While trying to cut conveyor belt, employees hand slipped and utility knife cut hand requiring three stitches

Employee was cleaning under the lateral 2 head drive with a shovel when a clump of mud slid down and hit him in the knee. The mud caused a bruise on his knee. The injury is a sprained or bruised knee. He was given a brace and took 3 days off.

He was driving miner bits into the auger head of the miner, when something struck his left eye. He did not notice much discomfort and continued to work, even reporting to work the next day for his entire shift. Woke up the following day with discomfort and went to eye doctor. Tests found a sliver of metal in his left eye

While attempting to replace a splice on the 3-Rt Conveyor Belt, employee was attempting to re-cut the belt for a new splice and when closing the belt cutter, he pulled his left hand back and caught his middle finger against the belt cutter blade, lacerating the tip of his finger, requiring 3 stitches

While attempting to sledge a piece of steel into place, the sledge hammer head glanced off the piece of steel causing the handle to continue thru the swing and smashed his pointer finger between another piece of steel and the handle, causing a laceration.

While cutting a piece of shot wire, employee cut left thumb with knife.

Injury was barring the feeder when he missed the mud he was barring. The bar hit his right pinky finger up against the feeder. He had four stitches and tore the nail off.

WHILE RELEASING A COMEALONG FROM REPLACING A BELT SPLICE, EMPLOYEE HAD HIS LEFT RING FINGER (TIP) CAUGHT IN THE RELEASING MECHANISM OF THE COMEALONG, SMASHING & CUTTING THE TIP OF HIS FINGER. 7 STITCHES REQUIRED.

To bend back into place a damaged cover/guard on the section conveyor tail piece with a pry bar, the bar slipped causing him to fall backwards. Upon landing on his back, he landed on a pair of vise grip pliers on his tool belt, which stayed in a vertical position upon impact, injuring his rib.

WHILE CHANGING A TRIP CABLE, THE MECHANIC STRUCK A B & O HAMMER, CHIPPING A PIECE OF METAL WHICH IMBEDDED INTO LEFT ARM. THIS LACERATION REQUIRED TWO (2) STITCHES TO CLOSE.

EE WAS ASSISTING IN HELPING TO TRY TO TIGHTEN UP THE CONVEYOR TAIL PIECE WHEN THE WRENCH HE WAS USING TO HELP TENSION THE TAILPIECE BROKE, CAUSING A PORTION OF THE WRENCH TO FLY LOOSE AND STRI KING HIM ON THE RIGHT SIDE OF HIS FACE. CAUSE OF INJURY WAS AWARENESS COMPLIANCE WITH RULES AND REGS WAS NOT A FACTOR, PROTECTIVE ITEMS OR TRAINING NOT A FACTOR.

EMPLOYEE WAS ATTEMPTING TO HELP STRAIGHTEN UP THE BELT DRIVE WITH A PRY BAR. AFTER THE BELT DRIVE HAD PLUGGED UP AND MOVED IT OUT OF ALIGNMENTTHE BAR SLIPPED STRIKING HIM IN THE FRONT OF THE JAW. CAUSE OF INJURY WAS AWARENESS.

HELPING TO ADJUST TRACKS ON DRAGLINE. JACK BEING USED JUMPED OUT OF POSITION, PINCHING HIS LEFT HAND MIDDLE FINGER.

VICTIM WAS WORKING WITH THE FOREMAN ON A TAILPIECE USING A PRY BAR TO STRAIGHTEN SOME PROTECTIVE SKIRTING. THE PRYBAR DISLODGED FROM THE FOREMAN'S HAND STRIKING THE VICTIM ACROSS THE CHEST. VI CTIM WENT TO HOSPITAL AFTER SHIFT-DIAGNOSED WITHBRUISED CARTILAGE OF THE RIBS.

EMPLOYEE CHANGING DRAGLINE DUMP CABLE, USING SLEDGE. A CHUNK OF STEEL CAME OFF AND WEDGED IN HIS LEG.

WHILE CUTTING A PIECE OF CANVEYOR BELTING WITH A KNIFE, THE KNIFE SLIPPED AND CUT THE EMPLOYEE'S LEFT HAND. ACCIDENT OCCURRED WHILE WORKING THE SHOP.

EMPLOYEE WAS HOLDING A TOOTH ON A DRAGLINE BUCKET. ANOTHER EMPLOYEE WENT TO TAP THE TOOTH UP A LITTLE BIT HIGHER WITH A 16PD SLEGE HAMMER. 1ST EMPLOYEE'S FINGER SLIPPED OVER THE EDGE OF THE TOOTH CAUSING THE SLEDGE HAMMER TO SMASH HIS FINGER INTO THE TOOTH.

WHILE SLEDGING TRACK PIN ON 4600, BAR MISSED HOLE & EMPLOYEE HIT FINGER ON DRAGLINE TRACK.

WHILE MKAING STEEL BOX EMPL USED 4' PIPE TO MAKE BACK OF BOX MEET OTHER PIECE OF STEEL WHILE OTHER EMPL WELDED BOX & PLATE TOGETHER. WHILE DOING THIS IT PUT STRAIN ON BACK.

EMPLOYEE WAS PULLING CONCRETE WITH A GARDEN RAKE FROM A PILE LEVELING IT OFF FOR THE CEMENT FINISHES TO SMOOTH AND FINSH THE JOB, WHEN HE FELT A PAIN IN HIS BACK.
