The maintenance employee was lifting a section of cutting-edge steel using a pry bar. During this process, the cutting edge shifted unexpectedly, pinching EE's finger between the edge and the bolt. This resulted in a small external laceration on the outside of EE's finger.

Mostrando los 17An employee was attempting to remove built up material next to load cells in a hopper. EE attempted to use a 5 foot long bar to remove this material. When EE got to more consolidated material, the employee used a hammer to strike the spade end of the bar assist in breaking up the material. A foreign particle became airborne and struck the employee in the chest and was embedded.
Employee was turning a bolt with a fixed point (standard) wrench to tighten it. The wrench slipped unexpectedly off the bolt and the employee's arm came into contact with a metal angle nearby. The employee sustained a 3 cm laceration which was treated at a local medical facility, including stitches
A subcontractor was using a home made tool to bend silo panel flanges during the installation of finished product storage silo walls. The employee was pulling the tool towards himself and it slipped out of position. The top edge of the tool handle struck him on the bottom of his safety glasses resulting in a laceration under his left eye that required six sutures.
Installing earth tooth on auger using a 3lb hammer. Piece of tooth broke off hitting face and cutting eyebrow.
EE was raking and sweeping loose material from the floor around an elevator when he overextended his reach and suffered a strained muscle in the lower back.
Employee was using a chipping hammer to chip hardened material off the cooler gear, a piece of the chisel side of the hammer broke off, penetrating employee's left thumb. This was classified as a first aid initially, it became a reportable on Nove 24, 2008 when the metal chip was surgically removed. My records indicate this infor. was faxed.
EE was removing worn center section on bucket lip of #3 992 Cat loader in mobile shop. EE was attempting to dislodge section from bucket by striking the section with 2 lb hard surface hammer. Small sliver of metal segregated from section edge striking him on left side of abdomen & embedding slightly under skin. EE was seen by physicain (lanced & sutured) & released to full duty.
Employee cut left thumb while slicing summer sausage for his lunch.
Employee was stripping CB antenna wire with a razor knife; the knife slipped and employee received a laceration in the palm of the right hand. He received 6 sutures to close the laceration.
EE was raking loose material under the belt into piles for shoveling. A shovel was leaning against the wall behind him. The shovel was bumped causing it to fall under the belt and thrown back against the wall. The shove handle broke and a piece of the handle struck EE on the left index finger resulting in a separation fracture at the knuckle joint.

EE STEPPED ON A RAKE LEANING AGAINST A SUPPORT POST. WHEN THE HANDLE OF THE RAKE FLIPPED UP, EE THREW RIGHT HAND UP TO PROTECT FACE. THE HANDLE STRUCK HAND RESULTING IN A CONTUSION AND BRUISE TO THUMB.
EE WAS TRIMMING EXCESS WASTE OFF TRANSFER ELEVATOR BREAKOVER ROLLERS WITH POCKET KNIFE. THE KNIFE SLIPPED AND PENETRATED HIS LEFT HAND BELOW THUMB. HE WAS WEARING LEATHER GLOVES AT THE TIME.
EE WAS ATTEMPTING TO DRIVE A BEARING OFF A 4 3/8" DIAMETER SHAFT USING A 3/16 HAMMER AND A PIECE OF KEYSTOCK. HE MISSED THE KEYSTOCK AND STRUCK HIS HAND WITH THE HAMMER. EE REC. 3 STITCHES AND WAS RELEASED TO WORK.
EE WAS CUTTING A MOUNTING HOLE IN RUBBER CHUTE LINER WITH UTILITY KNIFE. KNIFE SLIPPED AND HE CUT HIS LEFT INDEX FINGER WHERE IT JOINS AT HAND.
WHILE TIGHTEN BOLT THE WRENCH SLIPPED OFF CAUSING A LACERATION TO RIGHT FOREARM.
