Employee was using a rock bar to pry loose hardened material buildup. When the material broke free the bar slipped from the employee's hand and bounced off of an adjacent handrail. When bar bounced back up it hit employee in the chin causing a cut that required stitches.

EE's were installing the manhead (door) in a mill with the use of an overhead crane. As the door was raised in to position it got hung up overloading the rigging breaking the come along, striking and lacerating the EE's forehead
Employee was using sledge hammer to beat pins. Six days later employee's arm was still sore and the employee sought medical treatment.
