A pry bar was used to loosen material on feeder and sides in a chiseling motion and employee's hand slipped being cut by the handle of the pry bar.

Subject was cutting the tail pulley loose from conveyor. Once bearings were cut loose the tail pulley shaft teetered the opposite direction pinning hand between it and hopper above.

Employee was in the process changing the final drive oil on Cat 769C haul truck. He was using a breaker bar to loosen drain plug. Pushing down on breaker bar, he said plug came loose causing a snapping sensation in his left shoulder.

REMOVING WARE LINERS OFF A CRUSHER ROTOR & THE CHISEL FLEW OFF & HIT HIS SAFETY GLASSES, BROKE GLASSES, & HURT HIS LEFT UPPER EYELID.

EE WAS PRYING ROCK IN FEEDER WITH PRY BAR, BAR SLIPPED PINCHING THUMB ON RT HAND BETWEEN BAR AND SIDE OF FEEDER HOPPER, WHICH BROKE BONE IN RT THUMB.

I.NAME HAD PLACED TWO WEDGES IN THE JAW TO MOVE A ROCK WHICH WAS CAUGHT ONE OF THE WEDGES FLEW BACK AND HIT I.NAME IN THE RT FOREARM BUMP FORMED LATER AND CAUSED A BLOOD CLOT WHICH LED TO AN INFECTION IN ARM

I.NAME WAS STANDING STRADDLED OF TWO ANGLE IRON BARS THE BOTTOM ANGLE IRON BROKE LOOSE HE HAD BEEN REPLACING THE FEEDER MOTOR BELTS
