Employee was removing cast liners within rawmill for replacement. Using a sledge hammer to loosen the first liner, employee struck corner of liner and a piece fractured and came back and struck employee in the nose causing laceration and broken nose.

Employee was cleaning the tracks of the bulldozer with a shovel. EE heard a pop and experienced pain in EE's right arm. Employee continued to work unrestricted until EE went to an orthopedic specialist on 12/30/2025. EE was diagnosed with a tear of the right rotator cuff. The extent of the tear is unspecified. The employee is now on restricted duty.

Employee was trying to beat a slide gate on a silo closed using a sledgehammer. Cement sprayed into their eyes and knocked off their safety glasses. Employee was treated at the Hospital Emergency Department.

Employee was cutting a hose fitting off an airline and the knife slipped and lacerated EE's left pinky finger. EE received 3 sutures to close the laceration.

Employee was using a pry bar to pry a jammed rock in the chute. The bar slipped and EE pinched EE's fingers between the bar and the chute. EE received a closed fracture on the 5th tuft of EE's right hand.

Employee was prying on the lip shroud of a drag bucket. When the bar slipped, they strained their right shoulder.

Employee was using a breaker bar to remove a bolt on the wheel assembly on a Komatsu WA 800 when EE felt a pop in lower groin. EE was diagnosed with a hernia. EE remained on regular duty and this did not become an MSHA reportable injury until EE had surgery on 02/16/2018.

Employee was using a come-a-long to move an approximate 2,000 lb 5'X8' railroad street cart back onto tracks. Employee felt a strain in lower back with pain.

A chain the employee was using to pull tracks for a dozer broke and struck the employee in the left forearm causing a distal radius fracture.

Employee was cutting a conveyor belt with a utility knife and the knife slipped and cut left arm. EE went to a physician and they glued the laceration closed.

Employee was driving a steel hinge pin in, while installing a kiln door. Hit his thumb with a small sledge hammer.

Cleaning chute on Tyroc screens at crusher. Lost balance and fell on shoulder. Did not miss any work until weeks later injury bothered him and scheduled surgery for 6-16-2015.

Removing stuck chain with a bar. Pinched finger between bar and access door frame.

Employee was cutting a conveyor belt with a knife and he cut the back of his hand that was holding the belt. Three sutures were required to close the laceration.

EE was pulling in an upward motion trying to loosen a flange on top of the cement silo. ***EE missed no work due to this incident until he had rotator cuff surgery. First day work missed was 4/18/13.

Employee received a broken finger while using a pry bar to dislodge a piece of rock. The bar slipped off the rock and smashed his finger.

Employee was raising jack stand on the fuel tank. The hand crank slipped off the jack and struck the employee in the nose. Employee will have surgery on 9/16/10 to repair his nose.

EE was cleaning sticky coal from the buckets in the coal elevator. While cleaning a bucket, a piece of coal fell and struck him on the arm.

The original injury date was 3/28/09 when the employee was tightening an overhead bolt on the crusher and he felt a pain in his right shoulder. The employee was not placed on restricted duty or incur any lost time as a result of this injury until 5/20/09 when he had surgery.

A Bridgeburg employee from the Cowansville Shop was removing the parking brake from a CAT 990 with Come-A-Longs. One of the Come-A-Longs and a nylon strap slipped, and the unit pinched the workers forearm. The worker went to a physician the following day and was put on lite duty.

EE was cleaning spilled clinker from under #1 cooler. As he was pulling the broom, he hit the clloler framework with his left forearm.

While pounding steel with a hammer, the man had a steel splinter in his right thumb which was removed at the emergency room.

The injured man had a steel splinter in his left eye (no witnesses) late on shift. Went to doctor on Sat & had splinter removed. Said he did not know it happened until next day.

The injured was sledging a steel pin. A steel splinter hit and cut is right arm causing a small wound. The Emergency room Dr. gave antibiotics to treat for infection. No lost time or other treatment needed.

Poking chute in coal tunnel, when coal broke loose, knocking him over backwards.

The injured man was helping a mechanic remove a water pump from his rock truck. The mechanic bumped the man on the lips with a wrench. A month later the injured man's dentist noticed that his crown post was loose. We were notified of the incident on 5/8/06.

A SMALL METAL SLIVER FROM HAND CHISEL EMBEDDED IN RIGHT THIGH. THE SLIVER WILL BE LEFT IN THE THIGH UNLESS COMPLICATIONS ARISE IN THE FUTURE. EE WAS GIVEN AN ANTIBIOTIC & IS BACK TO WORK WITH NO RESTRICTIONS.

The injured man received one stitch in finger when a sliver of steel was removed.

THE INJURED PINCHED HIS LEFT THUMB BETWEEN A PRY BAR AND A SURFACE, LACERATING HIS LEFT THUMB, WHICH REQUIRED STITCHES.

MAN WAS CHIPPING SLAG FROM A WELD WITH A CHIPPING HAMMER WHEN HE COMPLAINED OF PAIN IN RIGHT ARM-SHOULDER AND FINGERS ALONG WITH NUMBNESS.

THE INJURED MAN WAS CHIPPING SLAG FROM A PIECE OF STEEL THAT HAD BEEN CUT. A SMALL PIECE GOT IN HIS RIGHT CORNEA & HAD TO BE CLEANED BY A PHYSICIAN.

SPLICING PLASMA TORCH HOSE CUT 3" FROM 1ST JOINT TO 2ND JOINT OF RIGHT THUMB TOP SIDE WITH UTILITY KNIFE, SENT HOSPITAL FOR STITCHES (5) RESULTS.

THE MAN WAS LOOSENING A NUT WITH A HAND WRENCH WHEN THE WRENCH SLIPPED OFF THE NUT. THE WRENCH STRUCK HIM ON THE CHEST AND CRACKED A RIB. HE CONTINUED TO WORK DAILY UNTIL THE 24TH WHEN HE WENT TO THE DR FOR A CHECK OUT. HE LOST ONE DAY OF WORK AND IS BACK ON REGULAR DUTY.

THE MAN WAS DRIVING A STEEL WEDGE TO TIGHTEN A TOOTH WHEN A PIECE OF STEEL CAME OFF THE TOOTH AND HIT HIM ON THE NOSE AREA HE REQUIRED 2 STITCHES IN HIS NOSE AND UPPER LIP.

REMOVING INSECTION DOOR IN FLOOR LEVEL TO CHECK WATER PUMP AND PULLED MUSCLES TO LOWER BACK. SENT OT HOSP 1-4-93 WORKED UNTIL 1-11-93. DR ADVISED INJURED TO STAY HOME.

THE INJURED MAN WAS USING A 10 FOOT STEEL PRY BAR. THE BAR SLIPPED AND HIT HIM ON THE HEAD CAUSING A CUT WHICH REQUIRED 3 STITCHES.

38504AN WAS TURNING A NUT WITH A WRENCH WHEN THE WRENCH SLIPPED OFF THE NUT AND HIT HIM ON THE FOREHEAD. THE MAN HAS A HISTORY OF BEING SICK AT THE SIGHT OF BLOOD. HE WENT HOM BUT DID NOT RETU 95036MPLAINING OF SORENESS.

INJURED MAN FEELS THAT HE INJURED HIS LOWER BACK & HIP WHILE SWINGING A 20POUND SLEDGE HAMMER (NOTE HE DID NOT HIT HIMSELF)

BUSTING LUMPS AT CLINKER RROLLER DISCHARGE END LUMP HIT BAR. BAR STRUCK INJURED IN R/CHEEK REQUIRING 6 STITCHES AT HOSPITAL

THE MAN WAS POUNDING ON A STEEL TOOTH WITH A HAMMER WHEN A PIECE OF STEEL BROKE OFF AND STRUCK HIM ON THE NOSE WHICH REQUIRED STEEL REMOVAL AND 5 STITCHES.

CUTTING TIE WIRE EMPLOYEE CUT LEFT THUMB SENT TO CLINIC. 3 STICHES AND TETNAUS SHOT.

USING STEEL BAR TO CLEAN GYP BIN BAR SLIPPED AND CAME BACK AND STRUCK EE IN NOSE SENT TO HOSPTIAL FOR STITCHES POSSIBLE BROKEN NOSE.

CLEANING UP BEHIND TAILPULLEY SHOVEL HIT BELT SPLICE & STRUCK INJURED IN L/CALF-TREATED HERE SENT TO DOC 07-31-90 DR OK TO RETURN 08-03-90 BUT FOUND TO HAVE A SMALL BLOOD CLOT INJURED HAS YET TO RETURN TO WORK

MAKING REPAIRS AT GYP ELEV LOCATION-USING WIRE SLINGS & COME ALONG-PULLED BACK MIDLINE. SENT TO CLINIC FOR ABSERVATION.
