Injured employee was using a knife cutting conveyor belt in two for removal. Employee was cutting toward self when the knife slipped from the belt and across employee's upper right leg causing a laceration requiring 13 sutures to close. No mining equipment was involved and employee has training and experience.

EE was assisting with splicing belt at the 7 North Belt Drive. EE was using a pair of pliers to pull on the belt. As they were pulling, the pliers slipped from the belt and they lost balance and fell backward, hitting an elbow against a belt cutting tool causing a contusion, pain & swelling in the elbow. No regulations were violated and EE is trained & was wearing required PPE.

Injured employee was cutting a small zip tie from a heat detection wire over a set of batteries that were being sealed. When the knife cut through the zip tie, the EE failed to control the knife and it continued up and into EE's left hand causing a cut requiring 3 sutures. EE did not violate any regulations and is experienced. Was wearing appropriate PPE.

Employee was installing bolts on the 4-E longwall recovery face. As EE was tightening a bolt using a breast drill, the machine jerked unexpectedly pulling on EE's left arm. Had pain and soreness in EE's shoulder. EE continued to work until 1-21-19 when EE sought medical treatment and began missing work.

The injured employee was using a "pick" hammer to cut the band around a bundle of wooden wedges. As EE was striking the wedges with the point end of the hammer in an attempt to break the band, the point of the hammer struck EE's right thumb causing a laceration requiring 5 sutures. Employee did not violate any laws and is properly trained.

Employee was replacing an ATRS jack on a continuous miner. EE was using a chain hoist to position the replacement jack into position. The chain slipped causing the replacement jack to slip and pivot striking the employee's right hand causing a laceration to palm. Employee didn't violate any rules or regulations and has adequate training and experience. Was wearing required PPE.

Employee was assisting in the installation of track rails on the 4-E section. EE was pulling on a joint of rail with a drag bar and EE's left arm gave out. Employee has continued to work until 5/21/18 when EE began missing work due to this injury. Employee overexerted EE's left arm pulling too much weight. EE didn't violate any rules or regulations.

Injured employee was using a track wrench tightening track bolts when EE felt pain in lower back. Employee continued to work until 02/13/2018 when EE began missing due to this incident. Employee didn't violate any rules or regulations, has proper training and was wearing required PPE. No mining equipment was involved.

Employee assisting in repairs to longwall drive sprocket. While using a large slide hammer they smashed the left ring finger causing a small fracture to the tip of the finger. Mining equipment was not involved in this accident. Employee didn't violate any rules or regulations, has proper training, and was wearing appropriate PPE.

Employee using a lifting jack to lower track rails while correcting a track violation. While letting the jack down to lower a rail, the jack lifting bar became stuck in rib screen. When EE freed the bar it sprung up and struck EE on the left side of face causing a laceration & contusion. EE didn't violate regulations & is properly trained & was wearing all required PPE.

Employee using a slide hammer to drive pin out of a shearer ranging arm. Employee got left little finger caught between the hammer slide and driver causing a cut that required 9 sutures. Accident caused by the employee's lack of attention to off hand. Employee didn't violate any rules, policy, or laws. Employee has a positive attitude for safety and was wearing proper PPE.

Contract employee was adjusting the lap belt in storage unit with a come-a-long. The arm for the drop away rollers fell down pinching EE's finger between the support bracket and arm. Cause of this injury was not securing the bracket from falling. Compliance with rules/regs., mining equipment/systems not factor. Miner Proficiency and/or PPE (Gloves) may be a factor.

Employee was trimming the fingers on a splice when the knife ee was using slipped and contacted left forearm resulting in a laceration that required 11 stitches. Cause of this incident was awareness and using wrong type of knife. Compliance with company rules a factor, PPE not a factor at the time, miner attitude could be a factor, mining equip/systems not a factor.

As the employee was removing the bolts from a set of track connector plates, the rail suddenly moved and struck the employee's left ankle. The accident occurred because the employee failed to heed the warning of co-workers telling EE that EE was not in a safe position. The employee is a multiple injury person and doesn't appear to have a good safety attitude.

Employee was prying a form loose and struck self in ribs with prybar.

While loading cables into a car, the employee got EE's right ring finger squeezed between the hoses and a slate bar EE was using. The accident occurred because the employee was not paying attention to the task EE was preforming. EE was properly trained and has a good attitude. EE was complying with the rules, regulations and policies. EE had on EE's PPE.
