Operator was using a 6' pry bar to free 5" steel balls inside of discharge kibble bin. While attempting to the pry under the 5" steel balls, the balls broke free and forced the pry bar to move in the direction of the EE's side of the right face striking EE on that side of the face.

Mostrando los 7IP was using a hammer to strike rods to loosen stuck joints when IP lost grip and control of the hammer causing it to bounce back.
IP was using chain hoist to raise sump pump when the hoist stopped. IP jerked on the chain and felt a pain in shoulder. IP continued to work the remainder of the shift and the following day. Injury was reported to operator on 5/17/23 and accepted by WC on 5/30/23.
Two employees were tasked with applying foam grout to the 2940 heading. While on a scissor deck a standpipe was sticking out and obstructing the deck. One of the employees found a ratchet tie-down strap and attempted to use it to pull the standpipe out of the way. While using the strap, the hook ends slipped and rebounded toward the employee, striking them on the left shoulder.
A mechanic was installing a bushing on the z bar of a scoop with a push plate by hitting it with a sledge hammer. While hitting the push plate with the sledge hammer a piece of the push plate flew off. The shrapnel flew and cut through the employees glove and lacerated the right hand knuckle.
The employee was installing steel in the main shaft. EE was using a hammer to pound the steel into place. The back of EE's hand made contact with a metal wall bracket causing a laceration that required stitches. The employee was wearing gloves.
The employee was working on the surface at the east north vent shaft discharge water line to fix joints in the pipe. The Employee strained their lower back while using the pipe jack to clamp together discharge water pipes. The Employee was in a crouched position when twisting and turning the jack handle.
