The miner was working on removing a bearing from a motor mount assembly. A small pry bar was being used to remove the bearing. The pry bar was under load when the miner struck the shaft with a hammer. The pry bar released and struck the miner in right cheek near the nose. The impact caused a laceration which required stitches and CT scans.

While striking the metal, the hammer missed its target and the miner's right index finger was struck by the hammer. The miner was wearing gloves and all other required PPE at the time. The strike made a laceration in the finger that required 5 stitches at the local emergency room.
EE was using a utility knife to trim a hole in a piece of rubber belting. The EE was attempting to widen the hole to hang the rubber, when the knife slipped, cutting the EE's hand causing a laceration to their left thumb.
EE and crusher foreman were doing maintenance on crusher CR-27. They were changing blow bars on the crusher head when the wrench that EE was holding slipped off the bolt and EE pinched the tip of finger, cutting the finger. EE was wearing gloves. EE was taken to emergency room for stitches. EE is on restricted duty and will follow up with doctor.
Employee doing crusher maintenance work, fabricating rubber belt cover when he took a box cutter knife to cut rubber, kneeling on 3'x4' rubber knife slipped and he cut his leg. Supervisor took him to clinic where they cleaned wound and applied sutures. When back to work with no restrictions.
Operator changing teeth on loader bucket. Hammer glanced off the segment striking the edge of the tooth. Teeth fragments came off, hitting him in the left forearm. This produced several lacerations, and foreign body retention. Taken to hospital to remove the metalic fragment, and suture of lacerations. Was returned to his normal duties the same day.
