Mostrando los 9Wrench slipped on nut and contacted operator's tooth resulting in a chip. On 7/14 employee had appointment requiring dental repair thus making it a recordable injury.
Operator was scooping material out of Supersack when operator felt a strain in thumb. Continued to work, and completed shift. Next day, 1hr into shift, thumb continued to be irritated. Completed shift. Sent to doctor, and was given restrictions. The restrictions caused this to be a recordable. This was not a Lost Work Time Incident.
Mechanic was working on a Ballformer. While one mechanic was lying on the ground making adjustments to the roller bearing bolts, the injured mechanic was using a track bar to leverage the support roller bearing into the proper position. The track bar slipped off the roller bearing striking the mechanic on the ground just above his right eye causing a laceration.
Employee was attempting to turn valve on Back Pressure Control, when he felt pain in his Left shoulder. Employee was using wrench and cheater bar. Employee quit upon feeling pain. Did not seek medical attention until 5/14/2007.
While adjusting #3 Pannivis Filter Head Pulley, employee was tightening the head bolts when he felt burning in lower back. Employee had been working on equipment for approximately 30 minutes, loosening and tighting bolts in an effort to align the Head Pulley.
Held open pocket knife in right hand while opening bearing box, cardboard tore allowing hands to slip. Left thumb made contact with open kife blade, resulting in 4 stitches to repair wound.
Employee suffered a lumbar area muscle strain while using wrench to tighten and loosen 3/4 nuts and bolts on #13 Precepitator Pump. He stated that he felt pain after completing task and self treated with ibruprophen and delayed reporting injury until 8/7/2006. He was sent to doctor and taken off work for one week.
Operator was swapping tanks at the Bldg. 45 Tank Farm. He had just closed #67 and had opened #71 and had stepped down from platform. He took a step on the ground and felt a sharp pain in his right lower lumbar area. Operators use 8 lb. sledge hammer to strike locken bar to open and close valves.
