Contract employee was using a plastic wedge to help remove a sheave from a old mechanism on a screen in the preparation plant. EE was hitting the wedge with a 4-lb hammer, wedge came loose and struck employee in the chin. Cause of this injury was awareness. Compliance with rules/regs, PPE, mining systems was not a factor. Employee was in the process of changing a blade in a utility knife on the 5W section when employee's hand slipped and cut employee's right hand approximately 1 1/4" long resulting in sutures. Employee was in the process of tightening a pipe T with a pie wrench. The T slipped in vise and injuring the right index finger resulting in a fracture. Cause of injury was awareness. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.

Employee was trying to recover rail in the 1A spur. The rail was on top of another piece when EE tried to attach a chain the rail shifted striking EE's right hand causing three fractures. It didn't involve any compliance issues. No equipment involved. It didn't involve job skills, proficiency, training or attitude. No PPE or protective devices on equipment involved.

EE was jacking up 62 jeep on mainline near 2S switch EEs pushed jeep to get back on track when the jack bar hit EE on left side injuring left side ribs. 5 days later it was found EE's spleen was bleeding. EE didn't miss work until 2-20-17. It didn't involve job skills, compliance issues proficiency training attitude PPE protective devices No other part 50 criteria apply.

Employee was in process of aligning the 4E belt with come a long when roller shifted striking employee's left hand causing lacerations resulting in the employee receiving sutures. Employee was in the process of taking rail apart on the 3E section.Employee was removing last fish plate when they struck it with a rail hammer the rail sprung and EE was hit in the face with either rail,fishplate or hammer.It was found that the scoop that they were using was putting pressure on the rail. No other part 50 criteria applies.
