While re-installing a depth counter chain sprocket on drill 110, a 7/16" open end wrench slipped off of bolt striking ee in the mouth chipping the left front incisor.

Mostrando los 6Employee was cleaning a strainer basket using a metal pick tool. The tool slipped off the edge of the strainer basket and lacerated the third finger of the employee's left hand. Lighting was good and there were no other contributing environmental factors
Utility person was moving a shovel, the shovel cable horn flipped over jerking the rope the utility person had a hold of which caused a pop in the employees left elbow area. An MRI showed the bicep muscle was torn, on 08/29/2017 the employee was prescribed a brace, surgery is scheduled for approx. 09/05/17. The employee has not missed any scheduled shifts until after surgery.
Employee was opening a packaging container and cut the back of left hand.
Mechanic was installing the right tracks on a dozer, employee was using a bar to align the bolts when the bar slipped and pinched employee's finger on employee's right hand resulting in a laceration to employee's ring finger which required 4 stitches. The employee was released to return to work and did not miss any scheduled shifts.
Mechanic was adjusting a jack stand, EE put EE's middle finger in the adjustment hole to help lift the jack stand, the jack stand came loose and fell pinching EE's finger in the hole causing a laceration that required stitches. The mechanic was released to work as tolerated and returned to the mine to finish the shift.
