While standing on a ladder breaking loose bolts for the air dryer on a 793C haul truck. The employee strained the left shoulder during the process.

The EE was helping put the conveyor chain of a C/M together. EE was hitting a master link pin with a small sledge. When EE hit the pin a small splinter of metal hit EE in the right pinky finger.

Employee was using a utility knife to open up the smashed prongs on the trailer light plug. The employee was opening up the last prong and the knife slipped cutting the top side of left hand between the thumb and index finger. Injury required 4 stitches to close.

Am employee was releasing a chain binder on a flinger duster with a roof bolter wrench. When the binder came loose the wrench hit the employee in the forehead causing a laceration. The employee was taken to the Emergency Department and received 9 stitches, then was released to come back to work with no restrictions.

While using a utility knife to remove a hose from an air pump, enough force was applied by the injured employee, to break the blade of the utility knife. A piece of the broken blade remained in the hose and the force being applied also caused the arm to strike against the broken blade after the blade broke. This resulted in a laceration requiring 3 stitches.

Employee was using a hand press to remove a u-joint from a light vehicle driveline when the driveline shifted and contacted the employee under the chin. The laceration required 3 stitches to close.

Employee pinched right middle finger between a slate bar and belt structure when EE was knocking ice away from the belt line. The employee received 3 stitches.

Employee was working on a side auger bulk unit and had hand on a hydraulic ram. When the employee moved the ram it closed completely, catching the employee's hand between the ends giving the employee a fracture and lacerations.

During repairs on the longwall a mechanic felt a pop in back as EE pushed on a 30 MM end wrench. EE returned to duty. On 3/27/2017 this mechanic sought chiropractic care, identified as medical treatment by the Yellow Jacket and converting it to a medical treatment case on that day.

Employee was struck by a piece of metal while operating a truck mounted hoist about 20 feet away from another employee that was using a hammer tapping a metal dragline bucket shroud. A piece of metal broke loose & struck the injured employee in the neck area. Surgery 10/03/2016 removed the piece. Employee was released to return to work at no restrictions or time lost.

While preparing to install a new belt splice, the bottom belt moved unexpectedly when a pinch clamp failed, causing the belt to toss the cutting tool up, striking the employee in the mouth, breaking a tooth.

EE was driving a keeper out of a tooth base. EE struck a keeper removal tool with an 8 pound sledge hammer. A piece of metal came off of the removal tool & struck EE in the left thigh area. On Tuesday, 8/9/16, the piece of metal was surgically removed.

On March 7 this employee was removing a staple from a hydraulic fitting, using a pry bar. The bar slipped, causing the employee's arm to swing across chest. EE felt a pop in shoulder. No medical treatment was provided until June 28, at which time EE received shoulder surgery.

Employee was using a pocket knife to cut a plastic zip tie on a fire extinguisher inspection tag in order to replace the inspection tag when the knife slipped causing a laceration to the employee's hand. The laceration required sutures to close the wound.

While using a OEM breaker bar to loosen bolts on a push arm hyd cylinder, employee lost his balance when the bolt broke loose. The resulting release of energy caused the employee to stumble and sprain right ankle.

The Employee stated that he was replacing an area light on a continuous miner. The Employee started to cut the conduit that entered the area light, the Employee was cutting towards himself. The knife slipped and lacerated his left palm. The Employee received 6 stitches in his left palm.

Employee was removing a compression fitting from a fuel hose using a crescent wrench. The wrench slipped, causing him to lose his balance & fall to the ground, hurting his shoulder. He has continued to work with the injury & had surgery on April 15, 2015.

EE was assisting mechanic removing ripper shank tooth. EE struck the ripper tooth with sledge hammer and a piece of metal struck them on the left leg. EE first day missed was 12/26/2014.

Student Summer Intern was cutting a ty-wrap that was used to hold an extension cord on a parade float. The knife slipped causing a laceration that required stitches.

The employee was bent over pounding stakes for a drill pattern. The employee felt pain in his lower back that radiated down his right leg. The employee was transported to the local clinic and the medical provider prescribed a prescription (muscle relaxer) which caused the employee to miss his next shift.

EE was changing the left front tire on his F-250 Ford pickup. They had jacked up the truck and removed the tire. The drill helpers tried to put the new tire on the hub, but the truck was not high enough. EE got under the truck to raise the truck to put the tire on, but the jack slipped and the truck fell on EE, on his right side.

EE removed & cut a door spring with a torch. EE was using a chisel & hammer to remove slag to free the spring. Injured EE was standing behind the EE when a sliver of slag flew off striking the injured EE in the face. Removing the sliver required opening the wound that then needed two stitches to close. EE was released to return to work performing his normal duties.

Incident was not report until 6/04/12. Employee was changing out grinder teeth with sledge hammer and 4 brackets. After changing the teeth the employee experienced pain in the right elbow. The employee did not seek medical attention until after reporting the incident on 6/4/12.

The employee was driving a post with a post driver when the driver slipped off the post and contacted the employee on his hard hat.

Employee cut is finger with his pocket knife while trying to cut his cheese during his lunch break. He was sitting in the cab of the truck. Truck was off, parking brake was set and wheels were chalked.

EE using 10lb sledge hammer while changing the teeth on a dragline bucket. The 1st 3 teeth came off. The 4th tooth was stuck, so he tapped harder. He felt something hit his leg, but did not hurt. Hit tooth again. Felt something warm on leg, it was blood on pant leg at knee. Taken to ER and X-rays done, showed piece of metal in leg. Unsuccessful to remove it, Dr put 3 stitches.

Employees were using a port-a-power to hold steering tire at full turn position. Employee's did not have the proper length of ram so they were using a block to make up the difference. Ram snapped out of place and struck injured employee on his right chin area requiring five stitches. No Lost Time -Employee returned to work with a full released after the doctor visit.

EE was removing a backhoe bucket tooth. He struck the pin with a 12 pound hammer, the pin chipped and cut his right middle finger. He was taken to a local ER for treatment. No stitches were required. The laceration was glued.

Employee (temporary welder) was removing wear plates from an old dragline bucket when a metal splinter flew off striking employee in the right index finger. Employee was using a 20 pound hammer to hit the wear plate. Employee taken to emergency room where three stitches and a tetanus shot were given.

Mechanic was injured while attempting to adjust a propeller on a barge pump propeller shaft in the shop for repair. 2 employees were using a 6' pipe on a pipe wrench to turn the propeller shaft of the pump. There was a burr on the end of the pipe. Wrench slipped & burr on the pipe cut EE's forearm. He was taken to ER-received 8 stitches to close the wound. full duty return.

Employee was cutting the top of a plastic jug off using a utility knife. The knife slipped, cutting his left wrist. He was taken to a local emergency room where he received two stitches for the laceration.

Employee was removing spacer plates from horizontal fairlead sheave. He was helping a welder break the bead off the plates. As he turned a plate the welder swung a hammer striking the employee on his right pinkie finger.

While repairing rear strut - raising the strut into position with a come-along, employee was attempting to line up strut onto top pin eye with a pry bar. The pry bar slipped & employee pinched finger between pry bar & the differential housing of the truck.

Employee was tightening bolts on a set of portable stairways when his socket wrench slipped & employee cut his right middle finger on the stairways metal edge. The resulting cut required 3 stitches to close the wound.

Employee was cutting metal banding around a pile of cutting edges. The banding contacted his right thumb. He was examined at the medical center and received 3 stitches in his right thumb. He returned to work with no restrictions.

Employee was splitting the plastic retainer tabs of a five gallon bucket lid with a utility knife so he could remove the lid. While doing this his hand slipped and he cut his right index finger. He was taken to a physician for treatment and released the same day with no restrictions.

Employee was trying to get space between a bull gear and a snap ring on a 120 ton coal hauler differential. The ring flew off and struck him in the mouth breaking his tooth. He was taken to a Dentist for treatment and released the same day with no restrictions.

Employee was trimming skirt rubber with a utility knife and the knife slipped off the rubber and punctured his right upper leg. He went to a local E.R. and received 3 stitches for the wound.

3 employees were changing a 4 dragline bucket teeth. One employee was hammering the keeper when a piece of metal came off & lodged in injured ee's left thigh. Inj ee went to the dr's 8/14/06. The metal was embedded & had to be cut out. The incision required 2 stitches to close.

EE was turning the drive shaft using a one foot pry bar. The bar slipped & struck him in the face. He went to a local ER where he had 5 stitches to his left cheek.

Employee was loosening bolts on hose clamp, and when the bolts finally came free the employee experienced a painful jolt in his right shoulder and neck. Neither time nor wages have been lost. Employee began physical therapy on March 03, 2006 to help alleviate the pain.

Employee was changing oil and using a 3/4 inch breaker bar to loosen the plug on the final drive of a D-10R dozer. When the plug loosened the employees finger was smashed between the breaker bar and dozer tracks. He was treated at the Colstrip Medical Center and released for his next scheduled shift.

The employee was using a "Handi-Man" jack to raise the back of a pickup. The handle bounced up, and struck him above the left eye. He was treated at the Colstrip Medical Center and four stitches were used to close the open wound. He was given a release to return to work his next scheduled shift. Neither time nor wages were lost.

Employee was preparing a boring bar for machining and was using a pry-bar to align the center bearing for the boring bar. The bearing did not align, and the bar struck the employee's finger. Ice was applied on site, and the employee was transported to the Colstrip Medical Center where he was treated and released for next shift with lifting restrictions. No time or wages lost.

EMPLOYEE WAS INSTALLING A HYDRAULIC CYLINDER ON A CAT SCRAPER IN OUR MAINTENANCE SHOP. HE ALLEGES THAT A PRY BAR HE WAS USING TO INSTALL THE CYLINDER SLIPPED CAUSING A STRAIN ON HIS LEFT SHOULDER. THE INCIDENT WAS NOT REPORTED TILL 10/11/04. hE WAS RELEASED TO WORK WITH NO RESTRICTIONS. EE HAD SURGERY ON 2/15/05 MAKING THIS INJURY REPORTABLE.

Employee was replacing a bearing on a crusher. The spanner wrench slipped and the employee's finger was pinched between the wrench and the machined edge of the bearing housing. He was treated at the Medical Center where the laceration on his left index finger was closed. To date neither time nor wages have been lost.

EE WAS KNEELING DOWN, HELPING ANOTHER MECHANIC WORK ON A SERVICE TRUCK'S BRAKES. THEY WERE REMOVING THE TIRE TO WORK ON THE BRAKES. THEY WERE USING A DUAL WHEEL TIRE JACK AND THE CHAIN ON THE TIRE JACK SLIPPED. THE TIRE FELL ON HIS FOOT, BRUISING IT. HE WORKED RESTRICTED DUTY FOR THREE DAYS.

The employee was using a sledge hammer to drive keeper pins from dragline teeth, and experienced pain in his right shoulder. At the time no medical attention was sought. On May 12, the employee reported that he had been receiving medical treatment, and needed surgery on his shoulder. He missed his first scheduled shift on May 14, 2004

THE EMPLOYEE WAS REMOVING WIRE ROPE TAILS OFF OF A WOOD WIRE ROPE SPOOL. AFTER THE NUTS HAD BEEN CUT OFF OF THE U-BOLT THAT RETAINED THE TAILS ON THE WOODEN SPOOL, HE USED A SHORT PRY BAR TO P RY THE U-BOLT OUT OF THE SPOOL. WHEN THE BOLT CAME FREE IT STRUCK HIS FACE. HE WAS EXAMINED AT THE MEDICAL CENTER & TAKEN IN FOR DENTAL WORK. HE WAS RELEASED FOR WORK WITH NO RESTRICTIONS 6/3
