Section foreman was helping repair a damaged shuttle car cable. The belt knife blade slipped out of the cable and cut through EE's glove and resulted in a laceration to EE's left hand that was holding the cable. EE received 5 stitches.
Injured was swinging a pipe to remove a roof jack support for the header during reclamation and hurt right shoulder. EE had been treated for the illness since 2006 and was supposed to have surgery on it in January 2016 and injured elected not to have the surgery to treat condition. Injured was repairing a damaged miner cable. He was cutting the outer jacket when then belt knife slipped and cut his inner left knee. He received a laceration that required 7 staples.
Injured cut right hand thumb with a belt knife while cutting a piece of suction hose for a Fletcher roof bolter. He received 5 stitches.
The injured was taking apart a water line fitting with 2 pipe wrenches on 2/06/2015. He felt a pain in his shoulder. He reported accident on 2/10/2015. He had not missed work and continued to do his normal job but required surgery on his right shoulder on 5/29/2015. He has not been released to come back to work since surgery on 5/29/2015.
