Employee was swinging a sledge hammer and strained his lower back. He went to his family Chiropractor and he is now fine and back to work full duty. No lost time, no restricted duty.
Employee was working on booster pump and was using a wrench. The wrench slipped and hit employees index finger on the left hand. He required stitches.
This accident just recently became reportable. EE was thought to have a bruised shoulder. Missed no time and was not under restrictions. However, EE is now going to therapy which makes it reportable. EE was using a pry bar and strained left shoulder.
Employee was cleaning sand screen with a brush. The brush got caught, and the employee jerked on the brush, it came back and hit him in the mouth. His front tooth was broken.
EMPLOYEE WAS MOVING IMPELLAR. COME-ALONG FAILED AND MASHED HIS RIGHT THUMB. NO BROKEN BONES.
EE WAS TIGHTENING A TAIL PULLEY, THE WRENCH SLIPPED AND HE HIT HIS RIGHT HAND, CONTUSION
WE WERE JUST NOTIFIED ABOUNT THIS ACCIDENT, EMPLOYEE DID NOT TURN IT IN WITHIN THE 10 DAYS. EMPLOYEE WAS PULLING ON A WRENCH WHEN THE WRENCH BROKE LOOSE & JERKED HIS ARM.
EE JERKED ON A WRENCH AND FELT A PULL IN HIS RIGHT SHOULDER, HOWEVER, HE THOUGHT IT WAS ONLY HIS RIGHT SHOULDER, HOWEVER HE THOUGHT IT WAS ONLY MINOR. IT IS STILL BOTHERING HIM SO HE REPORTED IT ON 5-23-02. HE MIGHT EVENTUALLY HAVE SURGERY.
EE WAS PULLING ON WRENCH. NOTICED STOMACH AREA HURTING. DR. SAID HE HAS AN UMBILICAL HERNIA.
EE WAS USING A PRYBAR. THE BAR SLIPPED & HIT EE IN THE RIGHT EYE.
EE WAS PULLING & PUSHING ON A WRENCH. HE STRAINED HIS LOWER BACK.
EE WAS BEATING SLAG OFF WITH A HAMMER. SLAG FLEW UP UNDER HIS SAFETY GLASS AND INTO HIS LEFT EYE.
EMPLOYEE WAS CUTTING CARDBOARD WITH A BOX KNIFE.HIS HAND SLIPPED AND HE CUT THE PALM OF HIS LEFTHAND.
EMPLOYEE WAS USING WRENCH TO TIGHTEN PACKING ON PUMP. HE PULLED UP AND STRAINED CAUSING LOWER BACK PAIN.
EMPLOYEE WAS PULLING ON A RATCHET. THE RATCHET SLIPPED AND HIT HIS THUMB ON THE RIGHT HAND. RESULT FRACTURED THUMB.
EE WAS CHIPPING FROZEN SAND OFF OF A CONVEYOR. SAND FLEW IN HIS EYE AND CAUSED AN ABRASION.
EE WAS LEANED OVER THE RAIL OF THE DREDGE PRYING ON A BAR. HE STRAINED HIS LOWER BACK. HE WAS ALSO PULLING CABLE.
