Employee was struck in the right hand by a piece of belt structure while replacing another piece of damaged belt structure along the 8 Haulage #1 Conveyor Belt. The employee was removed from work on 06/18/2018 by EE's doctor.
Ee was installing front drive line axle on 988 machine. Used a wobble socket to tighten bolts on the yoke, socket slipped and struck ee in mouth, causing laceration to upper lip and fractured 2 front teeth.
Employee was using a pole ax to loosen track ties from the rail at the end of 9M track. The employee started to feel a pain in lower back while bending over and striking the track ties with the pole ax.
Employee was removing a rock from the top of a continuous miner using a slate bar when employee's hand was caught between the slate bar and the continuous miner causing a laceration to employee's right little finger.
Employee was cleaning on the slope belt when struck in the left hand by a rock causing a laceration to the left ring finger.

Employee was using a slate bar to pry when the slate bar slipped and caused the employee to step backwards onto rock and gob causing injury to left foot.
While using a utility knife to make a splice on a shuttle car, the utility knife slipped off of the cable and struck the employees left index finger causing a laceration.

Employee was adjusting the drive belts on unit 98-A pump. He was using a pipe wrench to tighten the motor adjustment bolt on the drive side. The pipe wrench slipped catching fingers between the wrench and drive belt guard causing a hairline fracture to the left middle finger.
Injured was pulling a rock 3.5"X15"x6". As he was pulling the rock with a slate bar, another rock 1'x4'x2' on top of it fell sliding down the slate bar striking injured on top of right wrist. He was transported outside. He was treated on site by PA and received 5 stitches.
Injured was making a cable splice on the roof bolter when a rock fell from the 15' roof 8"x6"x2" to 0" striking him on the right shoulder driving the knife into of his right leg. He was transported to the hospital where he received 5 stitches in his right leg and contusions to right shoulder area.
Employee was removing a miner bit from a continuous miner and got his right finger caught in-between the miner bit he was extracting and the tool he was using to perform the work.
Employee was scaling to top when the rock he was scaling struck the slate bar he was using, and the slate bar struck the employee in the right thigh and knee.
EE, along with another EE, were pulling slack on cable and completed task and moved on to putting wrench on 1" pipe when pump broke free from pipe and spool struck EE on back shoulder & elbow on right side.
While attempting to place a jack under a de-railed mantrip, the employee heard a pop in his right shoulder.
Employees were removing a shaft from a Scoop gear box, and using a hammer to assist in relieving pressure due to the shaft being in a bind. While striking the shaft with a hammer, a piece of spline struck the injured employee in the left forearm.
Employee was on his knees, working on the track, when the employee behind the injured employee was using a slate bar and the slate bar slipped and struck the injured employee in the back of the head. The employee reported to work as regularly scheduled on 10/8/2013. On 10/14/2013 the employee was placed on restricted duty and was released to full duty on 10/23/2013.
Employee was removing a speed reducer with a hammer and a piece of metal peeled off and imbedded in the employees left hand.

The injured employee was holding a "punch" with a pair of channel locks, to knock out a pin on a take-up jack. Another employee with a hammer was striking the pin. The employee with the hammer missed the punch and struck the injured employee in the right thumb. Both employees were wearing approved eye and hand protection.
EE was retracking a motor with a lifting jack when he felt a tightness in his abdominal area. Later, he was turning a trolley pole when he felt a sharp pain in his lower abdominal area. This employee did not miss any work until 3/19/2012.
EE was leaning on a wrench to break a bolt loose. The bolt broke loose suddenly, jarring his wrist.
Employee was trying to fix plastic guarding around a drive at a junction. He had just changed his knife blade and was attempting to cut a zip tie while he was holding the guard. The knife cut through the zip tie and created an incision in his left wrist which required stitches.
Employee was trimming off a piece of rubber on a conveyor pulley. The knife slipped and he made a small cut in his leg that required 3 stitches.
Employee was building a stopping and attempted to use a sledgehammer to put a top block in. The resulting impact caused 8 blocks to fall out and hit him on the left shoulder. The stopping was approximately 76" high. He was taken to Welch Emergency. Confirmation of Diagnosis pending.
Employee was lifting belt structure with comealongs, when the hook on the comealongs slipped out of the clevice. When the belt structure fell, it shattered his fingertip bone and he received 12 stitches at Welch Hospital.
Injured was getting ready to make belt splice for straight edging. The knife slipped cutting his left thumb. Area was muddy and wet and the belt was very wet causing the knife to slip. Injured was required to get stitches for the cut.
EE was tigtening bolts with a cresent spud wrench, the wrench slipped off of the bolt and hit the left side of EE's chin causing a small laceration. EE completed shift and RTW the next day.
Employee was helping make a belt splice when he jerked on the splicing tool and felt pain in the middle of his back. He continue to work, the first day he missed work was 12/14/2010.
Employee was using a pair of channel locks to help other employees pull on a piece of 60" belt. The employee stated he felt a sharp pain in his right forearm. The employee did not report this to management until the following day. He has not missed any work and his first day of restricted duty was 12/7/2010.
Employee lacerated his left middle finger with a razor knife while attempting to replace a piece of water line. The laceration required 14 stitches
Injured was holding a special tool to drive a bearing into place. Foreman was striking tool with a sledge hammer. He missed striking injured on right thumb. Causing a laceration to right thumb. Injured was transported to hospital.
Injured was holding a roof bolt while his co worker hit it with a sledge hammer to drive a pin out of the miner. Co worker hit the pin with a glancing blow striking injured on the right forearm and wrist. Injured was transported to the hospital where he was treated for contusions and pain to right forearm and wrist.
Injured was using a miner bit to drive a broken miner bit from the bit lug when he struck his left index finger with a ball pien hammer. He was transported to the hospital where he was dianosed with a fracture and laceration to the left index finger.
Injured was driving miner bits with a hammer when the bit burst and a piece embeded into his left hand above the thumb. He was sent to the hospital where they sent him to a specialist to remove the metal.
Injured was shoveling belt line when he felt something give in his right shoulder. He stated that he had previously hurt the shoulder. He was transported to Welch Emergency Hospitol.
Employee was lifting a vibrator with a set of come alongs. The come alongs slipped and the injured felt pain in his neck and shoulder.
Injured was pulling loose roof with a slate bar when he felt pain in his groin area.
Injured was using a set of 3/4" come alongs to slew a wrecked jeep over to the rails when the roof bolt plate they were attached to pulled out causing the chain hoist to strike injured in the back of the head and neck. Injured was transported to the hospital where he was diagnozed with a whiplash of the neck area.
Had a set of come-alongs hooked to dolly on the belt takup. Dolly was picked up. The hook broke allowing the come-alongs to strike the injured on the right leg and thigh. He was taken to Wlch Emergency Hospital where he was diagnosed with a deep bruise. Treatment is several days off his feet.
Injured was repairing a belt splice at #5 belthead. He was using come alongs to pull the belt. The clamp came loose striking him in the mouth. He went to Welch emg. He hand a broken tooth.
Injured was removing a metal strap from a bundle of 16' straps. Using a mason hammer, When the metal strap came free his left forearm got into another strap that had been cut. This lacerated his left forearm. He had to have 5 stitches interally. Looking at a different type strap.
Injured was using a utility knife to trim the plastic from a knee pad that was irritating his knee. The knife slipped, lacerating the anterior side of the right palm. He was transported to a medical facility where the wound took 16 stictches to close the wound. Employees have been contacted to always cut away from the body.
