Employee was cutting plastic thread protector off of a new drill steel with a utility knife. The knife slipped and the employee cut his hand that was holding the drill steel. The cut required 2 staples in the palm of his right hand below his middle finger. An attempt was made to submit this on 2/10/12 but this system was down for maintenance

He was cutting a piece of wood with a pocket knife to secure the tube in the rod rack. the knife slipped and cut his left hand.
Employee was tempting to relocate the controls to the mixer truck from the cab to the rear of the mixer. In doing so this caused the drum to rotate. The hatch to the mixer was opened, then became stuck during rotation. Employee tried to release the hatch with a crowbar, while doing this employee fingers of left hand got smashed from the crowbar.
empolyee was chipping away on the Batch plant. hitting his right thumb.
While tightening a dewy-dag, the wrench that was being used slipped and struck the employee in the mouth
Using a cheater bar to loosen 10' HQ drill rod from the drill string. EE lost his grip, causing him to fall backwards, stiking his elbow onto standing (secured rods).
The employee was using a stone dressing tool to re-surface a grinder stone wheel. He said the tool caught on the grinder wheel and it jammed against his index finger cutting the tip of it off at the fingernail.
