Technician was removing worn bucket adapter tips on a 992G wheel loader using a sledge hammer to strike the tips. A piece of metal broke free and struck the technician in the knee.
Employee was boring saddle bore on a D9 Dozer and was removing the tooling after finishing the repair. He was attempting to inch the shaft in one direction only 1/4 of a turn in order to access the screws. During one turn, he missed the stop button and the shaft spun around further than 1/4 turn and struck him in the chin, causing laceration to chin.
EMPLOYEE WAS HOLDING HAMMER WHILE SOMEONE WAS STRIKING HAMMER TO DRIVE PIN IN DRILL TRACK. A PIECE OF STEEL BROKE OFF AND HIT SUBJECT IN THE LEFT SIDE OF HIS NECK.
