Employee was trying to cut a hole through plastic siding of the prep plant. EE removed EE's gloves to open the knife and forgot to put EE's gloves back on. The knife slipped and cut a finger on EE's other hand. EE was taken to the ER and received 4 stiches to hold the wound closed. EE returned to work the next day.

Hitting a pen on a track hoe with a sledge hammer. A piece of the pen broke off and lodged in left shoulder
