While removing return rollers and brackets, the employee had a clamp on the belt and the clamp came off the belt striking the employee in the elbow.
EE was cutting old v-belts off of the wash plant screen. EE was holding the knife in the right hand and applying pressure to the blade with the left hand. When the knife cut through, EE slipped and cut their left palm and pinky finger resulting in 15 stitches.
Employee was cutting flashing in the Jaw, so they could drop it down on the conveyor belt. Employee slipped with the knife EE was using a cut EE's leg.
Two employees were changing blow bars. As they started IE had the tool in place but wasn't quite ready. EE noticed the Tool was in place and swung the hammer striking the handle of the tool smashing IE's finger between the tool and the Rotor head. The tool used to complete the task was engineered to eliminate injury, but it takes 1 person to place tool and 1 to strike
A belt ripped on an 8x20 screen causing mud to fall through and build up on the tail Polly stopping the conveyor. LOTO was performed. EE started chipping mud off the Polly using a 30-inch pry bar to knock the mud away. EE used EE's right hand to brace self against the frame of the belt and used EE's left to chip. As EE was chipping, EE missed the Polly and hit EE's right thumb.
We had a breaker trip on our conveyor belt and had to check the motor wiring. EE was removing the putty tape and electrical tape form the wire lugs. EE was using a knife to cut the lugs free and slipped off cutting into EE's left hand EE was holding the cord with. EE was using a safety knife and had leather gloves on.
While cutting a zip tie off of a hydraulic hose on the cone, EE was cutting overhead and on a downward fashion with a pocket knife and cut self between the thumb and forefinger in the thumb web. Requiring 7 stitches.
Employee was adjusting the jaw wedge plates and couldn't get the nut loose with cordless impact. Employee decided to use an open ended wrench and got the nut to come loose, as it broke free elbow struck the corner of the jaw and cut elbow open. Employee received stitches and a prescription for an antibiotic.
Employee was inspecting and installing a toggle bolt spring. While tightening the bolt, the spring broke causing the spring cover to hit the employee in the forehead above his left eye causing a laceration requiring sutures.
EE was hauling dirt. While in the yard he noticed a large rock between two tires on his trailer. He grabbed his hammer and started hitting the rock to get it out of there. Hit it a couple of times and then both tires blew while he was hitting at the rock. After the tires blew, the loader operator looked and noticed him on the ground. He called his manager to call 911.
Employee was helping change the teeth on the 988 loader when he hit the tooth with the hammer. A small sliver broke off the tooth and struck his leg.
Employee cut his left pinky finger while removing a flexco out of the screen feed conveyer with a utility knife.
