Employee was cutting interior flooring of dump truck installing new ones and cutting to size. The blade slipped out of flooring and cut into left wrist which was holding flooring up to cut flooring easier. 911 was called and employee was transported to hospital where they received stitches in left wrist as well as prescribed 600mg of Motrin and 500mg of Tylenol.

Employee was cutting liner material with a utility blade, the blade slipped from the cutting path and cut the employee's hand.

Employee was cutting vegetation with lopping sheers. Employee twisted their body and strained their left ankle.

Miner was making repair. He was turning a wrench in upward direction when he injured his back.

54736YEE WAS WELDING A CRUSHER USING A TORCH CUTTER AND A SLAG HAMMER. AFTER FLAME CUTTING A PIECE OF METAL AND FRAGMENTS OF THE SLAG FROM THE METAL ENTERED HIS RIGHT EYE. ND FRAGMENTS OF THE SLAG FROM THE METAL ENTERED HIS RIGHT EYE.

EMPLOYEE WAS REPLACING A BEARING ON HEAD PULLEY ON MAIN CONVEYOR BELT. THE SHAFT SLIPPED OFF THE JACK, STRIKING THE EMPLOYEES LEFT INDEX FINGER.

REINJURED STRAINED HIS RIGHT KNEE PREVIOUS INJURY OCCURED ON 5-5-89. EE WAS SHOVELING WHEN HE FELT A SHARP PAIN IN HIS RIGHT KNEE.

ATTEMPT TO FREE LODGED BOULDER FROM CRUSHER ROCK BROKE PC SHOT UP STRIKING HIM ON L HAND TREATED FOR CUT NO LOST TIME.
