Mostrando los 38Employee was removing zip-ties off equipment with a pocket knife and cut his finger. The employee was given first aid and released to return to work. On June 1, 2015 his finger began to draw up. He was taken to the Dr. and surgery was required to repair cut tendon
Employee cut her thumb using a hook blade to splice some cable
Employee was working off a 4' ladder tightening & replacing a hydraulic line with a 1" 7/16 wrench. Right hand was being used to tighten the line while left hand was being used to support position. Employee states that as he was tightening the line he felt a twitch in left shoulder & heard a pop, he then climbed down ladder & realized his L shoulder was dislocated.
EE was removing grating clips when a piece struck him in the mouth causing lip laceration requiring suture.
Employee was lifting LN Welding machine up with rope from 1st level of scaffolding. Welder got hung up on scaffolding, he pushed welder away while pulling up on rope & felt pain in lower back
EE was using a pry bar to help guide a 12" header pipe, it slipped and hit him in the left leg. He was transported to ER they took X-ray's and nothing was broken. On 10/14/2013 he called and needed to see an authorized physician. Possible small fracture to foot. Needs some physical therapy.
While installing a Crusher sheave taper lock bushing, hitting the hammer and it "bounced" back and fracturing his nose.
EE states that 2 weeks prior that she is having pain in her elbow from mopping, sweeping, and lifting. Sent to medical provider, and she is receiving physical therapy with no restrictions at this time.
EE was cutting chain link, material slipped causing hands to come together. Hit rt middle finger again, hit this finger on 10/25/12 He had to go to the dr to have it drained on 10/31/12
An equipment operator using his pocket knife to remove a tie-wrap holding a lamp on his hard hat when the knife slipped and lacerated his left thumb. He received 4 stitches at Pikes Peak Hospital Emergency Room.
EE was attaching chain binders onto pipe tub, with forward momentum his hand slipped and binder was not fully locked so binder bounced back and the handle of the binder struck the EE on the forehead above the left eye.

While attempting to twist in a rod the break out wrench slipped pinching the ring finger of his left hand causing a laceration and a fractrure. Placed on light duty for two weeks.
During process of pulling drill pipe out of drill hole, and while attempting to break two pieces of drill steel apart, the wrench slipped off the pipe causing a contusion to the left hand. Employee was placed on light duty.
Tightening a bolt w/ a 3/4" rachet. Finger was pinched between the rachet handle and a sharp bolt
Employee was attemting to remove a drain plug in a grear box when the plug came loose. The employee smashed his middle finger on his right hand between the wrench and the fame. diagnosed with a fracture of the finger. Placed on light duty for 1 week.
EE was attenpting to remove a stuck shovel tooth pin with a double jack. A piece of the pin flew off and went under the sleeve of his shirt and coveralls. His arm was lacerated and required suturing. The piece of steel was not imbedded in his arm.
EE was sweeping when a dust particle entered his eye. He was wearing safety glasses.
At approximately 2:30 pm EE and EE were working on a u-joint from a pickup truck. He was using a hammer and punch to attempt to remove the u-joint from the drive line when he missed the punch and struck his left thumb with the hammer resulting in a fracture and laceration of the thumb.
Breaking tight connection of drill pipe with a slot wrench braced against the drill tower. When the connection was struck with a hammer the connection came loose causing the slot wrench to come off the pipe stricking the employee above the right eye resulting in lacerations above the eye and a fractured eye socket.
A driller was unscrewing a drill hammer when the hammer unexpectedly fell and pinched the middle finger of his right hand resulting in a laceration to his finger tip. The laceration was sutured and EE returned to full duty.

EMPLOYEE WAS ATTEMPTING TO REMOVE A PIECE OF WOOD STUCK IN A CHUTE WITH A TIMBER HOOK WHEN THE FIBERGALL HANDLE OF THE HOOK STRUCK HIM IN THE CHEST. THE EMPLOYEE WAS EXAMINED BY THE AUTHORIZED PROVIDER. MEDS WERE PRESCRIBED AND HE WAS RELEASED TO RESTRICTED DUTY.
EMPLOYEE WAS CUTTING LINER MATERIAL - MISUSED UTILITY KNIFE, KNIFE CUT THROUGH MATERIAL AND CAUGHT BACK SIDE OF LEFT THUMB - REQUIRED 5 (FIVE) STITCHES.

ANTIBACTERIAL EYE DROPS ARE PRESCRIBED AND ADMINISTERED. HE WAS EXAMINED BY THE CO DR THE FOLLOWING MORNING. NO ADDITIONAL TREATMENT WAS PERFORMED AND HE WAS RELEASED FROM CARE OF MMI. HE WAS
WHILE WRENCHING ON BORE WELDING MACHINE, WRENCH SLIPPED AND CUT FINGER.
EMPLOYEE WAS PULLING AN OIL SAMPLE FROM A CAT 777, WHILE TIGHTENING FILL PLUG RACHET GEAR STRIPPED OUT, AND THE WRENCH HIT HIM IN THE MOUTH.
EMPLOYEE WAS CUTTING OPEN AND EMPTYING SANDBAGS.KNIFE CUT THROUGH BAG AND CUT THE PALM OF LEFT HAND WOUND REQUIRED 5 STITCHES.
EE WAS CUTTING A PIECE OF TAPE ON A DOOR THAT HE WAS GETTING READY TO PAINT WHEN HE HEARD A LOUD NOISE TO HIS RIGHT THAT MADE HIM JUMP AND THAT'S WHEN HE CUT HIS RIGHT MIDDLE FINGER ABOUT THE 1ST JOINT AND RECEIVED 4 STITCHES.
EE WAS POUNDING OUT A LOWER PIN ON THE SIMON FORMS WHEN IT POPPED OUT & HIT HIM IN THE LOWER LIP.
EE COMPLAINED OF PAIN IN HIS RIGHT WRIST WHILE MANUALLY PULVERIZING. HE WAS EXAMINED BY THE COMPANY DOCTOR AND RELEASED TO WORK. PHYSICAL THERAPY WAS PRESCRIBED.
EE WAS CUTTING A PIECE OF STRING WITH HIS POCKET KNIFE AND CUT LEFT INDEX FINGER ON THE KNBUCKLE EE REQUIRED THREE STITCHES.
THE PART WOULD NOT FIT IN THE SPACE DUE TO CLOSE TOLERANCE. A SLEDGE HAMMER WAS USED TO DRIVE IT INTO IT'S SPACE. THE GRID BOX WAS BEING MODIFIED TO ACCEPT GRIDS AND A BRACKET WAS DRIVEN INTO PLACE WITH A SLEDGE HAMMER. LEFT PAD ON INDEX FINGER WAS PINCHED OFF.
THE EE WAS ATTEMPTING TO BREAK A DRILL STEEL FROM THE ROTARY HEAD. THE JAWS ONT HE DECK WRENCH WERE EIGTHER WORN OR SPRUNG AND COULD NOT GRIP THE FLATS ON THE DRILL STEEL. THE EE INSERTED A CH IPPING HAMMER BETWEEN THE DRILL STEEL AND A JAW OF THE WRENCH TO IMPROVE PURCHASE. THEN THE ROTARY HEAD WAS TURNED ON. THE CHIPPING HAMMER WAS EJECTED AND STRUCK THE EE IN THE RIGHT FOREARM.
EE WAS CUTTING A BOLT WITH A HACKSAW. ONE END OF BOLT WAS HELD IN A VISE & THE OTHER END WAS BEING HELD BY HAND. THE HACKSAW SLIPPED OFF THE BOLT AS IT WAS BEING CUT & LACERATED HIS LEFT IN DEX FINGER. THE WOUND WAS STITCHED & HE WAS RELEASED TO FULL DUTY
