EE was loosening a drain elbow on the HP3 cone hydraulic tank. The pipe wrench EE was using slipped from the plug and EE hit their hand on adjacent beam.

Mostrando los 6Employee was moving the hinge portion of the chute attached to the Simplicity shaker for the secondary plant. The amount of force used to pry the hinge portion of the chute caused the blade portion of the telehandler to jump when the piece was dislodged. The blade from the telehandler landed on the Employee's left index finger causing laceration.
Miner was attempting to remove a tooth from excavator bucket with hammer when a small shard of metal came off of bucket and struck miner in the thigh.
EE was using a piece of metal/pipe to clear an aggregate blockage at the Crushing Plant Chute. When the blockage began to clear, pieces of loose aggregate fell on ee right hand. The falling aggregates caused two small cuts, a swollen middle finger and some numbness in the pinky finger.
EE was using a chain come along to tighten/guide chain holding tubing for Cone Crusher. Chain lost tension snapped hitting EE on the right side of face.
Employee was hammering a tooth on an excavator bucket when a piece of metal broke off and became airborn, striking employee resulting in a minor laceration to the abdomen approximately 3/8" in length
