EE was was working on the plant. EE was applying pressure to a prybar when EE's hands slipped off and EE's right little finger was pinched by the bar, resulting in a fracture to the tip of EE's right pinky finger.

WHILE RE-ATTACHING A GUARD THAT HAD BECOME LOOSE, EE WAS CUTTING WIRE-TIE ATTACHMENT AND UTILITY KNIFE SLIPPED AND CUT EE LEFT THUMB
While cutting a conveyor belt, utility knife slipped cutting employee's right forearm.
STANDING ON THE FEED BOX OF A CRUSHER, 15' OFF THE GROUND, EE WAS REACHING ABOVE HIS HEAD TIGHTENING 2 BOLTS WITH A WRENCH. THE WRENCH SLIPPED OFF THE BOLT CAUSING EE TO FALL BACKWARD TO THE G ROUND.
EE CUT FINGER WHILE STRIKING A RETURN ROLLER WITH A HAMMER TRYING TO REMOVE IT.
EMPLOYEE ALLEGES THAT HE CUT HIS THUMB WHILE CUTTING A PIECE OF ELECTRICAL WIRE WITH A UTILITY KNIFE.
EE ALLAEGES THAT THE WRENCH HE WAS USING SLIPPED OFF THE NUT CAUSING HIS FINGER TO HIT ANOTHER NUT. HE JERKED HIS ARM BACK & STRUCK HIS ELBOW ON THE METAL CHUTE WHICH CUT HIS ELBOW. THIS CUT L ATER BECAME INFECTED, MAKING IT NECESSARY FOR HIM TO BE PRESCRIBED ANTIBIOTICS.
SHOVELING TINGLING IN L HAND AND WRIST
EMPLOYEE ALLEGES THAT HIS INJURY WAS CAUSED BY BENDING OVER, PICKING UP A SLEDGEHAMMER AND TWISTING.
EMPLOYEE IS NOT SURE OF WHEN OR WHAT EXACTLY CASUSED HIS MUSCLES BACK STRAIN. HE BELIEVES THAT IT MAY HAVE BEEN DURING A ROUTINE JOB OF UNPLUGGING THE JAW OF THE CRUSHER BY BENDING OVER THE R AIL AND DROPPING A BUCKET TOOTH SUSPENDED ON A ROPE ONTO PLUGGED ROCK MATERIAL.
