An employee was prying a chunk of material with a bar which slipped off the chunk, causing employee to lose balance and then went backwards into structure behind EE. Employee did not miss any work or no restrictions until the shoulder flared up again. It was determined EE has a slight labrum tear requiring surgery. Employee became a reportable and had surgery on 12/16/2020.
Employee clearing cement blockage got dust in eye.
Employee was poking #10 clinker vibrator in clinker tunnels when dust fell from ceiling area and blew into employee's Lt. eye. Prescription ointment was administer at clinic.
Emp was changing silo 23 to silo 20 and was hitting selector in an upward position w/slegehammer w/Rt.hand. Emp struck Lt.hand index finger. X-rays showed three fractures and a splint was placed on finger. Restrictions of "no Lt. arm work". Emp continues at in his same position training a new PA. No lost time.
EE was removing a Keeper off of Pin on Spillage Chain and did not realize his Thumb as in the way.
Emp was using a poke bar to clean out vessel on PreHeat Tower. While poking struck Lt. hand 5th finger (pinky) between edge of vessel door frame and bar. X-rays showed fracture of bones at tip of finger and nail was drilled to relieve pressure.
Emp had #108 belt shutdown and was starting to clean out mud from the drum area under tripper using a poke bar. Bar became lodged between drum and guard emp Lt. hand struck edge of guard causing laceration to thumb. Stitches (wearing gloves)
Emp reported he strained Rt. lower back when using a poke bar to unplug rotary feeder for fuel mill. Bar slipped causing emp to twist back. Emp was examined at Ill. Valley Com Hosp - Occ Health on 11/06/2012. It was determined emp could not work due to prescription drugs (muscle relax) he was taking.
Emp. was removing side liners on #4 Loesche Mill (Raw Mill) seal with 8mm allen wrench. While loosening bolt, wrench slipped causing emp. Rt. hand to strike edge of mill roll. Laceration approx. 3" long required 14 stitches.
Emp was inside 605 elevator running a comealong on the outside to remove a link out of chain. In the process of pulling the chain the pulling device on the chain straighten out came off of chain cause chain to strike emp in the back of head on hardhat and below hardhat. Emp received a laceration to back of head that required staples.
Emp was working with co-worker reinstalling headpulley cover. Co-worker was lowering cover from platform above pulley to emp below using a chainfall through an opening in platform. Cover got caught in opening as emp was attempting to loosen it - it broke free causing cover to spin and flange struck his upper lip. Returned to dr on 04/13/2011, lip was determined to be infected.
Emp was using a pry bar to remove a bent fender bracket from a semi. Emp cut bolts holding bracket in place and took pry bar to remove bracket. Bracket broke loose causing emp to loose balance, causing his Rt. hand ring finger to strike the battery box, causing a fracture to finger. A splint was placed on finger.
Emp was placing top covers on a screw conveyor. Emp was tightening bolts when wrench slipped off bolt causing Rt. index finger to strike cover causing a laceration that required 4 stitches.
Emp. was holding a box wrench under a conveyor belt clamp, while taking clamp off, bolt broke causing clamp to snap apart. Angle iron on clamp struck Lt. ring finger, causing laceration that required 6 stitches.
Employee was sitting on the top area of the #5 cooler fan damper housing using a rachet to tighten bolts. Employee's hand slipped off rachet handle causing employee to loose his balance and fall 10ft to the floor area below. Employee was not wearing / using proper PPE.
Emp was working in #5 Finish Mill removing mill liners. Emp was prying under liner while other emp was using a 20lb. sledge to loosen liner. Pieces of liner broke off striking emp in the upper Rt. arm and upper leg. Sm piece was removed from upper arm stitches were required. The piece in his upper leg required surgery- LT as of 08/28/09.
On 5/5/09 emp. was tightning bolts with wrench when it slipped causing emp. to fall forward striking Rt. knee on I-Beam. Emp. remained at his position until 10/28/09 emp. states that he aggrevated his knee while running to assist an emp. during an emergency at the plant. Emp. had surgery 12/18/09 and is a LT.
On 01/05/09, 5:30am ee was using a poke bar to loosen frozen material in rock hoppers. During his shift he felt discomfort in his Lt. arm, felt it was jarred while poking frozen material - he has been treating with Physical Therapy - further testing has indicated a torn rotator cuff in Lt. shoulder. Employee has become a lost time as of 03/31/2009 pending surgery.
Employee was poking out the clinker vibrator with a poke bar. Employee went to put the poke bar away in a metal locker when bar slipped and pinched employee's Rt. hand pinky finger between bar and metal locker door. Employee was taken to Occ Health for examination and laceration required stitches.
Employee was using pipe wrench to tighten a fitting. The pipe wrench slipped causing employee's rt. hand pinky finger to be cut on a sharp piece of metal. Employee's injury required five (5) stitches.
EE was using a pry bar to place shims under the motor of an ID fan, when the pry bar slipped and he fell from a platform approx. 4' to the ground.
EMPLOYEES WERE STRIPPING CONCRETE FORMS FROM AN ELEVATED PLATFORM. AS INDIVIDUAL WAS PRYING A FORM LOOSE, A 2ND CAME LOOSE AND FELL A COUPLE FEET, STRIKING ANOTHER EMPL0YEE IN THE BACK OF HIS HEAD - BENEATH THE RATCHET OF HIS HARD HAT. EMPLOYEE WAS GIVEN 1ST AID THEN TAKEN TO HOSPITAL FOR OBSERVATION - BLEEDING HAD STOPPED. DR DECIDED TO APPLY 2-3 STAPLES.
EE WAS CUTTING A HOSE OFF THE VACTOR TRUCK TO CHANGE IT. IN THE PROCESS OF CUTTING THE HOSE, HE CUT HIS FINGER. HE WAS USING A POCKET KNIFE THAT DID NOT STAY LOCKED AND CLOSED ON HIS FINGER, THE LACERATION TO HIS RIGHT FINGER REQUIRED STITCHES.
EMPLOYEE WAS CLEANING MUD OUT OF EXCAVATOR TRACKS WITH SPADE SHOVEL & WHEN POKING HIT METAL AND STRAINED RIGHT SHOULDER. EE REPORTED THAT ACCIDENT HAPPENED ON 2/25/2004 HE REPORTED INJURY ON 3/5/2004. DUE TO NEW SPPLIED RESTRICTIONS AND UPCOMING SURGERY EE BECAME A LOST TIME AS OF 3/25/2004.
EMPLOYEE HAD FOREIGN MATERIAL IN EYE. HE REPORTED IT TO US ON 1-6-04. TOOK TO EYE CLINIC, EYE WAS RINSED OUT AND EMPLOYEE WAS GIVEN EYE DROPS TO TREAT INFECTION CAUSED BY FOREIGN OBJECT. WE THOUGHT HE HAD DROPS TO SOOTH OR FOR NUMBING PURPOSES ONLY. UPON REVIEW, WE FOUND DROPS WERE PRESCRIBED FOR INFECTION ON 2/5/04.
EMPLOYEE WAS WORKING ON THE #5 BALL MILL REMOVING HEADLINERS ON FEED END. WHILE PRYING HEADLINER OFF THE FACE OF THE WALL, IT SLID DOWN AND HIT EMPLOYEE'S LEFT FOOT.
EE WAS ATTEMPTING TO LIFT HEAVY PART OF A BULLDOZER WITH A BAR AND A BLOCK OF WOOD AS A FULCRUM. WHEN HE PUSHED DOWN ON THE BAR TO LIFT THE OBJECT, HE STRAINED HIS BACK.
WHILE UNPLUGGING WEST COAL CHUTE ABOVE 504 COAL CONVEYOR EMPLOYEE STRUCK A LIFTING LUG ON SIDE OF CHUTE BETWEEN SLEDGE HAMMER HANDLE.
WHILE POKING A CHUNK OUT OF 3 HOPPER WITH POKE BAR EE CAUGHT HIS RIGHT FINGER BETWEEN HOPPER AND POKE BAR.

THE LEFT EYE WAS SCRATCH ED AND THE TREATING DR PRESCRIBED A PAIN RELIEVER, WHICH HE DESCRIBED AS MEDICATION WHICH MAY CAUSE DROWSINESS, AND WOULD NOT RELEASE UNTIL 1-7-94.
