employee when he felt a pull in the upper part of the middle of back and right shoulder.

Three employees were working on an A.L.Lee push out car. Co-worker went to raise the front of the car up with a fork lift to change out trunnion. Wne he began to set the car back down, the fork broke off striking employee on his left leg.

While loading crib blocks, employee struck a metal band with his right hand resulting in a laceration to his right thumb.

EE was picking up miner cable onto rib, when he felt a sharp pain under his left shoulder blade, near center of back.

EE was bringing drill steel out of the hole on last bolt in place when coupling broke. The bottom piece of steel spun hitting employee in the face on the left checkbone and under the eye and mouth.

Employee was dragging a curtain when he strained his groin. *Employee went on work restrictions due to this injury beginning 4/22/08.

Employee was moving a rock from behind ram of scoop bucket when he slipped and strained his back.

Employee was rock dusting rib when a rock rolled off the rib and struck his left foot. On work restrictions for 4 days.

Employee was checking belt splices at #7 tail piece 7 breaks outby section when rib rolled out and a rock or piece of coal struck him on the left foot.

While loading shuttle car, a rock slid off shuttle car and landed on employee's left foot.

Employee was operating a miner in #3 heading when a rib rolled out. The rib struck him in the back and on his right knee.

EE was trying to extract a miner bit when his hammer splintered causing a piece of the hammer to became lodged in his right hand index finger.

While loading roof bolter, EE turned around to pick up bolts, when he felt a pull & sharp pain in left shoulder.

The injured was helping set timber. When the timber slipped & fell, it struck him on the rt cheek causing a bruise & fractured cheek bone.

Employee was pulling belt under beltline when he felt pain in his left shoulder.

While roof bolting, EE finished drilling a hole and began to drop the head. As he dropped the hesd, he grabbed the drill steel and the head shifted and caught the tip of his finger of his left hand little finger between the head and drill steel resulting in amputation.

Employee was placing a piece of structure in a bucket when he stood up and felt a sharp pain in his chest.

EE dropped a hammer and when he twisted around to pick it up, he said he strained his right calf muscle.

EE felt a sharp pain in his lower back and his right groin area when he picked up miner cable and placed it on the rib of the mine.

Draw rock fell from roof striking EE on neck and back.

While walking, EE slipped on a wet rock causing him to twist his left ankle.

EMPLOYEE FELL OFF OF LADDER WHILE HANGING CURTAIN.

INJURED WAS HELPING MOVE THE CONTINUOUS MINING MACHINE WHEN A COAL RIB ROLLED OFF THE CORNER OF THE BLOCK STRIKING HIM ON HIS RIGHT LEG AND ANKLE. THIS RESULTED IN ABRASIONS AND BRUISES TO HIS RIGHT LEG AND ANKLE

The injured was helping move the cont mining machine when a coal rib rolled off the corner of the block striking him on his right leg and ankle. This resulted in abrasions and bruises to his r ight leg and ankle.

WHILE PUTTING UP ROOF BOLTS, EMPLOYEE GOT HIS LITTLE FINGER CAUGHT IN CLAMP THAT HOLDS ROOF BOLTS IN PLACE.
