Employee reported accident occurred while clearing plugged starter steel on drill rig. Compressed air had been used unsuccessfully to clear debris. Air was bled off Workers waited 2 minutes before trying to clear plugged steel with a pry bar. Plugged material came free, releasing residual compressed air, water, mud and debris onto worker's right hand and coveralls.

EE was using a snap-on filter wrench with a 24" breaker bar to loosen a filter the breaker bar slipped out of the filter wrench socket and his left elbow struck a strut housing. No reportable medical treatment until 7/12/07.

Reported pain after the job was completed. Returned to regular duty on 4-26-07.

A PIECE OF SPROCKET BROKE OFF, STRIKING EE IN THE NECK.

Employee reported using long handle scraper to clean accumulated material from floor of service pit. Scraper struck raise edge of 1/4" hidden under material resulting in jammed right shoulder. Employee did not seek medical attention until friday July 29th at 7 pm.

EE was laying on his stomach on top of 36" pipe. Reached over to adjust the come-a-long - was wearing rain suit. Slipped off the pipe, fell into a hole approximately 8' deep. Offered medical care - EE declined. Then requested care on 12-22-04.

BOX CUTTER SLIPPED, CUTTING EE'S LEFT KNEE.

THE WRENCH HE WAS USING SLIPPED, CAUSING EE TO FALL OF THE RAMP BACKWARDS.

STRAIN / INJURY BY USING TOOL.

BOX SLIPPED AT THE AREA BEING OPENED ABEL LOST BALANCE FELL BACKWARD, LANDING ON LEFT ELBOW & JAMMING LEFT SHOULDER

STRUCK BY FALLING OR FLYING OBJECT. EE WAS DRIV-ING PIN INTO TOOTH ON LOADER BUCKET WITH HAMMER,HAMMER STRUCK THE PIN-PIECE OF METAL FLEW INTO EE'S CHEEK/JAWBONE AREA.

CUT LEFT HAND WHILE CUTTING OLD BELT PRESS BELT PRESS BELT OFF. THE WEIGHT OF THE BELT PULLED HAND ACROSS KNIFE. SEVERAL STITCHES FOR 3 CM LACERATION. Y

CUT/PUNCTURE/SCRAPE. HOLDING DEVICE TO STABILIZETOOTH WHILE PIN WAS HAMMERED OUT.(ON LOADER) STEEL WENT INTO HIS ARM ON UNDERSIDE HALFWAY BETWEEN ELBOW AND WRIST.
